Early surgical site infection after geriatric trauma surgery: mortality, complications, and factors associated with mortality and infection persistence in patients older than 75 years.

PURPOSE: Early surgical site infection (SSI) after orthopedic trauma surgery remains a severe postoperative complication associated with increased morbidity and mortality. However, data specifically focusing on geriatric trauma patients remain limited. The aim of this study was to evaluate mortality, infection control, and postoperative complications associated with early SSI in patients older than 75 years, and to identify factors associated with mortality and infection persistence. METHODS: A retrospective monocentric observational cohort study was conducted in an academic orthopedic trauma

Mapping social work scholarship in child and adolescent mental health: a bibliometric analysis of Web of Science and Scopus, 2000-2025.

BACKGROUND: Child and adolescent mental health care depends on coordination among pediatric services, mental health care, schools, child welfare, family support, and community systems. Social work occupies a boundary-spanning position in this landscape, yet the organization and development of the relevant scholarship remain unclear. METHODS: Records published between 2000 and 2025 were retrieved from the Web of Science Core Collection and Scopus. The search captured literature explicitly identified as psychiatric, mental health, or clinical social work together with broader social work schola

Intentions Matter: Long-Term Patient Outcomes After Intentional, Interpersonal Injury.

INTRODUCTION: Any traumatic injury can lead to long-term challenges with physical health, mental health, and social well-being. Intentional injury due to interpersonal violence can further impair patients' sense of safety and may hinder recovery. We compared long-term patient-reported outcomes according to injury intent with the hypothesis that mental health outcomes would be worse in those injured by interpersonal violence compared to unintentional injury. METHODS: Beginning in 10/2019, we prospectively collected data on patient demographics, post traumatic stress disorder (PTSD), and patien

To validate the F-AST score as a CT decision tool in pediatric blunt abdominal trauma: A prospective diagnostic accuracy study.

INTRODUCTION: Focused assessment with sonography for trauma (FAST) is a valuable tool in managing adult trauma patients but has poor utility in children due to its poor sensitivity. Multiple studies have explored the addition of liver enzymes to improve the diagnostic accuracy of FAST in pediatric blunt abdominal trauma (BAT) patients. A retrospective study at our institute used FAST findings with liver enzymes (F-AST score) to rule out Intra-Abdominal Injury (IAI) in pediatric BAT patients, concluding that normal liver enzymes and a negative FAST scan can effectively rule out IAI. This study

Multicenter study evaluating target attainment of anti-Factor Xa levels using various enoxaparin prophylactic dosing practices in adult trauma patients.

STUDY OBJECTIVE: Enoxaparin is standard of care for venous thromboembolism (VTE) prophylaxis in adult trauma patients, but fixed-dose protocols are suboptimal. Dosing based on body mass index (BMI) or total body weight (TBW) improves target prophylactic anti-Xa level attainment and reduces VTE rates. A novel strategy using estimated blood volume (EBV) may be more effective based on results of a single-center study. This study compared BMI-, TBW-, EBV-based, and hybrid enoxaparin dosing strategies at achieving target prophylactic anti-Factor Xa (anti-Xa) levels in trauma patients. DESIGN: Mult

Horse-related injury patterns: a single center report.

BACKGROUND: For ages, humankind and horses have been closely related to occupational and recreational activities. The dangers of engaging with horses have been previously reported. Among sporting activities, horse riding is well-known for its risks. Despite multiple recommendations to wear protective gear, horse-related activities still comprise the risk of severe injuries. This study aimed to examine: (1) if specific mechanisms are correlated to particular injury patterns and (2) if injury types are related to patient demographics. METHODS: From one level I trauma center, between July 2019 a

Traumatic cerebral dural sinus vein thrombosis/stenosis in pediatric patients-is anticoagulation necessary?

INTRODUCTION: Cerebral dural vein thrombosis/stenosis (CDVT/S) is a condition that affects the venous drainage of the brain. Risk factors and causes associated with CDVT/S include systemic risk factors that cause hypercoagulability, or local factors such as head trauma. While consensus is that non-traumatic sinus vein thrombosis should be treated with anticoagulation therapy, treatment of patients with TBI-induced CDVT is not yet established. METHODS: Retrospective review of clinical data of pediatric patients presented to our medical center from July 2017 to August 2020. Inclusion criteria w

Association of Prehospital Plasma Transfusion With Survival in Trauma Patients With Hemorrhagic Shock When Transport Times Are Longer Than 20 Minutes: A Post Hoc Analysis of the PAMPer and COMBAT Clinical Trials.

IMPORTANCE: Both military and civilian clinical practice guidelines include early plasma transfusion to achieve a plasma to red cell ratio approaching 1:1 to 1:2. However, it was not known how early plasma should be given for optimal benefit. Two recent randomized clinical trials were published, with apparently contradictory results. The Prehospital Air Medical Plasma (PAMPer) clinical trial showed a nearly 30% reduction in mortality with plasma transfusion in the prehospital environment, while the Control of Major Bleeding After Trauma (COMBAT) clinical trial showed no survival improvement.

