Thermoregulation: physiological and clinical considerations during sedation and general anesthesia.

Mild hypothermia is common during deep sedation or general anesthesia and is frequently associated with patient discomfort and shivering. Greater declines in temperature can produce an even greater number of significant detrimental effects. This article reviews principles of thermoregulation and influences of anesthetic agents. An understanding of these will provide a foundation for strategies to reduce heat loss and better manage patient discomfort when it occurs.

Drug-drug interaction between itraconazole and the antiretroviral drug lopinavir/ritonavir in an HIV-1-infected patient with disseminated histoplasmosis.

We describe a drug-drug interaction between coformulated lopinavir/ritonavir and itraconazole in a patient infected with human immunodeficiency virus type 1 who had disseminated histoplasmosis. Coadministration of these agents led to a strong increase in itraconazole concentrations and a decrease in concentrations of its metabolite, hydroxyitraconazole, which is equally active pharmacologically. The dosage of itraconazole was reduced when it was used in combination with lopinavir/ritonavir.

Resorbable Purified Porcine Skin Gelatin Cross-Linked with Glutaraldehyde Spheres for Preoperative Embolization of Carotid Body Paraganglioma.

Chemodectomas are rare neuroendocrine tumors that typically arise at the carotid bifurcation and progressively englobe the internal and external carotid artery. Surgical asportation of the capsulated mass is the elective treatment. Pre-procedural embolization of this high vascular tumors is highly recommended because it has shown to improve surgical outcome by reducing both, mean blood loss and total operation time. Many different embolization techniques have been described in literature. In the here presented case we opted for an endovascular approach using resorbable purified porcine skin ge

New insights into the cause of Parkinson's disease.

Current concepts as to the cause of Parkinson's disease (PD) suggest an inherited predisposition to environmental or endogenous toxic agents. Study of the substantia nigra after death in PD has highlighted three major changes: (1) evidence of oxidative stress and depletion of reduced glutathione; (2) high levels of total iron, with reduced ferritin buffering; and (3) mitochondrial complex I deficiency. Which of these is the primary event, generating a secondary cascade of changes culminating in nigral cell death, is unknown. In presymptomatic Lewy body-positive control brains, the nigra shows

Staged approach to overcome hyperbilirubinemia: tailored chemotherapy in liver metastases-a case report.

BACKGROUND: Hyperbilirubinemia presents a significant challenge in the treatment of patients with hepatic dysfunction, particularly those with metastatic liver disease requiring chemotherapy. Many chemotherapeutic agents undergo hepatic metabolism and elimination, complicating treatment due to altered pharmacokinetics, increased toxicity risks, and the absence of standardized dose adjustments. Patients with severe hepatic impairment are frequently excluded from clinical trials, limiting available evidence on safe and effective treatment strategies for this group of patients. Emerging approache

Formation of hydrated PEG layers on magnetic iron oxide nanoflowers shows internal magnetisation dynamics and generates high in-vivo efficacy for MRI and magnetic hyperthermia.

Multicore magnetic iron oxide nanoparticles, nanoflowers (NFs), have potential biomedical applications as efficient mediators for AC-magnetic field hyperthermia and as contrast agents for magnetic resonance imaging due to their strong magnetic responses arising from complex internal magnetic ordering. To realise these applications amenable surface chemistry must be engineered that maintain particle dispersion. Here a catechol-derived grafting approach is described to strongly bind polyethylene glycol (PEG) to NFs and provide stable hydrogen-bonded hydrated layers that ensure good long-term col

Antithrombotic treatment of cerebrovascular disease.

The most common type of cerebrovascular disease is ischaemia or infarction from atherothrombosis or cardiac embolism. Antithrombotic treatment with an antiplatelet agent or anticoagulant assumes a prior clinical classification into categories of transient ischaemic attack (TIA) or minor stroke, acute partial stable stroke, stroke-in-progression, and completed stroke. Aspirin reduces the risk of stroke, myocardial infarction, and death after TIA or minor stroke secondary to atherothrombosis. Aspirin is effective in both sexes at a dose of 300 or 1200 mg/day. Ticlopidine (500 mg/day), a new anti

The Antiviral Activity and Underlying Mechanisms of Quercetin in Combating Largemouth Bass Virus Infection.

Largemouth bass virus (LMBV), a member of the Ranavirus genus within the Iridoviridae family, is a significant pathogen causing high mortality and substantial economic losses in largemouth bass aquaculture. Therefore, developing effective antiviral agents against this viral infection is crucial for the largemouth bass aquaculture industry. Quercetin, a widely utilised flavonoid known for its therapeutic and prophylactic properties, was investigated for its antiviral efficacy against LMBV and underlying mechanisms in this study. The results demonstrated that quercetin significantly inhibited LM

Vactosertib reverses ABCG2-mediated multidrug resistance through dual inhibition of drug efflux and transporter expression.

ATP-binding cassette (ABC) transporter ABCG2 is a key mediator of multidrug resistance (MDR) in cancer. Its overexpression limits the intracellular retention and cytotoxicity of chemotherapeutic agents. Identifying clinically relevant modulators capable of suppressing ABCG2 activity remains an important therapeutic objective. In this study, we examined whether selective ALK4/5 inhibitor vactosertib can reverse ABCG2-associated MDR. At non-cytotoxic concentrations, vactosertib significantly reduced the IC50 values of multiple ABCG2 substrates in both wild-type and R482-variant mutant ABCG2-over

Lactoferrin blocks orthopoxvirus entry via heparan sulphate and regulates host antiviral pathways.

Current antiviral therapies for orthopoxviruses face critical challenges, including limited efficacy and significant toxicity, which impede outbreak containment and clinical management. Here, we identify lactoferrin as a potent antiviral agent against multiple orthopoxvirus strains. Combination administration of lactoferrin with brincidofovir or tecovirimat demonstrated additive efficacy, suggesting a potential clinical strategy to reduce individual drug toxicity. Mechanistically, lactoferrin blocks viral entry by competitively binding to heparan sulphate proteoglycans (HSPGs). It also suppres

Cyclic dipeptide-based antimicrobials with potent antibacterial activity.

The spread of antibiotic resistance has become a major challenge in global public health, and there is an urgent need to develop novel antibacterial agents with low resistance. Peptide-based antibacterial agents have attracted increasing attention for their membrane-targeting mechanisms, which may reduce the risk of the development of resistance. Herein, a series of quaternary ammonium-modified cyclic dipeptides (named cHHn-m) were designed and synthesized through a quaternization reaction and their antibacterial activity and cytotoxicity were systematically investigated. The optimal cyclic di
