Addressing ethical challenges in HIV prevention research with people who inject drugs.

Despite recent advances in HIV prevention and treatment, high HIV incidence persists among people who inject drugs (PWID). Difficult legal and political environments and lack of services for PWID likely contribute to high HIV incidence. Some advocates question whether any HIV prevention research is ethically justified in settings where healthcare system fails to provide basic services to PWID and where implementation of research findings is fraught with political barriers. Ethical challenges in research with PWID include concern about whether research evidence will be translated into practice;

Epidemiology of chronic wounds in Germany: Analysis of statutory health insurance data.

Epidemiologic analyses in routine care of chronic wounds are scarce, and published studies show wide variations. This study analyzes the population-based prevalence and incidence of chronic wounds in Germany. Secondary analyses of data from a German statutory health insurance with about 9 million insured persons were examined (2010 to 2012). Internal diagnostic validations were used to control for different inclusion criteria. In 2012, 1.04% (95% CI 1.03-1.05) of insured patients had a wound diagnosis, including 0.70% with leg ulcers and 0.27% with diabetic ulcers. Wound treatment was received

Perceived Health System Causes of Obstetric Fistula from Accounts of Affected Women in Rural Tanzania: A Qualitative Study.

Obstetric fistula is still a major problem in low income countries. While its main cause is untreated obstructed labour, misconceptions about it still persist. This study aimed at exploring and describing perceived health system causes of obstetric fistula from women affected by it in rural Tanzania. This exploratory qualitative study included twenty-eight women affected by obstetric fistula. Semi structured interviews and focus group discussions were held and thematic analysis used to analyse perceived health system causes of obstetric fistula from women's account. Perceived health system cau

Injected with controversy: sales and administration of injectable contraceptives in drug shops in Uganda.

CONTEXT: Informal drug shops are the first line of health care in many poor countries. In Uganda, these facilities commonly sell and administer the injectable contraceptive depot medroxyprogesterone acetate (DMPA), even though they are prohibited by law from selling any injectable drugs. It is important to understand drug shop operators' current practices and their potential to provide DMPA to hard-to-reach populations. METHODS: Between November 2007 and January 2008, 157 drug shops were identified in three rural districts of Uganda, and the operators of the 124 facilities that sold DMPA were

The context of informal abortions in rural Ghana.

AIMS: This qualitative study explores the context of unsafe abortion in rural Ghana with the aim of identifying areas that should be considered when operationalizing abortion-related reproductive health strategies. METHODS: Data come from eleven narratives about planned or attempted abortions and seven narratives of abortion-related deaths. These individual data are supplemented by data from ten focus group discussions. RESULTS: Communities describe abortions as dangerous and, if they become public knowledge, shameful. Despite this, abortions were understood as necessary for some women in so

Depression and HIV risk behavior practices among at risk women.

In this study, we examined the relationship between depression and HIV-related risk behavior practices in a sample of 250 at risk, predominantly African American women living in the Atlanta, Georgia metropolitan area. Interviews were conducted between August 1997 and August 2000. Street outreach efforts were used to identify potential study participants, with further expansion of the sample via targeted sampling and ethnographic mapping procedures. Our conceptual model hypothesized a relationship between depression and HIV risk in which depression and condom-related attitudes were construed as

Pregnancy, hormonal contraceptive use, and HIV-related death in Rwanda.

BACKGROUND: Few studies have examined the influence of pregnancy and hormonal contraception on HIV-related deaths in African women. Rwanda is a country with high fertility, high HIV prevalence, and frequent use of hormonal contraception in urban areas. METHODS: Data from a prospective cohort study of 460 HIV-infected urban childbearing (18-35 years) women followed at 6-monthly intervals for 6 years in Kigali, Rwanda, were analyzed. The relationship of time-dependent measures of pregnancy and hormonal contraceptive use to death from HIV disease was assessed with multivariate models. RESULTS:

The NSW Health routine screening for domestic violence program.

