Isolated lesions of the lower subscapularis tendon: diagnosis and management.

PURPOSE: Isolated lesions of the caudal portion of the subscapularis (SSC) tendon are rare and have not been subject of major attention in the literature yet. However, these lesions can lead to tremendous impairment of shoulder function. The purpose of the present study was to raise clinician's awareness for these "hidden lesions" and to present the crucial steps for diagnosis and management. METHODS: Five patients (three men, two women; mean age 40) with traumatic isolated caudal SSC lesion were enrolled in the present study. After clinical and radiological examination, all patients underwen

[Idiopathic bilateral patellar tendon rupture].

In the absence of systemic disease, specific treatment or sport tendonitis, simultaneous bilateral patellar tendon rupture is rare. Often missed on the first glance, it represents a diagnostic difficulty that should not be overlooked at the initial medical visit. The loss of active extension of the lower limb and a radiographic patella alta, even in a bilateral context, should raise suspicion of this diagnosis. It is then necessary to search for predisposing causes and to evoke the differential, or frequently associated, diagnoses. The present report illustrates these diagnostic difficulties a

Objective analysis of lateral elbow exposure with the extensor digitorum communis split compared with the Kocher interval.

BACKGROUND: The most widely used surgical approach to treat radial head fractures is through the Kocher interval. However, the extensor digitorum communis (EDC) splitting approach is thought to allow easier access to the anterior half of the radial head, which is more commonly fractured. The aim of this cadaveric study was to compare the osseous and articular surface areas visible through the EDC split and the Kocher interval. METHODS: Four approaches were used in fresh frozen cadaveric upper extremities: EDC splitting (n = 6), modified Kocher (n = 6), extended EDC splitting (n = 6), and exte

Prepatellar continuation rupture: Report of an unusual case.

In anatomical studies the deepest soft tissue layer, related to the deep rectus femoris tendinous fibers, has been described as the "prepatellar quadriceps continuation". We present an unusual case of an isolated prepatellar continuation rupture, which to our knowledge is the first described case in the literature. Injuries to the extensor mechanism may include isolated rupture of the prepatellar continuation with intact quadriceps and patellar tendons. Diagnosis may be difficult with ultrasound scan and requires MRI scan for confirmation. Appropriate clinical assessment and regular physiother

[Latissimus dorsi transfer for the treatment of irreparable craniodorsal tears of the rotator cuff].

PURPOSE OF THE STUDY: Latissimus dorsi transfer is a relatively complicated procedure subsequently requiring an active, long-term physical therapy. The surgeon's performance and a patient's effort are therefore of worth consideration. However, sutures of massive rotator cuff tears do not always produce good results. The aim of the study is to evaluate the role of latissimus dorsi transfer in the treatment of these shoulder pathologies. MATERIAL: A total of 21 patients were evaluated. Their age at the time of surgery ranged from 48 to 63 years, with an average of 54.8 years. A minimum follow-u

[Rupture of the quadriceps tendon after lateral retinaculum release by arthroscopy].

Complications after arthroscopic surgery of the knee joint are infrequent. Quadriceps tendon ruptures after knee arthroscopy are rarities. Only two cases have been published in the medical literature. This article presents a case of a quadriceps tendon rupture that occurred in a 19-year-old patient 5 weeks after lateral release of the retinaculum by arthroscopy. The late occurrence differentiates this case from the other previously published cases.

Subcutaneous tendon rupture of extensor tendons on bilateral wrists associated with calcium pyrophosphate dihydrate crystal deposition disease.

Calcium pyrophosphate dihydrate (CPPD) crystal deposition disease is a well-recognized inflammatory joint disorder. Extensor tendon rupture associated with CPPD deposition has rarely been described. We report herein the case of a 58-year-old woman who underwent reconstruction for subcutaneous extensor tendon ruptures of the extensor tendons for the ring and little fingers on both wrists associated with CPPD deposition. The significance of this case is the occurrence at an earlier age compared to previous papers and the appearance on bilateral wrists.

Arthroscopic anatomy and surgical techniques for peritrochanteric space disorders in the hip.

Disorders of the lateral or peritrochanteric space (often grouped into the greater trochanteric pain syndrome), such as recalcitrant trochanteric bursitis, external snapping iliotibial band, and gluteus medius and minimus tears, are now being treated endoscopically. We outline the endoscopic anatomy of the peritrochanteric space of the hip and describe surgical techniques for the treatment of these entities. Proper portal placement is key in understanding the peritrochanteric space and should be first oriented at the gluteus maximus insertion into the linea aspera, as well as the vastus latera

Tendofascial island flap based on distal perforators of the radial artery: anatomical and clinical approach.

The possibility of transferring vascularized tissue to restore function and to resurface large defects, together with the use of composite flaps, has led to recent advances in "one-stage" reconstructive surgical procedures. On the basis of a previous study of the blood supply of the adipofascial flap and a new study of the blood supply of the flexor carpi radialis tendon from the transfascial and direct branches of the radial artery, a fascial island flap complete with tendon was devised and used to treat four male patients who had sustained traumatic soft-tissue losses on the dorsum of the ha

'Bald trochanter' spontaneous rupture of the conjoined tendons of the gluteus medius and minimus presenting as a trochanteric bursitis.

A 66-yr-old white woman presented with progressive complaints of right lateral hip and thigh pain associated with a disabling limp without an antecedent history of trauma. Physical examination revealed localized pain over the right greater trochanter to palpation. A full pain-free range of motion of the right hip was associated with weakness in the hip abductors. The patient ambulated with a compensated right Trendelenburg gait. Subsequent magnetic resonance imaging demonstrated a trochanteric bursitis and an effusion of the hip and a full-thickness tear of the gluteus medius muscle, with both

The locked metacarpophalangeal joint: report of four cases and review of the literature.

Four cases of locked finger metacarpophalangeal joints are reported. Two of these cases were caused by entrapment of the sesamoid behind an exostosis off the second metacarpal head. The third was from subluxation of the dorsal interosseous tendon over a dorsal osteophyte on the metacarpal. The fourth was from an exostosis tethering the accessory collateral ligament. Surgical exploration using a palmar approach was the treatment of choice. All patients regained full range of motion within two months. A review of the various causes and treatment options for this unusual condition is presented.

Orthotic devices in functional treatment of ankle sprain. Stabilizing effects during real movements.

Various orthotic devices generally employed for therapy of ankle sprain were tested under functional conditions. The experimental setup comprised inversion stimuli of 20 and 30 degrees (13 subjects) while standing on a tiltplatform as well as running movements of 8 and 12 km.h-1 on a treadmill (12 subjects). Ankle joint displacements were registered together with the surface EMGs from mm. peroneus long., tibialis ant., gastrocnemius med. and vastus med. In an additional study the relation between achilles tendon angle and talar tilt was determined under static conditions. Ten subjects with chr

[Management of recurrent patellar dislocation by semitendinous muscle transfer].

After review of the pertinent literature authors report on a new tendon transfer technique not having been found in the available literature. The method has been considered suitable for the treatment of patellar dislocations due to erroneous stretching direction occurring before the end of growth. The semitendinous muscle was separated and transfosed in the tunnel drilled into the patella. By means of this technique the function of the muscle was used against the lateralization tendency of the patella. The paper presents the follow-up of 9 cases.
