[Ocular manifestations in sarcoidosis].

PURPOSE: The ophthalmic examination of patients with diagnosed systemic sarcoidosis. MATERIAL AND METHODS: 33 patients (17 women, 16 men), … (nodular infiltrations of the eyelids, chronic uveal inflammation, signs of anterior and posterior uveitis in the past, optic disc oedema). In 3 cases ocular lesions preceded the signs of systemic sarcoidosis. This emphasizes the importance of the routine ophthalmic examination of patients with suspected or proven sarcoidosis.

[Disclosure of Behcet disease by concomitant dental infection].

BACKGROUND: Behçet's disease can be aggravated by chronic infection. Our case illustrates how the disease can be disclosed during an acute activation of a chronic infectious focus. CASE REPORT: A 40-year-old man had Behçet's syndrome associating fever, bipolar aphtosis, cutaneous pseudo folliculitis, anterior uveitis, and meningitis following an acute episode of a dental infection, with periapical granuloma. Antibiotic therapy was ineffective in calming this first flare-up of Behçet's disease. Corticosteroid and colchicine therapy were effective. DISCUSSION: Clinical and experimental work wo…

Interstitial keratitis as presenting ophthalmic sign of sarcoidosis in a child.

Interstitial keratitis is a rarely occurring sign in sarcoidosis. We report a case of childhood sarcoidosis with initial ocular presentation as interstitial keratitis. Erythema nodosum preceded the ocular findings by 4 years and arthritis developed 1 year after ocular findings developed. Although no history of intraocular inflammation was present, there was evidence of inactive chorioretinitis and vitritis found at initial presentation. Systemic steroids were used to treat the patient's skin lesions and topical steroids to treat his corneal inflammation. He developed posterior uveitis and opti…

[The risk factors of proliferative vitreoretinopathy after operation of retinal detachment].

160 failure cases (160 eyes) following retinal reattachment surgery were reviewed. 62 cases with proliferative vitreoretinopathy were divided into proliferative vitreoretinopathy (PVR) group, the other cases (98 cases) without PVR changes were divided into NO PVR group. The clinical characteristics of two groups were compared in order to determine the risk factors in the development of server PVR after surgical repair of retinal detachment. The study shows that retinal detachment with uveitis and low tension, preoperative and post-operative vitreous hemorrhage, multiple operations were the hig…

Endophthalmitis caused by exogenous nocardial infection in a patient with Sjögren's syndrome.

A 60-year-old woman receiving prednisolone therapy for Sjögren's syndrome presented with corneal perforation. Therapeutic penetrating keratoplasty was performed, but no causative organism was identified. Focal inflammatory infiltrates in both donor and host cornea and anterior uveitis developed postoperatively. Five months later the inflammation rapidly became much worse, with fulminating abscesses, and the eye was eviscerated because of severe endophthalmitis. Nocardia organisms were identified in the cornea, conjunctiva and vitreous by means of acridine orange and modified Ziehl-Neelsen stai…

Acute multifocal hemorrhagic retinal vasculitis.

The authors present a series of seven patients with acute visual loss associated with mild anterior uveitis, multifocal retinal vasculitis, retinal capillary nonperfusion, retinal hemorrhage, disc swelling, and vitreitis. Oral prednisone was of some benefit in these patients and oral acyclovir was generally ineffective. Neovascular complications including retinal, disc, choroidal, and iris new vessels were common, requiring photocoagulation in five patients. Horseshoe tears developed in two patients in zones of uninvolved retina but retinal detachment did not occur. The etiology remains unknow…

Vidarabine therapy of complicated herpes simplex keratitis.

Patients with active herpetic epithelial keratitis who had toxic reactions or were resistant to idoxuridine received vidarabine. Only one of 35 cases of herpetic epithelial keratitis without stromal disease failed to heal. Of 21 patients with active epithelial keratitis complicating stromal keratitis or uveitis. 11 had complete reepithelialization by day 14. Two patients were removed from the trial as treatment failures. The remaining cases healed in 21 to 48 days. In most instances the stromal keratitis was inactivated when the epithelial ulcer healed. In our patients treated with vidarabine,…

CARBONIC ANHYDRASE INHIBITORS FOR MANAGING UVEITIC MACULAR EDEMA: A Systematic Review and Meta-Analysis.

