[Retrospective Long-Term Results After Dellon's Nerve Decompression In The Lower Leg For Polyneuropathy].

BACKGROUND: Patients affected by polyneuropathy often report pain, paraesthesia and numbness and are at risk of having an increased propensity to fall with the corresponding complications. In case of persistent symptoms after all conservative and medical treatment options have been exhausted, a nerve decompression in the lower leg as described by Dellon can lead to an improvement of the discomfort for many patients. This article presents our long-term results with Dellon's operation. METHODS: In this retrospective study, we included 33 patients with diabetic or idiopathic polyneuropathy in wh

Clinical Outcomes of the Transforaminal Lumbar Interbody Fusion Technique Among Patients With Low Back Pain Showing Type 1 Modic Changes on MRI.

Introduction The unilateral transforaminal lumbar interbody fusion (TLIF) signifies a different surgical method, circumventing both the anterior method and the method via the spinal canal. Due to the shortage of literature available for clinical outcomes and consequences post-TLIF, we undertook the current study to assess the TLIF technique's clinical outcomes among patients with low back pain showing type 1 Modic changes on MRI. Material and methods A cross-sectional study was conducted between January 2019 and March 2021. All patients included in the study had Modic type 1 change and disabli

Retrospective Efficacy and Cost-Containment Assessment of 10 kHz Spinal Cord Stimulation (SCS) in Non-Surgical Refractory Back Pain Patients.

BACKGROUND: Non-surgical refractory back pain (NSRBP) is persistent, severe back pain that is not considered surgically correctable. Published studies have demonstrated clinically important long-term improvement in pain and functional capacity when 10kHz spinal cord stimulation (SCS) is used to treat NSRBP. This study examines if real-world patients in interventional pain practice obtain the same outcomes, and have any reduction in health care utilization (HCU) following 10kHz SCS implant. METHODS: We conducted a retrospective chart review of 105 patients from two clinical sites who underwent

Bupivacaine administered intrathecally versus rectally in the management of intractable rectal cancer pain in palliative care.

BACKGROUND: Unacceptable adverse effects, contraindications to and/or ineffectiveness of World Health Organization step III "pain ladder" drugs causes needless suffering among a population of cancer patients. Successful management of severe cancer pain may require invasive treatment. However, a patient's refusal of an invasive procedure necessitates that clinicians consider alternative options. OBJECTIVE: Intrathecal bupivacaine delivery as a viable treatment of intractable pain is well documented. There are no data on rectal bupivacaine use in cancer patients or in the treatment of cancer te

Sacroiliac joint pain in the pediatric population.

OBJECT: The sacroiliac (SI) joint can be a pain generator in 13%-27% of cases of back pain in adults. These numbers are largely unknown for the pediatric population. In children and especially girls, development of the pelvic girdle makes the SI joint prone to misalignment. Young athletes sustain repeated stress on their SI joints, and sometimes even minor trauma can result in lasting pain that mimics radiculopathy. The authors present a series of 48 pediatric patients who were evaluated for low-back pain and were found to have SI joint misalignment as the cause of their symptoms. They were tr

Candida albicans osteomyelitis of the spine: progressive clinical and radiological features and surgical management in three cases.

Candida albicans vertebral osteomyelitis is rare. Three cases are presented. Without antifungal treatment, they developed spinal collapse and neurological deterioration within 3-6 months from the onset of symptoms. There was a delay of 4.5 and 7.5 months between the onset of symptoms and surgery. All patients were managed with surgical debridement and reconstruction and 12-week fluconazole treatment. The neurological deficits resolved completely. The infection has not recurred clinically or radiologically at 5-6 years follow-up. Although rare, Candida should be suspected as a causative pathoge

The Efficacy of Synovectomy in Rotator Cuff Repair: A Randomized Controlled Trial.

BACKGROUND: Synovitis is frequently observed in patients with rotator cuff tears and has been associated with increased pain and inferior clinical outcomes. Although synovectomy has demonstrated clinical benefits in primary synovial disorders, its role as an adjunctive procedure during arthroscopic rotator cuff repair remains controversial, particularly regarding early postoperative pain relief. PURPOSE: To evaluate the effect of synovectomy on clinical outcomes after arthroscopic rotator cuff repair. STUDY DESIGN: Randomized controlled trial; Level of evidence, 1. METHODS: This prospective

Dorsal Root Ganglion Pulsed Radiofrequency with Scar Radiofrequency Ablation Combined for Chronic Postoperative Abdominal Pain: A Retrospective Study.