Routine questioning of women about abuse by their intimate partner has been progressively introduced in NSW in antenatal, early childhood, alcohol and other drug and mental health services since 2001. This was done because of the serious health consequences of domestic violence and the low identification rate of abuse by health services. Following a pilot that found strong support from female patients, this strategy is now well established in NSW public health services. Recent data indicate that approximately 10,000 women a month are asked these questions and that 7.3% of them report experienc

Physicians', nurses', and parents' attitudes to and knowledge about fever in early childhood.

This study investigated physicians', nurses' and parents' approach to fever in early childhood. A total of 2059 questionnaires was completed by the three groups. Though most of the responders (59.8%) believed that fever is a helpful bodily mechanism of the body, there was a significant difference between physicians (85.8%) and nurses and parents (63.9 and 43.1%, respectively) (P<0.001). The majority of parents (62.7%) believed it necessary to treat children with low-grade fever (<38 degrees C) without any other sign of illness, whereas the physicians and nurses did not (10.8 and 30.2%, respect

The problems of a social survey in epidemiology: an experience from a Zambian rural community.

A socio-economic study of 1097 people was carried out between November and December 1979. Demographic data and other health characteristics were obtained by census of the entire study population. Interviews covered disease awareness, perceived morbidity, health-care utilization, knowledge, attitudes and practices; all adults aged 15 years and above were interviewed 2 weeks before physical examinations were made. Age and literacy level were found to have no effect on the people's health-seeking behaviour in Kabinga. The results of this social survey failed to reveal the real practices of the co

Major Adverse Kidney Events in the American Indian or Alaska Native Population with Diabetes.

KEY POINTS: Knowledge of the risk of major adverse kidney events in the American Indian or Alaska Native population with diabetes is limited. Compared with the non-Hispanic White population, the American Indian or Alaska Native population was younger with poorer glycemic control. This population had a 20% higher risk of major adverse kidney events with social drivers of health and health care utilization as key predictors. BACKGROUND: CKD and diabetes disproportionately affect the American Indian (AI) or Alaska Native (AN) population, but understanding of major adverse kidney events (MAKE) in

Rural Youth Firearm Exposure, Mental Health and Interest in Safety Education.

BACKGROUND: Rural communities in the US are experiencing the fastest growing rate of suicide, where firearms are the leading cause of death for youth ages 14 to 18. This study investigates the mental health and firearm exposure burden among this age group in rural Pennsylvania. METHODS: We analyzed survey results from 1570 high school students ages 14 to 19 to identify the local prevalence of firearm violence exposure. Logistic regression models estimated the association between exposure and depression among students and their interest in violence prevention education. RESULTS: Results indic

The double-edged sword of social capital: Impacts on women's health in extreme poverty.

Social capital is extremely important in determining health outcomes yet relatively little is known about how different types of social capital impact on individuals' health outcomes, particularly for the poorest in society. We use secondary data generated from a quasi-experimental study conducted among extremely poor women in rural Bangladesh and apply Propensity Score Matching (PSM) technique to create comparable treatment and control groups. Using a comparative analytical approach, we examine how social capital effects vary and investigate how different social capital measures are associate

Feature recess-time sports activities as a school-based intervention to improve fitness in rural Chinese youth.

BACKGROUND: China's national school health policies face persistent implementation gaps, particularly in rural high schools prioritizing Gaokao. National surveys (2010-2019) documented alarming fitness declines: 50-m sprint speeds decreased (+0.3s boys; + 0.4s girls), pull-ups/sit-ups fell 29%/20%, and standing jumps shortened 6-7 cm. METHODS: A 16-week cluster quasi-experiment assigned intact rural high school classes (N = 98; age = 16.35 ± 0.48years) to: • Experimental (n = 50): Feature Recess-Time Sports Activities (FRTSA; 5x30-min/week). • Control (n = 48): Standard supervised running.
