Restless legs and periodic leg movements in sleep syndromes.

Restless legs syndrome (RLS) is a common sensorimotor disorder with an estimated prevalence of between 1% and 5%. The symptomatology is characterized by unpleasant sensations experienced predominantly in the legs and rarely in the arms. The symptoms occur only at rest and become more pronounced in the evening or at night. In addition, the patients suffer from a strong urge to move the limbs, typically manifest as walking around, which leads to complete but only temporary relief of the symptoms. Most of the patients with RLS have periodic leg movements (PLMS) during sleep and relaxed wakefulnes…

Ventilatory response to induced auditory arousals during NREM sleep.

Sleep state instability is a potential mechanism of central apnea/hypopnea during non-rapid eye movement (NREM) sleep. To investigate this postulate, we induced brief arousals by delivering transient (0.5 second) auditory stimuli during stable NREM sleep in eight normal subjects. Arousal was determined according to American Sleep Disorders Association (ASDA) criteria. A total of 96 trials were conducted; 59 resulted in cortical arousal and 37 did not result in arousal. In trials associated with arousal, minute ventilation (VE) increased from 5.1 +/- 1.24 minutes to 7.5 +/- 2.24 minutes on the …

[Pharyngeal pressure measurements in topodiagnosis of obstructive sleep apnea].

In order to improve the outcome of surgical treatment for obstructive sleep apnea syndrome certain reports have claimed to identify the site of obstruction in the upper airway. One of the various methods available for topodiagnosis is manometry using intraesophageal and intrapharyngeal pressure probes. In the present literature the surgical success rate for patients undergoing Uvulopalatoplasty (UPP) as treatment for retropalatal obstructions is approximately 50%. For patients with other sites of obstruction the success rate is below 10%. The aim of this study was to evaluate the different the…

Patient selection and surgical results in obstructive sleep apnea.

In recent years, therapeutic methods have been effective in the management of snoring and sleep apnea. Successful results have been possible through conservative and surgical approaches when the nature and site of obstruction is ascertained by careful investigation. Sagittal magnetic resonance imaging (MRI) of the upper respiratory tract has been the most valuable diagnostic tool in patients with obstructive sleep apnea. This has made it possible to measure the dimensions and distance of the hard and soft palate and tongue base to the posterior pharyngeal wall. Surgery is only indicated when a…

Sleep apnoea and sympathetic nervous system activity: a review.

A review was made of the literature relating sleep apnoea to sympathetic nervous system (SNS) activity, as inferred from catecholamine levels or muscle sympathetic nerve activity (MSNA). Twenty-four studies were located. Most studies reported an elevation of norepinephrine and MSNA, both during sleep and wakefulness among individuals with sleep apnoea. However, studies rarely controlled for known confounders of sympathetic activity, making the validity of the findings questionable.

Road traffic accidents in patients with obstructive sleep apnoea.

Three patients involved in road traffic accidents were suspected to have obstructive sleep apnoea (OSA). Two of them fell asleep while riding motorcycles and one patient fell asleep behind the wheel of a truck causing it to overturn. The diagnosis of OSA in each case was suspected based on a history of loud snoring, restless sleep, and excessive daytime somnolence and was confirmed by sleep studies.

Craniofacial abnormalities in obstructive sleep apnoea: implications for treatment.

Obstructive sleep apnoea (OSA) is a common disorder, and is characterized by repetitive closure of the upper airway during sleep. Upper airway narrowing and sleep-induced loss of muscle tone are important factors in the development of OSA. Over the last decade there has been a growing recognition that craniofacial abnormalities occur commonly in OSA patients. The more commonly identified abnormalities include mandibular deficiency, an inferiorly placed hyoid bone relative to the mandibular plane, a narrowed posterior air space, a greater flexion of the cranial base, and elongation of the soft …

Cephalometric analysis in patients with obstructive sleep apnea syndrome.

Cephalometry is useful as a screening test for anatomical abnormalities in patients having obstructive sleep apnea syndrome. In this study, various conventional parameters in cephalometry, such as the distance and angles determining the oropharyngeal space, and the areas of the soft palate, oral cavity, tongue and pharynx in both posterior-anterior and lateral view X-ray films, were measured before and after surgery in order to evaluate the preoperative predictability and usefulness of uvulopalato-pharyngoplasty and/or midline laser glossectomy (MLG) for the purpose of preoperative assessment …

Laser-assisted uvulopalatoplasty with Nd:YAG laser for sleep disorders.

