[The potentials of one-dimensional ultrasonic diagnosis in severe injuries to the chest cavity].

The author demonstrates the diagnostic possibilities of an ultrasonic examination of severe injuries of the chest by a one-dimension echolocation method on the basis of experimental and clinical tests of a modernized ECHO-P apparatus and a sensor. Synonymous and repeated acoustic characteristics of lung contusions, hemo- and pneumothorax have been revealed and described. The obtained data are of practical interest with regard to express diagnosis of the complicated chest injuries at the preclinical and early clinical stages of the treatment of the patients.

[Fracture dislocation of the thoracic spine in ankylosing spondylitis. A case of spontaneous occurrence and an uncommon course].

One case of spontaneous Th 10 - Th 11 dislocation complicating ankylosing spondylitis is reported. The only functional consequences were cracks when the patient was moving. Surgical treatment was refused nevertheless after twelve year follow bone union had occurred without neurological impairment.

Blunt chest trauma and suspected aortic rupture: reliability of chest radiograph findings.

The chest radiographs of 86 patients with suspected aortic rupture from blunt chest trauma were reviewed. Seventy-three patients had no evidence of aortic rupture on aortography or surgical exploration, and 13 patients had surgically confirmed rupture. Sixteen radiographic findings were analyzed for sensitivity and specificity in detecting aortic rupture. The following findings were not statistically significant: hemothorax on either side; rib fractures on either side; pneumothorax on either side; lung contusion; widened left paravertebral stripe; and widening of the mediastinum, along with an

Blunt chest trauma with tracheobronchial rupture.

The unusual complex of physiological problems associated with rupture of the tracheobronchial tree complicating blunt chest trauma is discussed. The mechanics of injury leading to rupture are abrupt compression of the chest with consequent fixation of the cervical trachea where it enters the mediastinum, separation of the lungs, and fracture of the bronchus over the vertebral bodies. Treatment by aspiration thoracentesis, tracheotomy, and thoracotomy with primary repair as well as expectant therapy are discussed. Two cases, one treated expectantly and one treated by thoracotomy, both with comp

[Post-traumatic rupture of the pericardium with cardiac luxation associated with-myocardial infarction, and a rupture of the aortic isthmus (author's transl)].

A case of rupture of the pericardium with cardiac luxation is reported. Violent trauma is necessary to produce such lesions, and this explains the frequency of rapid death before the pericardium rupture is recognised. A definite diagnosis is only possible during operation in one out of two cases that survive. This lesion should be suspected in all cases of chest injury, and complementary examinations, particularly radiological and electrocardiographic, should be requested. Treatment is surgical.

Clinical studies on the mucolytic effect of mesna.

Eighty intensive care patients requiring mucolytic therapy because of pulmonary mucus retention after chest (28 cases) or thoracic (52 cases) surgery were given the drug mesna by three different methods: bronchial lavage with a mixture of mesna (5% to 10%) and lidocaine (1%) in 15 to 20 ml, 10 to 20 times a day (10 cases); instillation into the bronchial tree of 15 to 20 ml of a 5% to 10% mesna solution three to five times a day (20 cases); continuous aerosolization of four to five ampules of mesna per 24 hours with a Bennett nebulizer (50 cases). The duration of mesna therapy ranged from 2 to

Pneumothorax of late onset after chest stabbings.

Pneumothorax is a well known immediate complication of penetrating chest injuries, but its late occurrence is not widely recognized. In a retrospective study of 56 patients who sustained stab wounds of the chest, 6 (10 per cent) developed pneumothorax between twelve hours and three days after injury. Combined thoraco-abdominal stab wounds were present in three patients and surgical emphysema was also apparent in three other patients. We conclude that pneumothorax of late onset may be commoner than has been suspected and this emphasizes the need for observation in hospital for 48 h and for seri

An analysis of conservative (non-surgical) management of thoracolumbar fractures and fracture-dislocations with neural damage.

The cases of thirty-four patients with thoracolumbar lesions were reviewed. The neural recovery rate was substantially the same as in comprehensive reviews of surgical and conservative methods by Dickson et. al. and Burke and Murray. We suggest that reduction and maintenance of vertebral body displacement is the single most important factor in the treatment of such fractures and fracture-dislocations. The loss of correction of an angular deformity, however, does not prejudice neural recovery or physical performance during and after rehabilitation. The results of non-surgical methods in achievi

[Emergency treatment in chest trauma (author's transl)].

The measures taken by the emergency physician can in many cases decisively affect the subsequent course of a chest trauma. In cases where the injured person has to be extricated from collapsed structures the medical officer has to decide on the mode and sequence of the rescue operation, prevent further damage and initiate shock therapy, intubation and artificial ventilation while the rescue operation is in progress. Emergency treatment also includes infusion therapy, if necessary via a central venous catheter; intubation, suction drainage and artificial ventilation; evacuation of fluid from th

[Pneumothorax and haemothorax (author's transl)].

Pneumothorax and haemothorax constitute serious complications of a blunt chest trauma. During 1970-1978 64 persons with pneumothorax and 109 cases of haemothorax were admitted to and treated in the Surgical Unit of the University Hospital, Göttingen. In nearly every case fracture of several ribs had also occurred. The method of choice for treating cases of pneumothorax was by Bülau drainage; surgical closure of the air leakage was rarely necessary. Conservative measures are indicated only if the pneumothorax is narrow or confined to the apical region. It is worth mentioning that in a large num

[Surgical aspects of open and closed chest injuries (author's transl)].

263 persons with chest injuries were treated over a period of 16 years. 47 had performating chest injuries, in the remaining cases it was a blunt trauma. Prompt recognition of a life-endangering situation (cardiac tamponade, tension pneumothorax, mediastinal emphysema, massive haemothorax) is essential; radiological and laboratory diagnostic methods play a secondary role. The most effective emergency treatment is intercostal continuous suction; in many cases it is the only one needed. A haemothorax must be evacuated completely because of the risk of complications and fibrin formation. Thoracot

Flail chest as a complication of cardiopulmonary resuscitation.

Records of all patients who developed flail chest after cardiopulmonary resuscitation at Rochester Methodist Hospital between January, 1966 and March 1976 were reviewed. Also, for comparison, records of patients with flail chest resulting from motor vehicle accidents and those of a matched group of patients who underwent cardiopulmonary resuscitation without developing flail chest were reviewed. The incidence of flail chest after cardiopulmonary resuscitation was about 5.6 per 100 survivors. The groups who did and did not have flail chest after cardiopulmonary resuscitation were alike in age a
