Most complex humeral head fractures are associated with a longitudinal tear of the rotator cuff. Reconstruction of this tear is frequently omitted from surgical reduction and fixation procedures. In 48 patients treated by surgery or by conservative means, we investigated the relationship between rotator cuff integrity and the functional results. The rotator cuff was assessed by ultrasonography, the functional results by the Constant Score. Common signs of rotator cuff alterations were partial or complete atrophy. In all patients affected, a longitudinal defect located at the rotator interval w

Eighty-seven consecutive patients were treated with an open Bankart repair for traumatic, recurrent anterior instability of the shoulder. Forty-six of these patients underwent a transosseous suture technique. Forty-one patients were operated on with suture anchors. After follow-ups that ranged from 18 to 85 months, 7 of the 87 patients showed signs of recurrence and another 14 patients reported apprehension. Compared with the 66 patients without residual instability, these 21 patients were more likely to have been operated on with suture anchors (P < .05), had a greater incidence of hypermobil

OBJECTIVE: MR imaging of the shoulder was performed in an asymptomatic population to determine the prevalence of MR-evident peritendinous and bone abnormalities. Findings were correlated with subject age and rotator cuff abnormalities.
SUBJECTS AND METHODS: Dual-echo T2-weighted oblique coronal MR images of the shoulder were evaluated for 100 asymptomatic volunteers who were 19-88 years old. Twenty symptomatic patients with arthroscopic correlation were included as controls. Images were assessed independently by two reviewers with no knowledge of subject history or symptomatology. Forty-seven shoulders with traumatic anterior instability were studied by magnetic resonance arthrography (MRA) and computed tomography arthrography (CTA) to compare the diagnostic performance of these examinations in the evaluation of Bankart lesions. All shoulders were examined by arthroscopy to verify the lesions. Labral damage evaluated by MRA and by CTA correlated significantly with arthroscopic findings (MRA, r = 0.55, p < 0.0001; CTA, r = 0.45, p = 0.0050). MRA possessed higher sensitivity in detecting torn labra (MRA, sensitivity = 87%, specificity = 75%; CTA, sensitivity = 33%, specif

High-velocity trauma now often results in complex injuries to the upper extremity, and especially the elbow joint. These can lead to both an enormous reduction in the range of motion of the shoulder, elbow and wrist joints, in severe cases with complete loss of upper extremity function. A complex injury is defined as a fracture and/or dislocation of the elbow in association with (1) a serial injury of the upper extremity, (2) a severe soft tissue trauma, or (3) concomitant injury to vessels or nerves. Serial fractures, in particular can lead to enormous problems with treatment and are often as

BACKGROUND: Work as a librarian involves exposure to potential risk factors for developing upper limb disorders. The prevalence of upper limb symptoms has, however, not previously been assessed in this occupational group.
AIMS: To estimate the 7-day and annual prevalence of self-reported neck and upper limb symptoms in librarians and to examine associations with specific tasks and ergonomic risk factors.
METHODS: A cross-sectional study using components of the standardized Nordic questionnaire. The study population consisted of librarians employed by a large local authority, and data collect

MRI is a useful diagnostic method for evaluating nerve disease at the shoulder and elbow. MRI can depict the normal anatomy of the nerves, confirm and identify the cause of the neuropathy, identify the site of entrapment based on muscle denervation patterns, and detect unsuspected space-occupying lesions. MRI can also narrow down the differential diagnosis of nerve disease, such as in the case of suprascapular nerve syndrome versus Parsonage-Turner syndrome, or radial tunnel syndrome versus lateral epicondylitis. Large prospective studies with surgical correlation, however, are still necessary

BACKGROUND: Neck spasms or myoclonus are an underreported sequela of HNC treatment, often described following radiation. The frequency of neck spasm following neck dissection is unknown.
METHODS: A phone survey was conducted of HNC patients following neck dissection to determine the incidence and severity of neck spasms using the Penn Spasm Frequency Scale and Neck Dissection Impairment Index. Demographic and treatment-related factors were compared between those with and without neck spasms.
RESULTS: 56/136 patients (41.2%) responded with 16 (28.6%) reporting neck spasms. Spasms ranged from

Midshaft clavicle fractures are commonly fixed with locking plates. The subclavian vein risks injury during this procedure and the consequences can be fatal. The purpose of this present study is to describe a clavicular osteotomy technique in order to equip orthopaedic surgeons with a means of rapidly accessing a subclavian vein injury. The osteotomy should only be performed following an urgent intraoperative vascular surgery assessment. There must be shared consensus from both orthopaedic and vascular surgery that direct repair of the subclavian vein is necessary, and further exposure is requ