Risk Factors Analysis for the Outcome of Indirect Traumatic Optic Neuropathy With Steroid Pulse Therapy.

BACKGROUND: Although recent evidence suggests a controversy effect of steroids in the management of indirect traumatic optic neuropathy (TON), steroid pulse therapy remains one of the reasonable treatments for patients with indirect TON. It is thought that microcirculatory spasms, edema, and nerve cell necrosis can be prevented or reduced by large doses of steroids. The aim of this study is to analyze the predisposing factors for the recovery of patients with indirect TON who were treated with steroid pulse therapy. MATERIALS AND METHODS: In tracing the 2008 to 2014 data from the Trauma Cente

The effects of alcohol on coagulation in trauma patients: interpreting thrombelastography with caution.

BACKGROUND: The effects of alcohol on coagulation after trauma remain unclear. In vitro studies show that alcohol may decrease clot strength and inhibit fibrinolysis. Observational data indicate that alcohol leads to altered thrombelastography (TEG) parameters indicative of impaired clot formation. Clinical studies have been inconclusive to date. METHODS: Longitudinal plasma samples were prospectively collected from 415 critically injured trauma patients at a single Level 1 trauma center and were matched with demographic and outcome data. Citrated kaolin TEG and standard coagulation measures

Central venous pressure versus pulmonary artery catheter-directed shock resuscitation.

Previously, we developed a protocol for shock resuscitation of severe trauma patients to reverse shock and regain hemodynamic stability during the first 24 intensive care unit (ICU) hours. Key hemodynamic measurements of cardiac output and preload were obtained using a pulmonary artery catheter (PAC). As an alternative, we developed a protocol that used central venous pressure (CVP) to guide decision making for interventions to regain hemodynamic stability [mean arterial pressure (MAP) >or= 65 mmHg and heart rate (HR) <or= 130 bpm]. Either protocol was available and required for traumatic shoc

Does routine serial computed tomography of the head influence management of traumatic brain injury? A prospective evaluation.

BACKGROUND: Computed tomography (CT) of the head is the current standard for diagnosing intracranial pathology following blunt head trauma. It is common practice to repeat the head CT to evaluate any progression of injury. Recent retrospective reviews have challenged the need for serial head CT after traumatic brain injury (TBI). This study intends to prospectively examine the value of routine serial head CT after TBI. METHODS: Consecutive adult blunt trauma patients with an abnormal head CT admitted to an urban, Level I trauma center from January 2003 to September 2003 were prospectively stu

The impact of pelvic and lower extremity fractures on the incidence of lower extremity deep vein thrombosis in high-risk trauma patients. Winner of the Best Paper Award from the Gold Medal Forum.

Lower extremity fractures (LEFx) and pelvic fractures (PFx) are believed to increase the risk of lower extremity deep vein thrombosis (LEDVT). We studied trauma patients at high risk for LEDVT to determine whether an increased incidence of LEDVT was associated with LEFx and/or PFx. From January 1995 through December 1997 4163 trauma patients were admitted to our Level I trauma center. One thousand ninety-three patients at high risk for LEDVT were screened with serial lower extremity venous duplex ultrasound. Their medical records were retrospectively reviewed for demographics, mechanism of inj

Routine prophylactic vena cava filtration is not indicated after acute spinal cord injury.

BACKGROUND: Acute spinal cord injury (SCI) is a devastating problem, with over 10,000 new cases annually. Pulmonary embolism (PE) is a well-recognized risk in SCI patients, although no clear recommendations for prophylaxis exist. We therefore evaluated whether routine placement of prophylactic inferior vena cava filters is indicated in SCI patients. METHODS: The trauma registry of a regional trauma center was used to identify patients sustaining acute SCI resulting in tetraplegia or paraplegia after blunt or penetrating trauma for a 5-year period beginning in January 1995. Patients were analy

Chest injuries in childhood.

Differences in anatomy and mechanisms of injury are believed to contribute to the unique response of children to thoracic trauma. To characterize the scope and consequences of childhood chest injury, we reviewed the records of 105 children (ages 1 month to 17 years, mean 7.6 years) with chest injuries admitted to a level I pediatric trauma center from 1981 to 1988. Nearly all injuries (97.1%) were due to blunt trauma, and more than 50% were traffic related. Rib fractures, commonly multiple, and pulmonary contusions occurred with nearly equal frequency (49.5% and 53.3%, respectively), followed

Economic evaluation of open vs endovascular repair of blunt traumatic thoracic aortic injuries.

OBJECTIVES: During the last decade, endovascular repair (EV) has replaced open surgical repair (OSR) as the preferred method of treatment of blunt traumatic thoracic aortic injuries (BTAIs) at many trauma centers. This has resulted in reductions in mortality, length of stay, and major complications, including paraplegia, with the added expense of the initial endograft, subsequent surveillance, and reinterventions. The purpose of this study was to conduct an economic evaluation comparing these two methods of repair. METHODS: We performed an economic comparison of EV and OSR for the treatment o…