PURPOSE: Uveitic macular edema (UME) is a leading cause of vision loss in uveitis. Although anti-inflammatory and immunosuppressive agents remain the mainstay of UME treatment, their use is often limited by corticosteroid-related side effects and the high cost of immunosuppressive therapies. This review aims to summarize the current evidence on carbonic anhydrase inhibitors as a potential treatment option for UME. METHODS: Electronic searches were conducted in PubMed/MEDLINE, Scopus, Google Scholar, Sage Journals, EBSCOhost, and the Cochrane Library for interventional and observational studie…

Noninfectious Posterior Uveitides - Atypicals, Variants, and Masquerades: The Jungle of Differential Diagnosis.

PURPOSE: Consensus efforts by the Multimodal Imaging in Uveitis (MUV) Task Force have established standardized diagnostic criteria for the major non-infectious posterior and panuveitides (NIPUs), historically referred to as "white dot syndromes". Nevertheless, a substantial proportion of cases deviate from classical presentations and fall into diagnostic "grey zones", blurring boundaries between diseases entities and complicating both differential diagnosis and management. This paper aims to describe the broad spectrum of atypical, variant, and secondary forms of NIPUs as well as masquerade sy…

Uveodermatologic syndrome presenting with concurrent aseptic meningoencephalitis in a dog.

Canine uveodermatologic syndrome (UDS) resembles human Vogt-Koyanagi-Harada syndrome but typically lacks associated neurological signs. We report a 3-year-old Australian shepherd dog diagnosed with UDS following bilateral enucleation and histopathology. Twelve days postoperatively, neurological symptoms developed, including head tilt, circling, and deafness. Brain MRI and CSF analyses were consistent with meningoencephalitis of unknown etiology (MUE). Oral immunosuppressive therapy (prednisone and cyclosporine) resulted in substantial improvement. Eight months later, during gradual dose reduct

Tofacitinib, a suppressor of NOD2 expression, is a potential treatment for Blau syndrome.

INTRODUCTION: Blau syndrome is a rare autosomal dominant autoinflammatory granulomatous disease caused by a mutation in the NOD2 gene. It is characterized by a clinical trial of granulomatous dermatitis, arthritis, and uveitis. Tofacitinib is a pan Janus kinase (JAK) inhibitor used for treatment of Blau syndrome and idiopathic sarcoidosis. Here, we evaluated its effect on inflammatory pathways associated with Blau syndrome. The effect of tofacitinib on downstream pathways regulated by mutant NOD2 was analyzed using luciferase assays with overexpression of NOD2 mutants. METHODS: The effect of …

Successful Treatment Bilateral Panuveitis with Multiple Systemic Infection in HIV/AIDS Patient: A Case Report.

There is an increasing number of HIV/AIDS patients in Indonesia, starting from <0.1% in 2010 to 0.4% in 2012, which warrants awareness of ocular manifestation in HIV. This might appear in 70-100% of patients with HIV. A 47 years old man came to the infection and immunology clinic with blurry vision on both eyes. He had been treated before but there was no clinical improvement. Examination showed both eyes had vitreous haziness. Visual acuity was 1/60 in both eyes with appearance of flare and cells within +3. Uveitis workup showed positive results for HIV, HSV and syphilis. Patient was given 10…

Exploring choroidal angioarchitecture in health and disease using choroidal vascularity index.

The choroid is one of the most vascularized structures of the human body and plays an irreplaceable role in nourishing photoreceptors. As such, choroidal dysfunction is implicated in a multitude of ocular diseases. Studying the choroid can lead to a better understanding of disease pathogenesis, progression and discovery of novel management strategies. However, current research has produced inconsistent findings, partly due to the physical inaccessibility of the choroid and the lack of reliable biomarkers. With the advancements in optical coherence tomography technology, our group has developed

[Recommendations for the use of methotrexate in patients with juvenile idiopathic arthritis].

OBJECTIVES: To develop a consensus document of recommendations for the use of methotrexate (MTX) in patients with juvenile idiopathic arthri … specific risk factors. Treatment should be initiated with a dose of 10-15 mg/m(2)/week. In cases of uveitis or polyarthritis, an initial dose of 15 mg/m(2)/week should be considered. For a better bioavailability and tolerability, it is preferable to administer MTX parenterally if the dose is ≥15 mg/m(2)/week. It is necessary to periodically perform an ana …

Therapy of ocular Behçet disease.

Ocular involvement in Behçet disease (BD) is characterized by recurrent inflammatory attacks and spontaneous resolution of acute inflammatory signs. Both frequency and severity of uveitis attacks determine the magnitude of irreversible damage to intraocular structures and long-term visual prognosis. Recurrent attacks of occlusive retinal vasculitis lead to vision-threatening complications such as cystoid macular edema, retinal neovascularization, optic atrophy and retinal atrophy. This manuscript updates about the role of various drugs in the management of BD, discussing corticosteroids, disea…