BACKGROUND: Chronic postoperative abdominal pain (CPAP) manifests as neuropathic or mixed pain, and conservative treatments often only provide temporary relief. This study evaluated the efficacy and safety of combining dorsal root ganglion pulsed radiofrequency (DRG-PRF) with scar radiofrequency ablation (RFA) for a better treatment. METHODS: A total of 48 CPAP patients were divided into the scar RFA group (RFA group, n = 25) and the combination of DRG-PRF and scar RFA group (RFA+PRF group, n = 23). The visual analogue scale (VAS) and Globe Pain Scale (GPS) questionnaires were used for pain i

Repeat Surgery for Recurrent or Refractory Trigeminal Neuralgia: A Systematic Review and Meta-Analysis.

OBJECTIVE: Surgery can effectively treat Trigeminal neuralgia (TN), but postoperative pain recurrence or nonresponse are common. Repeat surgery is frequently offered but limited data exist to guide the selection of salvage surgical procedures. We aimed to compare pain relief outcomes after repeat microvascular decompression (MVD), percutaneous rhizotomy (PR), or stereotactic radiosurgery (SRS) to determine which modality was most efficacious for surgically refractory TN. METHODS: A PRISMA systematic review and meta-analysis was performed, including studies of adults with classical or idiopath

Pain Control for Sickle Cell Crisis, a Novel Approach? A Retrospective Study.

Background and Objectives: Pain management poses a significant challenge for patients experiencing vaso-occlusive crisis (VOC) in sickle cell disease (SCD). While opioid therapy is highly effective, its efficacy can be impeded by undesirable side effects. Local regional anesthesia (LRA), involving the deposition of a perineural anesthetic, provides a nociceptive blockade, local vasodilation and reduces the inflammatory response. However, the effectiveness of this therapeutic approach for VOC in SCD patients has been rarely reported up to now. The objective of this study was to assess the effec

Clinical Efficacy of Pulsed Radiofrequency Neuromodulation for Intractable Meralgia Paresthetica.

BACKGROUND: Meralgia paresthetica (MP) is a neurologic disorder of the lateral femoral cutaneous nerve (LFCN), which is characterized by a localized area of paresthesia and numbness on the anterolateral aspect of the thigh. In most patients with MP, symptoms can be successfully managed with conservative treatment. However, in a small group of MP patients who are refractory to medical treatment, more aggressive low-risk treatment should be considered. OBJECTIVE: The objective of this study was to evaluate clinical outcomes of pulsed radiofrequency (PRF) neuromodulation of the LFCN in MP patien

Feasibility of monomodal analgesia with IV alfentanil during burn dressing changes at bedside (in spontaneously breathing non-intubated patients).

INTRODUCTION: The severe pain related to repeated burn dressing changes at bedside is often difficult to manage. However these dressings can be performed at bedside on spontaneously breathing non-intubated patients using powerful intravenous opioids with a quick onset and a short duration of action such as alfentanil. The purpose of this study is to demonstrate the efficacy and safety of the protocol which is used in our burn unit for pain control during burn dressing changes. PATIENTS AND METHODS: Cohort study began after favorable opinion from local ethic committee has been collected. Patie

Case Report: Oral Fluphenazine for Treatment of Chronic Sacral Pain.

Management of chronic pain syndromes represent significant challenges to clinicians. Fluphenazine was shown to attenuate mechanical allodynia in rat neuropathic pain models via antagonism of voltage gated sodium channels and has been identified as a candidate drug for chronic pain. We present a 66 year old man with treatment-refractory chronic pain syndrome. Right ischial pain developed over time as a result of his years practicing as a dentist sitting on a stool with a tilted seat pan. Imaging and neurologic investigations led to a presumed diagnosis of chronic microtears of the sacrotuberous

Adjuvant Analgesics in Acute Pain - Evaluation of Efficacy.

PURPOSE OF THE REVIEW: Acute postoperative pain impacts a significant number of patients and is associated with various complications, such as a higher occurrence of chronic postsurgical pain as well as increased morbidity and mortality. RECENT FINDINGS: Opioids are often used to manage severe pain, but they come with serious adverse effects, such as sedation, respiratory depression, postoperative nausea and vomiting, and impaired bowel function. Therefore, most enhanced recovery after surgery protocols promote multimodal analgesia, which includes adjuvant analgesics, to provide optimal pain
