Supraventricular tachycardia during the first year of life: is subclinical inflammation the trigger?

BACKGROUND: Neutrophil/lymphocyte ratio (NLR) and red cell distribution width (RDW) may be associated with the onset of arrhythmias in adults, thus underlining a possible inflammatory etiology. Paroxysmal supraventricular tachycardia (SVT) is the most frequent pathological tachycardia in childhood. AIM: To verify NLR and RDW levels in a group of children (<1 year) affected by SVT with a structurally normal heart and without fever or inflammatory diseases; to compare NLR and RDW before and after SVT resolution, to verify whether the latter was related with the reduction in inflammatory state;

Mechanical muscle and tendon properties of the plantar flexors are altered even in highly functional children with spastic cerebral palsy.

BACKGROUND: Recent ultrasound studies found increased passive muscle stiffness and no difference in tendon stiffness in highly impaired children and young adults with cerebral palsy. However, it is not known if muscle and tendon mechanical properties are already altered in highly functional children with cerebral palsy. Therefore, the purpose of this study was to compare the mechanical and material properties of the plantar flexors in highly functional children with cerebral palsy and typically developing children. METHODS: Besides strength measurements, ultrasonography was used to assess gas

Efficacy of two injection-site localisation techniques for botulinum toxin injections: a single-blind, crossover, randomised trial protocol among adults with hemiplegia due to stroke.

INTRODUCTION: Botulinum toxin injections are an effective treatment for limb spasticity following stroke. Different tracking techniques are used for this purpose: palpation, electrostimulation, electromyography and ultrasound. Yet very few studies have compared these different techniques, and none has successfully proved the superior efficacy of ultrasound-guided injections compared to another tracking method. The primary objective of our study was therefore to compare the efficacy of botulinum toxin injections depending on the tracking technique used: ultrasound versus electrostimulation. ME

Central line-associated bloodstream infections in Australian intensive care units: Time-trends in infection rates, etiology, and antimicrobial resistance using a comprehensive Victorian surveillance program, 2009-2013.

BACKGROUND: The epidemiology of central line-associated bloodstream infections (CLABSI) in Australian intensive care units (ICUs) has not previously been reported. We sought to describe time-trends in CLABSI rates, infections by ICU peer-groups, etiology, and antimicrobial susceptibility of pathogens in a large cohort of Australian ICUs for the period January 1, 2009-December 31, 2013. METHODS: Using National Healthcare Safety Network methods, CLABSI surveillance in adult patients was performed by hospitals participating in the Victorian Healthcare Associated Infection Surveillance System (n

Evaluation of the impact of abdominal obesity on glucose and lipid metabolism disorders in adults with Down syndrome.

We aimed to describe anthropometric differences in weight-related disorders between adults with Down syndrome (DS) and healthy controls, as well as their disparate impact on glucose and lipid metabolism disorders. We underwent a cross-sectional study of 49 consecutively selected, community-residing adults with DS and 49 healthy controls in an outpatient clinic of a tertiary care hospital in Madrid, Spain. Siblings of adults with DS were studied as controls in 42 cases. Epidemiological data (age and gender), anthropometric data (body mass index, waist circumference, and waist-to-height ratio [W

Prevalence and determinants of gambling disorder among older adults: a systematic review.

AIMS: This paper aims to systematically review studies on the prevalence of gambling disorder among older adults aged 60years and above and to summarize the evidence on the determinants, risk factors as well as the comorbidities associated with it. METHODS: A systematic search was conducted for peer-reviewed, journal articles between 1st January 1995 and 1st May 2013. Only original studies which were published in English, included participants older than 60years of age and provided prevalence figures specifically among them, and where participants were assessed to have a gambling disorder by

Lower exercise capacity in children with asymptomatic atrial septal defect associated with circulatory impairment.

The exercise capacity and limitation in children with asymptomatic atrial septal defect (ASD) have not been explored thoroughly. The aim of our study was to examine the influence of asymptomatic ASD on exercise capacity in children. Fifty children with asymptomatic ASD who had undergone medical interventions at least 4 years ago and fifty normal children were recruited in this study. The exercise capacity was assessed by the symptom-limited exercise test through the Bruce treadmill protocol. The pulmonary function was also evaluated by the spirometry. Circulatory and ventilatory impairments we

Atypical category processing and hemispheric asymmetries in high-functioning children with autism: revealed through high-density EEG mapping.

