Transmitter interactions in the central cholinergic control of blood pressure regulation.

There are at least five mechanisms by which the central nervous system regulates neural and humoral systems that control the blood pressure (BP). Particular attention has been paid to central cholinergic-adrenergic interactions in the regulation of BP. Physostigmine and other anticholinesterases which penetrate the blood-brain barrier, both carbamates and organophosphates, produce an increase of BP. This effect can be abolished by atropine, but not by methylatropine. The available evidence indicates that physostigmine and other AChE inhibitors initially produce an activation of central muscari…

Role of sympathetic efferent nerves in blood pressure regulation and in hypertension.

This article presents some aspects of the role of sympathetic efferent nerves in the regulation of blood pressure in humans. Lessons have been learned from disorders that cause either sympathetic underactivity or overactivity. In chronic autonomic failure, pressor stimuli (mental arithmetic, isometric exercise, or cold) are unable to raise blood pressure, whereas stimuli that normally activate sympathetic efferent nerves to maintain blood pressure (head-up tilt, exercise, and food ingestion) can cause marked hypotension. Recognition of specific defects, such as the inability to synthesize nore…

Pressure-diameter relationships of the upper airway in awake supine subjects.

In awake supine normal subjects, dimensional changes of the oropharyngeal airway were measured during exposure to negative intraluminal pressures. The pressure was generated 1) "actively" by subjects inspiring against an externally occluded airway or 2) "passively" by external suction at the mouth during voluntary glottic closure with no inspiratory effort. Airway dimensions were imaged with X-ray fluoroscopy and anteroposterior diameters measured at levels corresponding to cervical vertebra 3 and 4 (C3 and C4). Cephalad axial displacement of the hyoid bone (CDHY) was also measured. During the…

[Changes in the circulation of the eye caused by Vörösmarthy preoperative ocular pressure].

Prior to, directly after and 15 minutes after preoperative Vörösmarthy oculopression (30, 40, or 50 mm Hg; 15 min), 30 patients were examined by oculooscillodynamography after Ulrich. Systolic retinal and ciliary perfusion pressures were higher directly after oculopression, whereas the systolic and diastolic ocular blood pressures (intramural pressures) were lower. These changes did not depend significantly on the level of oculopression. Fifteen minutes after oculopression, the ocular perfusion and blood pressures were largely normalized. The hemodynamic changes observed reflect the decrease i…

Pressure responses and conduction of peristaltic wave in guinea-pig ureter.

Physiological properties of guinea-pig ureteral segments were studied by measuring the internal pressure responses to electrical stimulation in iso-volume conditions and by recording the contraction elicited by local electrical stimulation in iso-pressure conditions with a video-system. In iso-volume conditions, the phasic pressure response of the ureter increased in magnitude with increasing resting intra-luminal pressure and its threshold was lower in the proximal end, i.e., the part nearest to the renal pelvis, and higher in the distal end. In iso-pressure conditions, the average conduction…

Cerebral perfusion pressure and CO2-reactivity in the evaluation of carotid obstructions.

In order to evaluate the hemodynamic effect of a carotid artery obstruction, the CO2-reactivity was determined and the systolic carotid artery pressure was measured in 34 patients with a stenosis (20) and/or an occlusion (17) of the internal carotid artery. In 19 patients this was done bilaterally and in 15 unilaterally, a total of 53 observations. A decreased CO2-reactivity was found in 49% (26/53). With a carotid artery blood pressure of less than or equal to 90 mmHg the reactivity Index was diminished in 86% (12/14), with greater than 90 mmHg in 36% (14/39) of cases. It is assumed that in t…

Alcohol intake and blood pressure in young adults: the CARDIA Study.

Associations between self-reported average daily alcohol intake and blood pressure were assessed in 5031 black and white men and women ages 18-30 from the Coronary Artery Risk Development in Young Adults Study (CARDIA). In general, intake was positively but weakly related to both systolic and diastolic blood pressure. Associations with systolic pressure were generally stronger than those with diastolic pressure. With average daily alcohol intake categorized as none, 0.1-9.9 ml, 10.0-19.9 ml, 20.0-29.9 ml, and 30.0+ ml, mean systolic pressure, adjusted for age, body mass index, education, smoki…

The effect of nimodipine on ICP and CBF in patients with normal-pressure hydrocephalus.

Eight patients with normal-pressure hydrocephalus (NPH) were studied. The resting mean arterial blood pressure (MABP) was 100 (90-125) mmHg and the mean intracranial pressure (ICP) was 11 (5-17) mmHg. ICP and MABP were continuously measured intraventricularly and intra-arterially, respectively. Changes in global cerebral blood flow (CBF) were estimated by the arteriovenous oxygen difference method. Intravenous nimodipine (15 microgram/kg/hour) was given in the first 2 hours and 30 microgram/kg/hour in the next 2 hours. MABP was reduced 23 (4-47) mmHg (p less than 0.05). ICP was increased 3 (0-…

Response of tertiary centres to pressure changes. Is there a mechano-electrical association?

