Outcomes of Local Bone Graft and Fixation of Proximal Pole Scaphoid Nascent Nonunions and Nonunions.

Background  Fractures of the proximal pole of the scaphoid have an increased risk of nonunion due to its tenuous blood supply. The optimal treatment of proximal pole scaphoid nonunions remains controversial. Objectives  To review a single surgeon's experience with proximal pole scaphoid nascent nonunions (delayed unions) and nonunions that underwent surgical fixation with a cannulated headless compression screw and local autologous bone graft from the distal radius. Patients and Methods  After obtaining Institutional Review Board approval, the electronic medical record of one tertiary care cen

Osteoporosis in rheumatoid arthritis.

OBJECTIVE: To answer and comment on a number of controversial issues in relation to osteoporosis and rheumatoid arthritis (RA), including: Is osteoporosis an extra-articular manifestation of rheumatoid arthritis? Does periarticular osteoporosis reflect disease activity in early arthritis? Is there a threshold for corticosteroid-induced osteoporosis? Can anti-resorbing drugs prevent rheumatoid arthritis progression? Are stress fractures rare in rheumatoid arthritis Is methotrexate toxic for bone? METHODS: Confrontation of current literature and our own experience in order to formulate a genera

Validation of an automatic three-dimensional method for distal radius measurements and malunion quantification.

Preoperative CT measurements of distal radial inclination and volar tilt are useful for surgical planning, but current methods are time-consuming and user-dependent. This study validates an automated 3D measurement method for these measurements. We analysed 20 bilateral CT scans, including healthy and pathological sides with extra-articular malunions. Three senior observers performed manual measurements and an automated algorithm generated measurements based on the radiocarpal surface. Agreement between manual and automatic methods was evaluated using concordance correlation coefficients and B

Comparison of operatively and nonoperatively treated isolated mason type II radial head fractures: a systematic review and meta-analysis.

BACKGROUND: Radial head fractures are the most common bony injury of the elbow in adults. The current literature does not agree on whether isolated stable type II radial head fractures should be treated operatively or nonoperatively. This review aims to determine the preferred treatment for Mason type II radial head fractures and compare the outcomes of conservative and surgical treatment. METHODS: Our study used PRISMA guidelines and conducted a thorough search of multiple electronic databases, including PubMed, Cochrane, Embase, Web of Science, CNKI, and Wanfang databases, initially identif

Do volar locking plates fit the volar cortex of the distal radius?

UNLABELLED: The purpose of this study was to measure the volar cortical angles of the commercially available volar locking plates and evaluate how well they fit the distal radius. We measured the volar cortical angles of the radial and intermediate columns of eight volar locking plates and compared them with the volar cortical angles of 90 cadaver distal radii. The mean radial and intermediate column volar cortical angles of the wider plates were significantly larger than those of narrower plates. The mean radial and intermediate column volar cortical angles were significantly larger in wider

Open Galeazzi fracture with ipsilateral elbow dislocation.

Combination of the Galeazzi fracture and dislocation of the elbow joint in same extremity is very rare. In this article, we report a 26-year-old male patient with a posterolateral dislocation of the elbow and ipsilateral volar type Galeazzi fracture. We performed closed reduction for the elbow dislocation during admission to the emergency department. Patient was taken to the operating room in the sixth hour of his application to emergency department and open wound on the ulnovolar region of the wrist was closed primarily after irrigation and debridement. We performed open reduction and interna

The Factors Influencing the Refracture of Pediatric Forearms.

PURPOSE: Pediatric forearm fractures are common and usually heal uneventfully. The purpose of this study was to review the refracture rate and to identify trends and risk factors that may lead to a refracture. METHODS: Using current procedure terminology code and subsequent chart review we retrospectively identified 2590 patients who sustained forearm fractures over the past 10 years (2000 to 2010) and were treated at a single, large pediatric orthopaedic practice. RESULTS: We identified 37 patients who met our search criterion which yielded a refracture rate of 1.4%. Average length of immob

Paediatric fracture dislocation of the forearm: an unusual presentation.

Traumatic injuries of the elbow in children pose a diagnostic challenge. They demand precise knowledge of radiographic anatomy, growth characteristics of healthy and injured bones as well as specific fracture dynamics in this age group. Dislocation of the elbow is rare in children, accounting for 3-6% of all childhood elbow injuries. Fractures at the distal humerus and proximal forearm have been extensively discussed in the literature. We describe a unique case of a 4-year-old child presenting with a traumatic radial head fracture and ulnar dislocation not previously reported. Diagnosis and re

Is distal ulna resection influential on outcomes of distal radius malunion corrective osteotomies?

INTRODUCTION: The mechanical repercussions of distal radius malunion on the distal radio-ulnar (DRU) joint are common and inconsistently corrected by radius osteotomy alone. Ulnar resection has thus become a palliative solution. HYPOTHESES: Does ulna resection influence the outcomes of distal radius malunion corrective osteotomies? What preoperative factors warrant preserving the distal radio-ulnar joint? PATIENTS AND METHODS: Twenty-one corrective osteotomies of the radius were retrospectively reviewed. Ulna resection was performed in cases of cartilage damage, joint incongruence, or persis

Reduction of bowing forearm fractures in the ED under propofol sedation.

Bowing fractures of the long bones result from a deforming force that bends the bone similar to an archer's bow. In some of the earlier descriptions of bowing fractures, the recommended treatment for these was closed reduction under general anesthesia in the operating room. The aim of this study was to describe 2 pediatric patients with bowing fractures of the forearm that were successfully reduced in the emergency department (ED) under deep sedation with propofol. Two cases of bowing fractures of the forearm reduced in the ED under propofol sedation were described. Some bowing fractures of th

Interosseous membrane reconstruction for the Essex-Lopresti injury.

The Essex-Lopresti lesion, or radioulnar longitudinal dissociation, results from an axial load to the forearm with injury to the radial head and disruption of the interosseous membrane and the distal radioulnar joint. Unfortunately, an appreciation of the true extent of injury is not always realized in the acute setting, and patients present later with persistent or new reports of forearm discomfort and wrist and elbow pain. Reconstruction of the central band of the interosseous membrane with a bone-patellar tendon-bone graft is useful in the chronic setting and is described.

Orthopedic problems in Lesch-Nyhan syndrome.

Lesch-Nyhan syndrome is an X-linked disorder of purine metabolism. The orthopedic problems and results of treatment of nine Lesch-Nyhan patients are reviewed. Associated orthopedic problems included hip subluxation or dislocation (nine of 18 hips), fractures (three), autoamputation, infections (three), minor scoliosis, and contractures. Lesch-Nyhan patients can safely undergo orthopedic procedures and the results of surgery are satisfactory and similar to those of patients with spastic cerebral palsy. All of the seven operated-on hips maintained good reduction at 6-year mean follow-up. With ad
