Antibiotic choices in surgical intensive care unit patients.

Antibiotics have dramatically changed the care of the critically ill patient over the last 60 years. Patients with complex physiological conditions present with infectious processes requiring the effective use of antimicrobial drugs. In many situations, the inability to eradicate the infectious process is complicated by the progressive development of resistance among the causative organisms. Systemic antibiotic prophylaxis is warranted only for the prevention of wound infections. Regimens in these cases should use large doses of nontoxic antibiotics covering the spectrum of organisms likely to

[Written information from hospital to primary physician about discharged patients].

Discharge communications from hospitals to general practitioners in respect of 203 patients have been analyzed in two municipalities in Western Norway. The average interval between discharge from hospital and the first visit to the GP was 25 days (1-198 days). The mean period before arrival of the final report was 28 days (0-175 days). In 38% of the cases the GPs had received no written communication from the hospital upon first contact after discharge. 42% of the preliminary reports and 18% of the final reports were judged to be inadequate. At the first attendance, the GPs were uncertain abou

[Total cutaneous electron beam therapy of mycosis fungoides].

Electron beam irradiation of the entire skin surface was used to treat 25 patients with mycosis fungoides from 1977 to January 1988. A plexiglas screen was used to reduce the energy of the 8 MeV beam of a Sagittaire linear accelerator to 4 MeV. A total dose of 30 Gy was delivered in 12 fractions over days. This series includes 17 men and 8 women with a mean age of 44 years (range 13-78 years) and a mean follow-up of 34 months (range 6-92 months). The following-up staging system was used: stage A: superficial lesions covering less than 50 p. 100 of the body surface; stage B: superficial lesions

Heat and mechanical hyperalgesia induced by capsaicin. Cross modality threshold modulation in human C nociceptors.

Quantitative thermal and mechanical algometry was studied in 4 human subjects exposed to various concentrations of capsaicin administered topically to the skin of the palm or forearm. Treated skin patches were assessed for changes in heat pain threshold and in mechanical pain threshold at various controlled temperatures. The results showed that: (1) in addition to heat hyperalgesia, capsaicin consistently induces overt mechanical hyperalgesia; (2) thermal and mechanical hyperalgesias are linearly dependent on the log of capsaicin dose; (3) mechanical hyperalgesia is increased by increasing ski

Subcutaneous palisading granulomas (benign rheumatoid nodules) in children.

Subcutaneous palisading granulomas (SPG), also known as benign rheumatoid nodules, occur in otherwise healthy children. Although these lesions have the same histology as rheumatoid nodules in adults, subcutaneous granuloma annulare, and diabetic lipoidicum, they appear to represent a distinct clinicopathologic entity. We reviewed our experience with 17 cases of SPG. All patients presented with one or more painless, nonmobile, subcutaneous nodules. Lower extremity involvement was most common, but lesions presented in the upper extremities and scalp as well. There were 14 girls and three boys ra

Lidocaine inhibition of esophageal peristalsis and lower esophageal sphincter pressure in baboons.

Intravenous lidocaine was infused at 0.82 ml/min in a concentration of 1.2 mg/ml (2.3 mg/kg) for 120 min in awake chair-restrained baboons (Papio anubis), and measurements of esophageal peristalsis and LES pressure were compared before and after lidocaine or control infusions. Lidocaine produced a progressive and significant (P less than 0.05) decrease in amplitude in the peristaltic wave in the smooth muscle portion of the distal esophagus during the 120-min infusion. Lower esophageal sphincter pressure was similarly significantly lower than control after the infusion of lidocaine (P less tha

Effect of neurointensive care upon outcome following severe head injuries in childhood--a preliminary report.

From March 1978 till August 1981 neurointensive care was applied to 24 children with severe head injuries (Glasgow Coma Scale less than or equal to 7, mean 5 +/- 1) corresponding to 7% of a population with head injuries observed during the same period. The neurointensive care included continuous intracranial and arterial pressure monitoring and normalization of intracranial and cerebral perfusion pressure with intubation, hyperventilation, control of body temperature, dexamethason, barbiturates and osmotic agents. The follow-up time is 1.5 to 4.4 years (mean 2.5 years). The results obtained by

[Enhanced recovery after Caesarean delivery: a practice survey in two French regions].

AIMS: Although most components of an enhanced recovery programme (ERP) can be applied to caesarean delivery, it is unknown if their implementation is large in France. TYPE OF STUDY: Structured interview by telephone or e-mailing of an anaesthetist to describe usual perioperative practice in two French regions (Provence - Alpes - Côte d'Azur [PACA] and Île-de-France [IDF]). METHODS: Questionnaire related to scheduled caesarean delivery. RESULTS: Response rate 74% (111/149 maternity units). Multimodal analgesia was almost universally applied and intrathecal/epidural morphine used by 86% of re

RYR1-related diseases patient-led international research workshop: Novel perspectives, treatments and interventions.