We evaluated the apnea index (AI), the oxygen saturation above 95% (SA95), the lowest oxygen saturation (LSAT), and snoring before and after laser-assisted uvulopalatoplasty (LAUP) in 106 patients with obstructive sleep apnea syndrome (n=59) or snoring (n=47). Type 1 LAUP was performed in 42 patients and type 2 LAUP in 64 patients. A 50% or greater reduction in AI was observed in 15 patients (35.7%) who underwent type 1 LAUP and 37 patients (57.8%) who underwent type 2 LAUP. Snoring was diminished in 18 (51.4%) of 35 patients who underwent type 1 LAUP and 30 (55.6%) of 54 patients who underwen…

[Acoustic evoked potentials (AEP) in obstructive sleep apnea syndrome].

Patients with obstructive sleep apnea syndrome (OSAS) present upper airway obstruction during sleep which can be documented by electromyography. The cause of weakness in oropharyngeal muscles is still unknown. Lesions of pons and medulla oblongata have to be expected. Brainstem auditory evoked potentials (BAEP) may indicate pathological changes in these regions. Several studies described normal BAEP in OSAS-patients. Moderate forms of OSAS as well as central sleep apnea syndromes were investigated, however. In our study 20 patients (17 men, 3 women, mean age 53.9 +/- 2.1 ys) with severe OSAS (…

[Non-apneic respiratory arousal in obstructive sleep apnea syndrome].

Hypersomnia in obstructive sleep apnea syndrome (OSAS) is due to sleep fragmentation caused by arousals related to repeated episodes of apnea. Arousals may also be triggered by other non-apneic respiratory events (NAREs). The contribution of NARE to sleep disruption and nocturnal desaturation has been assessed by evaluating the polysomnograms of 58 consecutive OSAS patients. We studied 28 cases (25 men) presenting more than 5 NAREs/hour of sleep in addition to apneic events. Mean age was 48.8 +/- 14 years, mean body mass index was 38.1 +/- 10.2 kg/m2 and mean oxygen saturation at baseline, bef…

Resolution of obstructive sleep apnea after microvascular brainstem decompression.

Lesions of the brainstem have been associated with obstructive sleep apnea in previous reports. We now report a case in which retromastoid craniectomy with microvascular decompression of the medulla and ninth and tenth cranial nerves resulted in the complete resolution of severe obstructive sleep apnea. Possible mechanisms for this observation are discussed.

Respiratory sinus arrhythmia during obstructive sleep apnoeas in humans.

Respiratory sinus arrhythmia (RSA) reflects parasympathetic modulation of heart rate (HR) during the respiratory cycle. Since the time-course of RSA during obstructive sleep apnoea (OSA) is not known, an analysis was made of ECG in samples of consecutive OSA recorded in 5 patients during NREM sleep while breathing room air (OSA-AIR, mean lowest SaO2 83.0 +/- 6.5%) or supplemental oxygen (OSA +/- O2, mean lowest SaO2 91.7 +/- 2.2%), respectively. For each breath, HR at the transition from expiration to inspiration (HRei), and HR at maximal inspiration (HRie) were calculated, and the inspiratory…

Multidisciplinary approach to obstructive sleep apnoea: a case report.

Obstructive sleep apnoea is a well recognized clinical problem. This paper presents a case in which a joint orthodontic and surgical approach was used to diagnose and treat a patient suffering from this condition. Initially these cases could be treated with a conservative approach, which has a potential role in establishing a diagnosis.

EEG sleep characteristics in dysthymia and major depressive disorder.

All night EEG sleep recordings were compared in patients with dysthymia and major depressive disorder. Subjects were selected according to DSM-IV and underwent 2 weeks of treatment with placebo before the sleep evaluations. All patients with major depressive disorder were classified as nonmelancholic and belonged to the recurrent subtype, without full interepisode recovery. Patients with major depressive disorder have a shorter duration of total sleep time, a longer sleep latency and a lower sleep efficiency. However, similar sleep architecture and REM sleep characteristics were found in the t…
