Prescriber-recorded indications for antipsychotic use in children and adolescents in Spain, 2015-2023: a population-based, real-world study.

The use of antipsychotics (APs) in children and adolescents has increased notably worldwide, often beyond approved indications. However, most real-world studies lack direct information on the clinical reasons for prescribing. In this population-based study conducted in the region of Valencia, Spain, we analysed 128,057 AP treatments, comprising 741,213 prescriptions, issued to 30,383 individuals aged 0-19 years between 2015 and 2023, using real-world data that uniquely includes prescriber-recorded indications. We described the distribution and temporal evolution of the number of AP treatments

Heartbeat evoked potentials in patients with post-traumatic stress disorder: an unaltered neurobiological regulation system?

BACKGROUND: Early life maltreatment is a risk factor for psychiatric disorders, including post-traumatic stress disorder (PTSD). Post-traumatic stress disorder is a severe and heterogeneous disorder with fluctuating states of emotional over- and undermodulation, including hypervigilance, dissociation, and emotion regulation deficits. The perception and regulation of emotions have been linked to interoception, the cortical representation and sensing of inner bodily processes. Although first therapeutic approaches targeting bodily sensations have been found effective in patients with PTSD, and d

How do prescription opioid users differ from users of heroin or other drugs in psychopathology: results from the National Epidemiologic Survey on Alcohol and Related Conditions.

OBJECTIVES: To study substance use and psychiatric disorders among prescription opioid users, heroin users, and non-opioid drug users in a national sample of adults. METHODS: Analyses of data from the 2001-2002 National Epidemiologic Survey on Alcohol and Related Conditions (N=43,093). RESULTS: Four groups were identified among 9140 illicit or non-prescribed drug users: heroin-other opioid users (1.0%; used heroin and other opioids), other opioid-only users (19.8%; used other opioids but never heroin), heroin-only users (0.5%; used heroin but never other opioids), and non-opioid drug users (

Clinical characteristics of obsessive-compulsive disorder comorbid with obsessive -compulsive personality disorder: subtype implications.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a psychiatric condition characterized by persistent obsessions and compulsions that lead to significant distress and functional impairment. Obsessive-compulsive personality disorder (OCPD) frequently co-occurs in individuals with OCD and has been identified in several studies as the most prevalent personality disorder within this population. However, the clinical relevance of this comorbidity remains uncertain, particularly in terms of whether it reflects a distinct OCD subtype characterized by specific onset patterns and symptom dimensions. What we have learned from the Methadone Maintenance Treatment of Dual Disorder Heroin Use Disorder patients.

Mental Disorders and Heroin Use Disorder (HUD) often co-occur and constitute correlated risk factors that the authors believe are best considered from a unitary perspective. In this article we review and discuss data collected by the V.P. Dole Research Group in Dual Disorder (V.P. Dole DD-RG) patients according to the following six discussion points: (1) Treatment of personality disorders during Methadone Maintenance Treatment (MMT); (2) Treatment of Mood Disorders during MMT; (3) Treatment of Anxiety Disorders during MMT; (4) Treatment of Psychotic Disorders during MMT; (5) Treatment of viole

Psychometrics of the Personality Inventory for the Diagnostic and Statistical Manual of Mental Disorders to measure the international classification of diseases trait model: A meta-analysis.

Although the Personality Inventory for Diagnostic and Statistical Manual of Mental Disorders (5th ed.; PID-5) was initially designed to assess the Diagnostic and Statistical Manual of Mental Disorders (5th ed.) trait model, a modified scoring algorithm was developed to evaluate the International Statistical Classification of Diseases and Related Health Problems (11th ed.; ICD-11) trait model constructs. This meta-analysis aimed to examine the internal (structural and discriminant types) and external (convergent and criterion types) validity of the PID-5 to measure the ICD-11 trait model constr

Accelerated epigenetic aging in women with emotionally unstable personality disorder and a history of suicide attempts.

Emotional unstable personality disorder (EUPD; previously borderline personality disorder, BPD) is associated with excess natural-cause mortality, comorbid medical conditions, poor health habits and stress related epigenomic alterations. Previous studies demonstrated that GrimAge - a state-of-the-art epigenetic age (EA) estimator - strongly predicts mortality risk and physiological dysregulation. Herein, we utilize the GrimAge algorithm to investigate whether women with EUPD and a history of recent suicide attempts exhibit EA acceleration (EAA) in comparison to healthy controls. Threading the Needle: Practical Considerations for Merging Theory-Driven Computational Psychiatry With Data-Driven Analytics to Enhance Precision Health at Scale.

The rapidly evolving field of computational psychiatry enables quantification of specific cognitive processes and their underlying mechanisms in a translational and potentially scalable manner, using a combination of data collection via mechanistically informed behavioral tasks and theory-driven mathematical modeling. In parallel, transdiagnostic, dimensional approaches to psychiatric diagnostics, such as the Research Domain Criteria and Hierarchical Taxonomy of Psychopathology, seek to facilitate links between clinical research and real-world clinical reality, which rarely respects traditiona

Survey of local patterns of fentanyl exposure and harm reduction strategies in the Southeastern United States.