CASE: A 31-year-old Caucasian man presented with a greater arc perilunate variant injury after falling from a friend's shoulders onto an outstretched hand. Imaging revealed a minimally displaced scaphoid waist fracture and a nondisplaced transverse fracture through a previously unrecognized lunotriquetral coalition. A volar intercalated segmental instability (VISI) deformity was present. Open reduction with osseous fixation (a headless compression screw for the scaphoid waist fracture and 3 Kirschner wires across the midcarpal joint) and repair of the torn volar ligaments partially restored th

The purpose of this study was to determine the relationship between rotator cuff (RC) tear and the orientation of the glenoid. Ninety-six shoulders (94 patients) that underwent open RC repair were grouped according to the type of tear. We measured on MRI the acromio-glenoid angle (AG) and the supraspinatus fossa glenoid angle on the anterior-posterior (SGAP) and axial (SGAX) views. RC patients had a smaller AG angle (76+/-7 degrees vs. 86+/-10 degrees ) and a larger SGAP angle (112+/-6 degrees vs. 102+/-7 degrees ) compared to controls (p<0.001). We also found a highly significant difference (

A 56-year-old woman noticed non-progressive weakness in the proximal part of the right upper extremity from her childhood. At the age of 37 years, she was diagnosed as "spinal muscular atrophy" by an orthopedic surgeon. At the age of 56, neurological examinations revealed muscular atrophy and weakness confined to the right deltoid, biceps brachii and brachioradialis together with minor sensory disturbance in the lateral side of the right shoulder and forearm without pyramidal sign in the lower extremities. The neurological features of this case differed from those of juvenile type of distal an

BACKGROUND: Lateral collateral ligament injuries may occur in patients with chronic lateral epicondylitis. The present study aimed to compare the clinical outcomes of arthroscopic debridement between patients with chronic lateral epicondylitis combined with a partial ligament injury and those without a ligament injury.
METHODS: Between 2016 and 2018, patients who underwent arthroscopic debridement for lateral epicondylitis were evaluated. Partial injury to the lateral collateral ligament was defined as discontinuity or thinning with increased signal of the lateral ligament on magnetic resonan

PURPOSE: Differences in illness perception about hand injury may partly explain the variation in health behaviours such as adherence to post-operative therapy, coping strategy, emotional response and eventual clinical outcome. This study examined the illness perception of patients with hand injuries in the acute trauma setting.
METHODS: The disability and severity of injury were assessed using the Disability of the Arm, Shoulder and Hand (DASH) questionnaire and the Hand Injury Severity Score (HISS). The revised Illness Perception Questionnaire (IPQ-R) was used to explore patients' illness be

The current study evaluated the relative correlation of apatite crystal-induced inflammation and rotator cuff deficiency in the development of cuff tear arthropathy. Thirty-seven patients with full thickness rotator cuff tears were evaluated by history, physical examination, and plain radiographs. Thirty patients had surgical intervention for their rotator cuff defects, and calipers were used intraoperatively to quantify the size of the tear in its largest diameter. The remaining seven patients were treated nonoperatively and the size of the tear was quantified using magnetic resonance imaging

BACKGROUND: Horse riding is a common sport, but no study specifically addresses joint dislocations. The purpose of this study is to describe joint dislocations due to equines using a national emergency department (ED) database. Such data can be helpful in injury prevention strategies.
METHODS: The National Electronic Injury Surveillance System (NEISS) database for the years 2000-2023 was used. The NEISS database is a statistically representative sample of injuries seen in EDs across the US giving national estimates. Mechanism of injury was tabulated into groups: 1) did the patient fall from t

BACKGROUND: Bodybuilders are uniquely at risk of shoulder injuries given training intensity, frequency, and heavy overhead loads through a full arc of motion. Latissimus tears are rare injuries that have increased incidence in overhead and weightlifting athletes, following forceful hyperabduction/adduction during sport-specific activities. Humeral avulsions of the latissimus tendon are treated with endosteal button fixation.
INDICATIONS: Endosteal button repair is indicated for acute or chronic latissimus tears up to 2 years after injury, in patients who are unable to return to their desired …

PURPOSE: A subset of patients referred to brachial plexus clinics are eventually diagnosed with conditions unrelated to the plexus. This study characterizes these individuals to provide insights into the types of patients encountered by referring clinicians and surgeons caring for brachial plexus injuries (BPIs).
METHODS: A retrospective review of all new consultations at a quaternary multidisciplinary BPI clinic over a 10-year period was undertaken. Patients were included when their final diagnosis did not involve a brachial plexus lesion.
RESULTS: Of 1,415 new patient consultations, 99 (7%

Background: In patients with a zone 1 flexor digitorum profundus (FDP) avulsion fracture, we suture the FDP tendon to the A5 or A4 pulley using an absorbable suture as an adjunct to standard surgical fixation. This minimises the proximal retraction force of the tendon during the early postoperative period. The aim of this study is to report the clinical outcomes of this approach. Methods: This is a retrospective study of all patients who underwent this technique over a 2-year period between January 2020 and December 2022. Patients aged 18-65 with isolated zone 1 FDP avulsion fracture and who c
