The Application of Surgical Navigation in the Treatment of Temporomandibular Joint Ankylosis.

OBJECTIVE: The purpose of this study was to assess the safety and the accuracy of surgical navigation technology in the resection of severe ankylosis of the mandibular condyle with the middle cranial fossa. METHODS: The computed tomography scan data were transferred to a Windows-based computer workstation, and the patient's individual anatomy was assessed in multiplanar views at the workstation. In the operation, the patient and the virtual image were matched by individual registration with the reference points which were set on the skull bone surface and the teeth. Then, the real-time naviga…

FACE-R--a 3D database of 400 living individuals' full head CT- and face scans and preliminary GMM analysis for craniofacial reconstruction.

In the past, improvements in craniofacial reconstructions (CFR) methodology languished due to the lack of adequate 3D databases that were su … rmed a preliminary geometric morphometric (GMM) study using 3D data that produces a general idea of skull and face shape correlations. The vertical position of the tip of the (soft) nose for a skull and landmarks such as rhinion need to be taken into account. Likewise, the anterior nasal spine appears to exert some influence in this regard.

Aplasia cutis congenita associated with multiple congenital anomalies: case report.

INTRODUCTION: Aplasia cutis congenita (ACC), is a rare anomaly presenting with a solitary scalp lesion of skin abrasion. Lesions can be multiple and on different surfaces of the body but are mostly seen on the scalp (%70) as a solitary lesion. As it is a rare disease, we aimed to describe our case with ACC. CASE REPORT: Our case was a newborn infant with a large full thickness skin and skull defect on the scalp at birth. He also had hypoplasia on the fingers, omphalocele, ectopic anus, cleft lip and palate anomalies and multiple cardiac anomalies. We planned further evaluation but he died 23 …

Management of unexpected cerebrospinal fluid leak during endoscopic sinus surgery.

PURPOSE OF REVIEW: Cerebrospinal fluid leak during endoscopic sinus surgery is a potentially devastating complication; however, identification and appropriate management can prevent unfavorable outcomes. The purpose of this review is to outline the recent advances in the management of cerebrospinal fluid leaks that occur during endoscopic sinus surgery. RECENT FINDINGS: The low incidence of this complication makes prospective and randomized study difficult. The expanding role for endoscopic skull base surgery has brought new techniques and advances in the treatment of skull base reconstructio…

Skeletal analysis of the Fgfr3(P244R) mouse, a genetic model for the Muenke craniosynostosis syndrome.

Muenke syndrome, defined by heterozygosity for a Pro250Arg substitution in fibroblast growth factor receptor 3 (FGFR3), is the most common genetic cause of craniosynostosis in humans. We have used gene targeting to introduce the Muenke syndrome mutation (equivalent to P244R) into the murine Fgfr3 gene. A rounded skull and shortened snout (often skewed) with dental malocclusion was observed in a minority of heterozygotes and many homozygotes. Development of this incompletely penetrant skull phenotype was dependent on genetic background and sex, with males more often affected. However, these cra…

Aneurysmal bone cyst of the sphenoid bone extending into the ethmoid sinus, nasal cavity and orbita in a child.

An aneurysmal bone cyst (ABC) typically involves the long bones of the extremities, thorax, pelvis, or vertebrae. Skull base involvement is rare. We describe the case of a 9-year-old girl with ABC of the skull base. The patient had presented with nasal obstruction and headache over a period of approximately 8 months. The patient had no history of trauma or surgery. Physical and neurological examination findings normal. Magnetic resonance imaging (MRI) showed a multicystic lesion arising from the sphenoid sinus and extending into ethmoid sinus, superior nasal cavity and medial walls of the orbi…

Hydroxyapatite cement resistant to fragmentation following full cerebrospinal fluid bathing.

Prolonged cerebrospinal fluid bathing of cranioplasty cement frequently results in breakdown of the cement implants. A 5-year-old boy with a history of severe head trauma at 2 weeks of age presented with marked protrusion of the entire superior temporal bone and inferior parietal bone. The defect was elevated by more than 1 cm and was associated with a 4.5 x 3-cm skull defect located above and behind the right ear. There also was pulsatile tissue at the depths of the defect. A computed tomographic scan taken of the head revealed an expanding skull fracture from a dural defect with underlying b…

Osteoma in the upper cervical spine with spinal cord compression.

Osteoma is a common benign tumor. It occurs dominantly at the skull bone. Outside skull osteoma is rare, and primary intra-canal osteoma is extremely rare. To the author's knowledge, only 14 cases of osteomas of the spine had been reported, in which only seven cases were in English literature. The authors reported two rare cases of intra-canal osteoma of the upper cervical spine with cord compression. Included are pertinent history, physical examination, rontgenographic evaluation before and after operation, surgical interventions, pathological study, and outcome. The available literature is a…

[Surgical management of dumbbell-shaped trigeminal neurinomas].

OBJECTIVE: To investigate the best surgical approach for the removal of dumbbell-shaped trigeminal neurinomas (DSTNs). METHODS: The clinical materials of 46 patients with DSTNs surgically treated were analysed retrospectively. The patients were divided into two groups. In the early group (1978 - 1984), a series of conventional intradural approaches were used, and in the late group (1985 - 2000), an epidural approach via the skull-base craniotomy was used. RESULTS: Total tumor removal was achieved in 42% (5/12) of the early group and 85% (29/34) of the late group (chi(2) = 8.8, P < 0.01). Tem…

Central nervous system phenotypes in craniosynostosis.