Behavioral evidence for an impaired ability to group objects based on similar physical or semantic properties in autism spectrum disorders (ASD) has been mixed. Here, we recorded brain activity from high-functioning children with ASD as they completed a visual-target detection task. We then assessed the extent to which object-based selective attention automatically generalized from targets to non-target exemplars from the same well-known object class (e.g., dogs). Our results provide clear electrophysiological evidence that children with ASD (N=17, aged 8-13 years) process the similarity betwe

Youth work service providers' attitudes towards computerized CBT for adolescents.

BACKGROUND: Attitudes of social service providers towards computerized CBT (cCBT) might affect use of cCBT by their clients and may provide important insights that should be considered in dissemination. There is no literature exploring the attitudes of providers of youth work services towards cCBT despite the likelihood of them having close relationships with young people at high risk of mental ill-health. METHOD: Focus groups and semi-structured interviews were undertaken with a total of 40 providers (21 youth workers and social service staff providing alternative schooling, justice or other

"Right here is an oasis": how "recovery communities" contribute to recovery for people with serious mental illnesses.

OBJECTIVE: "Creating Communities" is a study that examines the influence of stable housing on recovery within intentional communities of people living with severe mental illnesses in Washington, DC. We label these configurations "recovery communities" (RCs). The authors aim to identify features of the contextual environment of RCs that contribute to recovery from the perspective of RC residents. METHOD: Focus groups were conducted with RC residents at 4-month intervals to inquire into day-to-day life in the communities. Focus group transcripts were reviewed and thematic analysis was conducted

Effects of word frequency and phonological neighborhood characteristics on confrontation naming in children who stutter and normally fluent peers.

UNLABELLED: In a prior study (Newman & Bernstein Ratner, 2007), we examined the effects of word frequency and phonological neighborhood characteristics on confrontation naming latency, accuracy and fluency in adults who stutter and typically fluent speakers. A small difference in accuracy favoring fluent adults was noted, but no other patterns differentiated fluent speaker responses from those obtained from the adults who stutter. Because lexical organization or retrieval differences might be more easily observed in less mature language users, we replicated the experiment using 15 children who

Adolescent sexuality and sexual behavior.

PURPOSE OF REVIEW: Adolescence is a time of self-discovery and physical, as well as cognitive, development. It is within this context that adolescent sexual development and sexual behavior occur. While curiosity and experimentation are normal, sexual behaviors, both coital and non-coital, place adolescents at risk for undesired consequences including sexually transmitted disease acquisition and pregnancy. Trends in adolescent sexual behavior are changing, and health care professionals must be aware of these trends to provide necessary medical care and education to this population. RECENT FIND

Characterization of the bone mineral density of children with spinal cord injury.

The purpose of this study was to compare the bone mineral density in children with spinal cord injury (SCI) with age- and sex-matched controls in three anatomic areas of the proximal hip. In addition, post hoc analysis looked for differences in bone density between sub-groups considering several factors associated with spinal cord injury: the presence or absence of spasticity, the level of injury and the presence or absence of pathologic fractures. Fifty-one pediatric patients with spinal cord injury between the ages of 3 and 20 underwent bone density measurements using dual photon absorptiome

Stage dependent alterations in PBMC mitochondrial bioenergetics in pediatric obesity: from insulin resistance to type 2 diabetes.

BACKGROUND: Pediatric obesity is associated with early-onset cardiometabolic disorders related to metabolic syndrome (MetS), including insulin resistance (IR) and type 2 diabetes (T2DM). Mitochondrial dysfunction plays a key role in their pathogenesis, but evidence on mitochondrial health in children and adolescents with obesity remains limited. METHODS: In this cross-sectional study, mitochondrial oxygen consumption rates (OCR, pmol/min/3 × 105 cells) and ATP production (pmol ATP/min/3 × 105 cells) were assessed using extracellular flux analysis in peripheral blood mononuclear cells (PBMCs)

Hypercapnia at admission, regardless of acidosis, may worsen the outcome of hospitalised patients with chronic obstructive pulmonary disease exacerbations.

In patients with exacerbation of chronic obstructive pulmonary disease (ECOPD), the presence of respiratory acidosis is considered "a marker of severity", identifying the need for ventilation treatment. The evaluation of ECOPD patients with hypercapnia, even without acidosis, may provide information on patients not commonly defined as "at risk". We retrospectively assessed 407 hospitalised patients with ECOPD, divided into three groups at admission: patients with normocapnia (N = 176), hypercapnia (PaCO2 > 45 mmHg and pH ≥ 7.35, N = 126), and acidosis (PaCO2 > 45 mmHg and pH < 7.35, N = 105).