INTRODUCTION: Fentanyl, a synthetic opioid, is frequently found as a contaminant in illicit substances, significantly increasing overdose risk. A growing trend toward intentional fentanyl use further complicates the opioid crisis. This study aimed to identify patterns of local fentanyl use and contamination and evaluate harm-reduction interventions provided by psychiatric teams to patients at high risk of opioid overdose. METHODS: This observational study combined prospective surveys with retrospective chart review. Adults with a positive fentanyl urine drug screen within 6 months of a psychi

Exploring Nurses' Experiences in Caring for Medical-Psychiatric Comorbid Patients: A Qualitative Interview Study.

Approximately one-third of patients who visit general hospitals have a mental disorder, which is associated with poorer outcomes and higher costs. Caring for these patients can be challenging for nurses, particularly when behaviours such as aggression, wandering, or lack of cooperation occur. To explore how nurses experience caring for patients with medical-psychiatric comorbidities in general hospital wards a qualitative study using semi-structured interviews was conducted. Sixteen registered nurses with varying experience from eight wards in a Dutch university medical centre participated. Da

Exploring the multifaceted side effects of isotretinoin: a deep dive into case reports and observational studies.

This review addresses gaps in understanding isotretinoin's side effects for acne vulgaris. While effective, the risks are often underestimated. It emphasizes careful patient selection and monitoring to optimize outcomes and minimize harm. By providing a comprehensive evaluation, the review aims to enhance patient safety and inform clinical decision-making. This review examined isotretinoin's side effects through case reports and observational studies. A literature search on Google Scholar, PubMed, Scopus, and Web of Science covered publications from 2000 to 2024 using terms like "isotretinoin"

Psychoneural reduction revised: The case of suicidality in bipolar disorder.

This paper uses suicidality in bipolar disorder (BD) to illustrate that multidimensional frameworks account for complex phenomena with cognitive, psychological, socioenvironmental and physiological components better than their reductionist counterparts. We challenge level-based reductionist models that regard a particular dimension as fundamental. To comprehensively understand complex psychiatric phenomena, multidimensional models that acknowledge the heterogeneity of aetiological factors, rather than homogenising them under a single dimension, must be utilised. Multidimensional models allow a

Validating the efficacy and value proposition of mental fitness vocal biomarkers in a psychiatric population: prospective cohort study.

BACKGROUND: The utility of vocal biomarkers for mental health assessment has gained increasing attention. This study aims to further this line of research by introducing a novel vocal scoring system designed to provide mental fitness tracking insights to users in real-world settings. METHODS: A prospective cohort study with 104 outpatient psychiatric participants was conducted to validate the "Mental Fitness Vocal Biomarker" (MFVB) score. The MFVB score was derived from eight vocal features, selected based on literature review. Participants' mental health symptom severity was assessed using t

Psychological Pain as a Risk Factor for Suicidal Ideation: An Ecological Momentary Assessment Study on Inpatients With Depression With and Without Comorbid Borderline Personality Disorder.

Objective: Psychological pain (PP) is a potentially important risk factor for suicide. However, its temporal stability and association with suicidal ideation (SI) remain obscure. Whether PP represents a risk factor for SI independently of depression, anxiety, and hopelessness or is more prominent and temporally unstable in patients with depression and borderline personality disorder (BPD) is also unclear. Methods: From November 2020 to December 2022, psychiatric inpatients with depression without (N = 37) and with (N = 30) BPD were recruited to an ecological momentary assessment (EMA) study, w

Operationalisation of post-COVID condition case definition in a comprehensive research protocol.

BACKGROUND AND PURPOSE: Post-COVID-19 condition (PCC) is a prevalent and high-burden sequela of SARS-CoV-2 infection. Because of the complexity of its manifestations, PCC case definition currently lacks standardisation and reproducibility. We aimed to devise a simple screening tool to boost reproducibility and comparability of PCC case definition across PCC studies, and to provide a framework in which to reliably identify suspected PCC cases. We also developed a comprehensive assessment protocol based on the most frequently reported PCC characteristics. METHODS: Within a European multi-centre

"When something goes wrong".

BACKGROUND: Aspiration of food or liquids can result in suffocation, evolving in coughing, difficulty breathing and forced exhalation. Asphyxia occurs when the aspirated material occludes the upper airways, either in the proximal or distal tract, resulting in the inability to breathe. The risk of asphyxiation death, is increased if a person makes sudden movements while eating, walks or runs while eating, or even becomes distracted or frightened. It is higher in individuals with neurological diseases, intellectual disability (ID), cognitive impairment, psychiatric pathologies or their pharmacol

Opioid medication use in patients with gastrointestinal diagnoses vs unexplained gastrointestinal symptoms in the US Veterans Health Administration.

BACKGROUND: While opioid prescriptions have increased alarmingly in the United States (US), their use for unexplained chronic gastrointestinal (GI) pain (eg, irritable bowel syndrome) carries an especially high risk for adverse effects and questionable benefit. AIM: To compare opioid use among US veterans with structural GI diagnoses (SGID) and those with unexplained GI symptoms or functional GI diagnoses (FGID), a group for whom opioids have no accepted role. METHODS: Veterans Health Administration (VHA) administrative data from fiscal year 2012 were used to identify veterans with diagnosti
