70 patients presenting to a surgical unit with radiation enteritis were followed up. 11 of the 61 who underwent operations died of operation-related causes. The 51 patients who survived for more than 3 months were followed up for up to 12 years (median 12 months). 24 had no further symptoms related to their radiation enteritis. The other 27 patients had persistence of symptoms, post-operative complications, new radiation-related problems, or a combination of these. Colovesical and colovaginal fistulas following irradiation for pelvic malignancy represent a formidable surgical problem. Although complex surgical procedures to close the fistulas and restore continence have been described, often a defunctioning colostomy with an associated urinary conduit is the only feasible option. We reviewed clinical and electrodiagnostic features of 16 patients with neoplastic brachial plexopathy (NBP) and 17 patients with radiation-induced plexopathy (RBP). The groups were similar in symptom-free interval after cancer diagnosis and location of the plexus lesions. Fifty nine stage II-distal (i.e. with involvement of medium third of parametrium and/or vagina), 220 stage III and 5 stage IV cervical carcinomas were treated with radiotherapy alone (combination of intracavitary radium therapy and megavoltage external irradiation on the pelvis) in the years 1971-79. The 5-year relapse free survival by actuarial method was 52.0% for stage II-distal and 40.6% for stage III, while all stage IV patients relapsed within the third year from the start of the treatment. In a total of 791 cases of peripheral neuroparalysis, we found 77 cases of late paresis corresponding to approximately 10 per cent of the total number of patients. Almost half the number of late paresis were pareses of the ulnar nerve. The average latency period was 18 years. Late complications involving the bladder and ureter occasionally occur following irradiation to the minor pelvis. Intermittent light bleeding from the bladder can be contained by nonspecific installation therapy. With life-threatening hemorrhage, silver nitrate or formalin instillation is effective. The treatment of irradiation ulcers, irradiation contracted bladder, vesicovaginal fistulae, and late irradiation reactions of the ureter are discussed in incurable bladder contractions and vesicovaginal fistulae, a supravesicle urinary diversion is necessary.

We serially assessed the hearing sensitivity of 61 children with acute lymphocytic leukemia who were admitted to our Total Therapy IX study between December, 1975, and July, 1977. Twenty-three patients underwent 32 operations in a previously irradiated abdominopelvic cavity. The overall postoperative morbidity rate was 62% and the mortality rate was 6.2%. The data from this series do not support small bowel bypass as the only acceptable method of management of surgical radiation enteritis. Wide resection with primary anastomosis reduces the long-term morbidity of the "blind-loop syndrome", perforation and fistulization of the isolated loop, and does not have an increased mortality.