RYR1-related diseases (RYR1-RDs) encompass a wide spectrum of disease manifestations with limited available disease-modifying therapies. The RYR1-RD phenotypes include congenital myopathies of different histotypes often with early-onset muscle weakness, feeding difficulties, and respiratory insufficiency of varying severity. RYR1-RDs also include a group of episodic diseases such as exercise intolerance, exertional rhabdomyolysis, periodic paralysis, and malignant hyperthermia susceptibility. The RYR-1 Foundation hosted the second patient-led international research workshop focused on RYR1-RDs

Neonatal apnea research: a bibliometric analysis of trends, clusters, and paradigm shifts.

BACKGROUND: The clinical paradigm for neonatal apnea has shifted profoundly, from managing acute cardiorespiratory events to preventing adverse long-term neurodevelopmental outcomes. This evolution has generated a vast and complex body of research, yet a systematic map of its historical trajectory, intellectual structure, and emerging frontiers is currently lacking. This study provides the first comprehensive bibliometric analysis of the global research landscape of neonatal apnea. METHODS: Publications from the Web of Science Core Collection up to December 31, 2025 were retrieved, with 2,517

Short-course vunakizumab associated with rapid improvement of acute flares in paediatric generalised pustular psoriasis: a three-patient case series.

BACKGROUND: Paediatric generalised pustular psoriasis (GPP) is rare, frequently relapsing and associated with limited evidence for targeted therapy. Vunakizumab is an anti-interleukin-17A monoclonal antibody, but data in paediatric GPP are scarce. METHODS: We retrospectively reviewed three boys with recurrent GPP who received short-course vunakizumab during acute flares. Clinical characteristics, diagnostic details, treatment timelines, follow-up outcomes and Generalized Pustular Psoriasis Area and Severity Index (GPPASI) scores were collected. RESULTS: A 13-year-old boy (87.5 kg) with ciclo

Clinical screening and peer interventions to prevent substance use in at-risk adolescents: A longitudinal single-arm trial protocol.

BACKGROUND: Mental health conditions and substance use disorders commonly co-occur and adolescence is a period of increased risk for both. Adolescents with mental health conditions often begin using substances earlier than those without. Early initiation increases the risk of later substance use disorder. We propose to measure the impact of a peer-developed, primary care-based intervention to prevent substance use initiation among adolescents with mental health diagnoses and to evaluate responses to the intervention. METHODS: This is a single-arm 12-month longitudinal unblinded pilot observat

Stigma and discrimination among children affected by leprosy: a systematic review of a skin-related neglected tropical disease.

Little is known about the stigma and discrimination children with leprosy are subject to and the subsequent impact on their psychological well-being. The aim of this study is to systematically review the literature for evidence that closes this knowledge gap and informs practice. In line with a PRISMA-P Protocol (PROSPERO CRD420251089128), we conducted a systematic review, underpinned by an extensive search strategy. There was no inclusion restriction on methodology, time period, country, or language. Data extraction was completed through a double-review process, which was guided by a deductiv

Effect of Boswellia serrata on Pain Intensity, Central and Peripheral Sensitization, and Pain Modulation in Healthy Volunteers-A Randomized, Double-Blind, Placebo-Controlled, Cross-Over Pilot Trial.

BACKGROUND: Boswellia serrata has traditionally been used in Ayurvedic medicine for its anti-inflammatory and antioxidant properties. Although several studies support clinical analgesic efficacy, the underlying mechanisms have not been investigated in human experimental pain models. This randomized, double-blind, placebo-controlled, crossover pilot trial aimed to examine the mode of action of Boswellia serrata to differentiate between its peripheral and central effects. This exploratory pilot study was designed to generate preliminary effect size estimates and assess functional pain-processing

High-dose-rate vaginal brachytherapy for treatment of recurrent high-grade vaginal intraepithelial neoplasia.

PURPOSE: High-grade vaginal intraepithelial neoplasia (HG VaIN) is a rare preinvasive lesion characterized by a high risk of recurrence. Management strategies encompass both surgical excision, ablative procedures, and topical treatments. Brachytherapy provides a less invasive option, particularly for recurrent HG VaIN cases where other therapies have been unsuccessful or are not feasible. The present study assessed effectiveness and safety of brachytherapy in treating HG VaIN, with a focus on long-term outcomes, including local disease control, progression to invasive carcinoma, and treatment-
