Wrist drop and muscle weakness of the fingers induced by an upper cervical spine anomaly.

PURPOSE: This report presents a case of wrist drop and muscle weakness of the fingers as a false localizing sign induced by stenosis of the upper cervical spine caused by a bony anomaly. METHODS: A 77-year-old male complained of severe muscle weakness of the right hand. Cervical spine MRI showed a severe and sharp compression of the spinal cord from the dorsal side between C2 and C3 with intramedullary intensity changes and mild stenosis at C3/4 and C4/5. RESULTS: The patient underwent laminectomy of C2, cranial side laminotomy of C3, and laminectomy of C4. Decompression of the spinal cord w…

Long-term follow-up of a case of aneurysmal bone cyst in the lumbar spine.

BACKGROUND CONTEXT: Aneurysmal bone cyst (ABC) in the spine is relatively rare, so little is known about the natural history of the disease. PURPOSE: The purpose of this study was to describe a spinal ABC that was followed for an extended period from lesion enlargement to spontaneous regression. STUDY DESIGN: Case report. METHODS: A 63-year-old woman had a 1-year-long history of mild low back and bilateral leg pain without any significant neurologic deficits caused by lumbar spinal stenosis. At her first visit to our hospital, magnetic resonance imaging (MRI) showed a small cystic lesion on…

Subdural tuberculous abscess of the lumbar spine in a patient with chronic low back pain.

INTRODUCTION: Despite modern imaging methods, tuberculous abscess in the subdural space of the spine can lead to misdiagnosis and to neurogical complications development, even more up to paraplegia. We presented an extremely rare case of subdural tuberculous abscess of the lumbar (L) spine and paraparesis in imunocompetent a 49-year-old patient. CASE REPORT: A patient with chronic L syndrome and a history of intervertebral (IV) disc L3 and L5 operations got severe back pain late in July 2007. At the same time the patient had a purulent collection in the left knee, and was treated with high do…

Intradural extramedullary primary hydatid cyst of the spine: a case report and review of literature.

Primary intradural extramedullary hydatid cyst is a rare form of parasitic infection, causing focal neurological signs, commonly observed in … limbs, which had lasted for 1 month. On the basis of imaging results, arachnoid cyst of the lumbar spine was diagnosed. Due to rapid progression of the patient's symptoms toward spastic paraplegia, he underwent an emergency surgical decompression procedure. The patient underwent exploratory surgery using a posterior approach. A L1-L2 laminectomy was pe …

Accumulation and rapid decay of non-LTR retrotransposons in the genome of the three-spine stickleback.

The diversity and abundance of non-long terminal repeat (LTR) retrotransposons (nLTR-RT) differ drastically among vertebrate genomes. At one … s behave as neutral alleles. Here we examined the dynamics of amplification of nLTR-RT in the three-spine stickleback (Gasterosteus aculeatus). In this species, the vast majority of nLTR-RT insertions are relatively young, as suggested by their low level of divergence. Contrary to expectations, a majority of these insertions are fixed in lake and oceani …

Athletic cervical spine injury in the setting of fusion failure of the anterior and posterior atlas.

This article describes a rare congenital abnormality of anterior and posterior C1 fusion failure presenting after an acute athletic injury to the fibrous nonunion. C1 congenital malformations are rare, occurring in approximately 2% of patients; even rarer are combined anterior and posterior arch malformations in the same patient. Posterior ring abnormalities are more common than anterior ring injuries (4.5:1, respectively). To the authors' knowledge, combined anterior and posterior ring congenital malformations with subsequent injury have not been previously described. In the current patient,

Single-stage combined decompression for patients with tandem ossification in the cervical and thoracic spine.

STUDY DESIGN: Retrospective study of clinical outcomes of single-staged combined cervical and thoracic decompression for patients with tandem ossification (TO). OBJECTIVE: To describe primary clinical outcomes of this procedure. TO is introduced to described a double ossification lesion of the posterior longitudinal ligament (OPLL) or the ligament flavum (OLF) at the cervical, thoracic and lumbar spine. In clinical practice, cervical OPLL combined with thoracic OPLL or/and OLF are the most common types of TO. However, little is known about the clinical outcomes of surgical treatment and there…

Surgical treatment of ossification of the ligamentum flavum associated with dural ossification in the thoracic spine.

Between January 2006 and December 2009, dural ossification (DO) was found intraoperatively in 26 of 98 patients with thoracic spinal stenosis due to ossification of the ligamentum flavum (OLF). MRI showed single-level (n = 2), two-level (n = 8) and multilevel (n = 16) areas of low signal intensity in the ligamentum flavum of the thoracic spine. Of the 68 ossified segments in the study population, 11 (16.2%) were located in the upper thoracic spine (T1-T4), nine (13.2%) were located in the midthoracic spine (T5-T8), and 48 (70.6%) were located in the lower thoracic spine (T9-L1). All patients u…

Hip and spine surgery is of questionable value in spina bifida: an evidence-based review.

BACKGROUND: Although many children with spina bifida and associated scoliosis or dislocated hips undergo spine or hip surgery, the benefits are uncertain. QUESTIONS/PURPOSES: The purpose was to perform an evidence-based review on the benefits and risks of surgery for dislocated hips and scoliosis in spina bifida. METHODS: I performed a Medline(®) and Embase(®) search from 1950 to 2009 for Level I to Level III studies investigating the benefits and risks of surgery for scoliosis and hip dislocation in patients with spina bifida. When available, I extracted types of surgery, complication rates…