STUDY OBJECTIVE: To investigate the mechanism of pressure related ventricular arrhythmias by examining them during atrioventricular (AV) block. DESIGN: Complete AV block, where all ventricular beats are ectopic, was induced by AV node ablation and/or by toxic digitalisation, and rhythm changes were studied while arterial blood pressure was repeatedly raised and lowered. SUBJECTS: 15 anaesthetised mongrel dogs, weight 15-28 kg, were used. AV block was induced in eight by chemical or mechanical ablation of the AV node. In five of these and in seven other dogs, 5.0-7.5 mg digoxin was also given…

Circadian variations of catecholamines and blood pressure in patients with pseudohypoparathyroidism and hypertension.

The relationship between 24-h recumbent blood pressure levels and secretory patterns of catecholamines was investigated in 4 patients with pseudohypoparathyroidism (PsHP) and hypertension and in 9 patients with essential hypertension. A clear circadian rhythm of blood pressure and catecholamines was documented in both groups with lowest levels of blood pressures and catecholamines occurring during sleep. During the 24-h period of recumbency mean arterial blood pressure (MAP) was correlated (r = 0.63, p less than or equal to 0.01) with plasma norepinephrine (N) in the patients with essential hy…

The influence of middle ear pressure on the vestibular nerve activity in cats.

The influence of middle ear pressure on the vestibular nerve activities was investigated in anesthetized cats. The vestibular nerve activities were recorded intraaxonally and positive or negative pressure was applied to the middle ear cavity through a tympanic membrane perforation. The firing of vestibular nerve fibers, especially the regular type nerve that responded to horizontal semicircular canal stimulation, increased with positive pressure and decreased with negative pressure. Most vestibular nerves that responded to anterior or posterior semicircular canal stimulation were not influence…

Association of magnesium deficiency with the blood pressure-lowering effects of calcium.

The role of dietary calcium and magnesium in the development of hypertension was studied in nine groups, each consisting of nine spontaneously hypertensive rats aged 8-31 weeks. The animals were fed AIN 76A semi-purified diets varying in calcium (0.075, 0.5 and 2.5%) and magnesium (0.01, 0.05 and 0.75%) concentrations according to a 3 x 3 factorial design. Dietary calcium and systolic blood pressure were inversely related, significantly (P less than 0.05) after 12 weeks. Total and ultrafilterable serum calcium concentrations were also significantly negatively correlated with blood pressure (r …

Cochlear and cerebrospinal fluid pressure: their inter-relationship and control mechanisms.

The patency of the cochlear aqueduct is a key factor in intra-cochlear hydromechanics. If patent, the cerebrospinal fluid (CSF) provides the reference pressure for the perilymph and also to a large extent the endolymph, since Reissner's membrane can only withstand a relatively small pressure differential. The aqueduct often becomes sealed as a natural process of ageing. In this instance the reference pressure is from a source, its position unknown, within the boundaries of the cochlea itself. Relatively large and rapid changes in the cerebrospinal fluid pressure may result from everyday events…

Screening for newly elevated blood pressure in an emergency unit setting.

We found a prevalence rate of elevated blood pressure readings in our sample of 86 nonacute EU patients without history of hypertension that was similar to that reported by the author of a previous study (Franklin, 1987) done in the same setting and continue to advocate the use of the EU as a site for hypertension screening. However, the noted prevalence rate in this and other samples may not accurately reflect the true prevalence of elevated readings or true hypertension in this population. Most of the elevated readings in our study were in the borderline or mild categories, and many of the r…

Clinical experience with a pressure-adjustable valve SOPHY in the management of hydrocephalus.

Seventy-four patients with hydrocephalus due to a variety of causes were treated with a cerebrospinal fluid (CSF) shunt, incorporating a recently developed pressure-adjustable valve SOPHY (PAVS). The PAVS may be changed percutaneously with the help of an externally applied magnet in order to select a high-, medium- or low-valve opening pressure, whenever the need for a change in pressure characteristics seems necessary to the neurosurgeon. The percutaneous pressure adjustment obviated up- or downgrading of a medium pressure position by surgical means in half of our patients during the follow-u…

Regulation of blood pressure in pregnancy: pressor system blockade and stimulation.

Contributions of the autonomic nervous system (ANS), renin-angiotensin system (RAS), and arginine vasopressin (AVP) to basal mean arterial pressure (MAP) were evaluated in near-term pregnant and virgin rats as follows. MAP and heart rate (HR) were measured before and after ganglionic, alpha-adrenoreceptor, RAS, and/or AVP blockade. In addition, pressor responses to angiotensin II (ANG II), norepinephrine, phenylephrine, or AVP were determined in ganglionic-blocked animals. In both groups decrements in MAP were greatest after ganglionic or alpha-blockade, intermediate after RAS blockade, and ne…
