Negotiating multiple barriers: health workers' access to counselling, testing and treatment in Malawi.

Malawi is facing a severe HIV and AIDS epidemic with an estimated 12% of its population living with the virus. Health workers are on the front lines of the HIV epidemic and they face the risk of HIV infection in both their personal and professional lives. This mixed method study aimed to explore the enablers and barriers to HIV counselling and testing and antiretroviral therapy by health workers in Malawi. After qualitative data were collected through in-depth interviews with health workers in the Mchinji and Nsanje districts, a survey questionnaire was constructed and administered to 906 heal

Imperatives for Action and Impact of Humanitarian Crises Affecting Children: The Case of Haiti-Lessons and the Way Forward.

Haiti's ongoing humanitarian catastrophe has had a devastating impact on the Haitian health care sector that has yet to be fully quantified. The history of this small island nation in the Caribbean-the first, in 1804, to abolish slavery and obtain independence from France-has been tumultuous since its inception. A chaotic succession of coups d'état, dictatorial regimes, and international interventions and occupations, coupled with harmful economic and development policies from former colonial powers and allies, has led to political and economic fragility over the course of Haiti's maturation a

[A comparative analysis of different approaches to identifying cardiovascular diseases in coal miners during medical selection].

An expert evaluation of identifiability of cardiovascular diseases was carried out together with a clinical and functional examination of certain groups of miners of basic underground occupations at different ages and lengths of service, that showed a high incidence of cardiovascular diseases along with a low informative value of methodical approaches, indices and criteria used for their diagnosis in conducting preliminary and periodic health check-ups. To improve the quality of diagnosis of diseases of the circulatory system it is necessary that standardized methods of investigation should be

Persons of lesser values--moral argument and the "final solution".

For many persons, 'Holocaust-abomination' is a fixed point on their moral compass: if anything can be evil, it was. Yet at least one of the justifications deployed by its perpetrators (the eugenics argument) involves widely-held values concerning human health and procreation. Hence persons endorsing many current activities based on those values (e.g. genetic counselling) have been charged with being on a morally deplorable slippery slope. This paper sketches the necessary structure of a moral position capable of consistently embracing those values without placing its occupants on that slippery

Prognostic factors in labyrinthectomy.

Labyrinthectomy for unilateral Menière's disease may be complicated by persistent unsteadiness postoperatively. The aim of this study is to identify the unfavourable factors in recovery following labyrinthectomy. Twenty-one patients gave a detailed history and underwent neuro-otological, ophthalmic and cardiovascular examination, as well as psychological and psychiatric assessment. Sedentary occupations, external health locus of control, extraverted personality, and an anxious and/or depressed psychiatric state were associated with poor post-operative recovery. These factors therefore should b

"You can sleep when you die": How culture, race, gender, and migration shape sleep health among women of color in Utah.

BACKGROUND: Women of color face disproportionately high rates of cardiometabolic disease, partly due to inadequate sleep. Culturally tailored sleep health interventions are urgently needed to address these disparities. Community health workers as trusted community members offer valuable insight to inform intervention development. We sought to examine female community health worker perspectives on individual and sociocultural influences shape sleep health within African Immigrant/Refugee, Black/African American, Hispanic/Latinx(a), and Native Hawaiian or other Pacific Islander communities in Ut

'I'm Not Sure Who to Refer You to': Experiences of Clinicians Accessing Allied Health for Their Patients With Parkinson's Disease.

RATIONALE: Allied health interventions have been shown to improve impairments and quality of life in people with Parkinson's disease (PwPD). However, globally allied health is underutilised, and referrals tend to be reactive and occur in moderate to advanced disease. Currently little is known about the referral patterns of PwPD to allied health in Australia. AIMS: This study examined the allied health referral patterns of neurologists, general practice physicians (GP) and Parkinson's disease nurse specialists (PDNS) treating PwPD in New South Wales (NSW), Australia. METHODS: Four neurologist

The psychological impact of COVID-19 on 'hidden' frontline healthcare workers.

BACKGROUND: The novel coronavirus disease (COVID-19) pandemic has led to rising death tolls and stressed healthcare systems, resulting in an unprecedented psychological stress on healthcare workers worldwide. However, the majority of studies only accounted for frontline healthcare workers with direct patient exposure. AIM: This study aims to look at the psychological impact of COVID-19 in a specific, vulnerable and yet hidden group of healthcare workers, namely laboratory healthcare workers who are at high risk exposure to SARS-CoV-2 virus from handling infected patients' blood samples, in ad

Prevalence, incidence and risk factors of carpal tunnel syndrome in a large footwear factory.

The study was conducted to assess the prevalence and incidence of carpal tunnel syndrome (CTS) in a large modern footwear factory and to identify factors predictive of CTS. To this end, 199 workers were examined in 1996, and 162 of them were re-examined in 1997. Ergonomic and psychosocial risk factors of CTS were assessed by workpost analysis and self-administered questionnaire. The prevalence of CTS at baseline in 1996 and in 1997 was 16.6% (95%CI: 11.4-21.7) and 11.7% (95%CI: 6.7-16.8), respectively. The incidence rate of CTS in 1997 was 11.7% (95%CI: 6.7-7.8). The impact of U.S. policies to protect healthcare workers from bloodborne pathogens: the critical role of safety-engineered devices.

In the United States (U.S.), federal legislation requiring the use of safety-engineered sharp devices, along with an array of other protective measures, has played a critical role in reducing healthcare workers' (HCWs) risk of occupational exposure to bloodborne pathogens over the last 20 years. We present the history of U.S. regulatory and legislative actions regarding occupational blood exposures, and review evidence of the impact of these actions. In one large network of U.S. hospitals using the Exposure Prevention Information Network (EPINet) sharps injury surveillance program, overall inj