Though reduction in the number of cranial elements through loss of a suture is a recognized trend in vertebrate evolution, the premature closure of cranial sutures in humans, craniosynostosis, is considered a pathological condition. Previous research on craniosynostosis has focused primarily on the skeletal phenotype, but the intimate relationship between the developing central nervous system (CNS) and skull is well documented. We investigate the morphology of the CNS in patients with isolated craniosynostosis through an analysis of cortical and subcortical features using 3-D magnetic resonanc…

[Intracranial occlusion of the internal carotid artery after minor closed head injury].

Thrombosis of the extracranial portion of the internal carotid artery as a result of nonpenetrating head and neck injury is not uncommon. However, intracranial occlusion of the internal carotid artery after minor head and neck injury without skull fracture is rare. We report a case of 14-year-old male who suffered a minor head injury during an athletic meeting of his school and developed a right hemiparesis and a lethargy state resulting from thrombosis of the supraclinoid portion of the left internal carotid artery. On admission, skull films and a CT scan revealed no abnormality. One hour lat…

Ewing's family of tumors involving structures related to the central nervous system: a review.

This review consolidates information gleaned from several case reports and larger series on Ewing's sarcoma family of tumors (EFT) involving structures related to and found in the central nervous system (CNS). These tumors involve the skull, the spinal column, adjacent soft tissues, the meninges, and the brain. We have separated the cases by skull region and spinal column level, and we discuss the attendant differences in prognosis following treatment by neurosurgery, radiation, and chemotherapy. Light and electron microscopic features can be used to differentiate EFT from other small round bl…

Possible case of spondylo-epiphyseal dysplasia in a medieval skeleton.

Multiple anomalies of vertebral and long bone growth were observed in the incomplete skeleton of an adult male from a collection of 693 medieval skeletons excavated at Abingdon, Oxfordshire. The skeleton consists of a fragmented skull, right scapula and humerus, left humerus, radius, ulna and partial hand, vertebrae from C1 to T11, and fragments of the ribs and sternum. The bones were examined, measured, and radiographed. The humeri are abnormally short and thick, with exaggerated muscle attachments. Both shoulder joints show extensive osteoarthritic destruction. Some vertebrae show slight fla…

Postoperative computed tomography scan study of the pterygomaxillary separation during the Le Fort I osteotomy using a micro-oscillating saw.

PURPOSE: The objective of the study was to evaluate the type of pterygomaxillary separation that occurs with use of a micro-oscillating saw. PATIENTS AND METHODS: Sixteen patients underwent a postoperative computed tomography scan. RESULTS: Ideal or near-ideal separations occurred on 26 of 32 sides (81%), while low-level fractures occurred in 6 of 32 sides (19%). No high-level fractures of the pterygoid plates, or fractures extending to the base of the skull or orbit, were seen. There was a striking difference in the number of ideal separations on the right-hand side (94%), compared with the…

[Advances in the understanding of the etiopathogenesis of non syndromic craniostenoses and therapeutic approaches].

The treatment of craniostenoses has not been revolutionized over the last decade, but the methods of investigation, particularly 3D CT scan, histological studies, and refinement of techniques, have allowed substantial progress in the understanding of the aetiopathogenesis and treatment of craniostenoses. The deformities of the components of the base of the skull can now be more precisely analysed. The authors present these elements for each of the various types of craniostenosis. The role of soft tissues has been more clearly defined; the dura mater, corresponding to an internal periosteum, ha…

[Diagnosis in recurrent bacterial meningitis; a literature study].

Recurrent bacterial meningitis can be caused by different mechanisms. In order to make the diagnostics more efficient, we studied literature to find these underlying mechanisms. Two groups could be identified: group I, patients with a congenital or posttraumatic defect in the bones of the skull and group II, patients with a deficiency of a component of complement. The patients in group I were much younger at the time of their first meningitis and suffered more often from otitis, rhinorrhoea and deafness diagnosed before the first meningitis. Streptococcus pneumoniae was most frequently culture…

Radiological findings in patients with mucopolysaccharidosis I H/S (Hurler-Scheie syndrome).

The development of radiological changes in two patients with mucopolysaccharidosis (MPS) Type I H/S is described. Radiological findings reveal enlargement of the sella in one patient, impression of the basilar skull in the other patient. Sclerosis and thickening of the base of skull was observed in both patients. The mandibular necks were short with striking flattening of the superior surfaces of the condyles, when the patients reached adult age. At the age of 12, hypertelorism and spaced teeth were already present in both patients. In addition, they showed early costal, clavicular and scapula…

Hyperostosis generalisata with striations of the bones, a further report in two related families.

Hyperostosis generalisata with striations of the bones falls within the large group of sclerotic bone dysplasias and is characterised by a marked widening of cortical bone, sclerosis of the base of the skull, and dense striations in the cancellous bone. All the bones of the skeleton, with the possible exception of the vault of the skull, may be involved to some extent. A milder form exists in which thickening of cotrical bone occurs, mainly in the long bones of the limbs and striations are absent. The disorder has never been described in the female. Apart from a slight elevation of the serum a…
