Choosing the tool for osteoporosis risk prediction.

PURPOSE OF REVIEW: Predicting fracture risk is a major challenge because it allows the prevention of major osteoporotic fracture in high-risk populations. With the aging of the population, this matter will become of even greater importance. In recent years, novel clinical, biochemical, and imaging tools have been developed to improve the assessment of fracture risk. RECENT FINDINGS: The present review summarizes novel clinical strategies, Dual energy X-ray absorptiometry (DXA)-derived tools, imaging techniques, and biochemical markers that have been developed recently to improve fracture risk

Bone calcium/phosphorus ratio determination using dual energy X-ray method.

Non-invasive dual energy methods have been used extensively on osteoporosis diagnosis estimating parameters, such as, Bone Mineral Density (BMD) and Bone Mineral Content (BMC). In this study, an X-ray dual energy method (XRDE) was developed for the estimation of the bone Calcium-to-Phosphorous (Ca/P) mass ratio, as a bone quality index. The optimized irradiation parameters were assessed by performing analytical model simulations. X-ray tube output, filter material and thickness were used as input parameters. A single exposure technique, combined with K-edge filtering, was applied. The optimal

Urine melatonin levels are inversely associated with sarcopenia in postmenopausal women.

OBJECTIVE: Mainly secreted in the pineal gland, melatonin is a neurohormone that has versatile functions, including cellular protection. Decreased melatonin levels are associated with the aging process and age-related conditions. Melatonin may be associated with sarcopenia because it is one of the most common age-related diseases and because disrupted cellular vitality in skeletal muscle is known to be involved in its pathogenesis. Therefore, we investigated the relationship between first-morning-urine 6-sulfatoxymelatonin (aMT6s) levels and sarcopenia in Korean postmenopausal women. METHODS:

Impact of vitamin D, bisphosphonate, and combination therapy on bone mineral density in kidney transplant patients.

BACKGROUND: Osteoporosis can develop and become aggravated in kidney transplant patients; however, the best preventive options for post-transplantation osteoporosis remain controversial. METHODS: We retrospectively analyzed cohort of 182 renal transplant recipients of mean age 46.7 ± 12.1 years including 47.3% women. Seventy-three patients received neither vitamin D nor bisphosphonate after transplantation (group 1). The other patients were classified into the following 3 groups: calcium plus vitamin D (group 2; n = 40); bisphosphonate (group 3; n = 18); and both regimens (group 4; n = 51). B

Determining the precision of dual energy x-ray absorptiometry and bioelectric impedance spectroscopy in the assessment of breast cancer-related lymphedema.

BACKGROUND: The composition of breast cancer-related lymphedema (BCRL) has been shown to evolve from the initial accumulation of fluid to the development of fibrotic lesions and abnormal fat deposition. Therefore, precise and reliable assessments of BCRL are required to develop accurate staging and management. Although dual energy x-ray absorptiometry (DXA) and bioelectric impedance spectroscopy (BIS) have been used to assess BCRL, no study has evaluated the precision of these two modalities in the same cohort. METHODS AND RESULTS: We determined the precision of DXA and BIS in lymphedematous

Effect of number of pregnancies on bone mineral density.

This study investigated the effect of the number of pregnancies on bone mineral density (BMD) in 320 post-menopausal women aged between 49 and 64 years. Cases were selected from people with similar lifestyles and stratified into five groups according to the number of pregnancies. Each group was divided into two subgroups according to the duration of the menopause (3-5 years versus 6-10 years). Lumbar (L2-L4) and femoral neck BMD were measured by dual-energy X-ray absorptiometry and T-score values were used in the statistical evaluation. No significant difference was found between the number of

A preliminary investigation of the relationship between the "moment of resistance" of the canine spine, and the frequency of traumatic vertebral lesions at different spinal levels.

The aims of this work were to re-evaluate the moments of resistance (MoRs) of the vertebral column proposed by Slijper (1946) in the dog and to study the relationship between the MoRs and the frequency distribution of vertebral traumas in the same species. The spine (C2-L7) of four dogs was measured to obtain MoRs and subsequently scanned with a dual-energy X-ray absorptiometer (DXA). These findings were then analysed retrospectively the frequency distribution of canine vertebral fractures and luxations, diagnosed radiographically at a referral practice. The highest percentage of vertebral inj

A new technique for precisely and accurately measuring lumbar spine bone mineral density in mice using clinical dual energy X-ray absorptiometry (DXA).

Dual Energy X-ray Absorptiometry (DXA) is effective in measuring bone mineral density (BMD) in mice for early detection of osteoporosis. However, scanners designed for use with small animals (i.e. PIXImus) are very expensive. Used human DXA machines are cheaper to obtain, but analysis of scans from these instruments is operator-dependent. Obtaining reliable data depends on having a single operator analyze the scans in a blinded fashion. Scan quality is improved by excising the bone prior to scanning, which does not allow serial measurements. This study describes a novel method of analyzing lum

Utility of bone densitometry in diagnostic evaluation and monitoring in regional migratory osteoporosis.

Regional migratory osteoporosis (RMO) is a rare, self-limited disease of unknown etiology, characterized by migratory arthralgia of the weight-bearing joints of the lower extremities. RMO is most commonly seen in middle-aged adults, particularly men. Several case reports suggest that bone changes seen in RMO may be detected as focal decreases in bone mineralization, but the utility of serial bone density studies has yet to be formally evaluated in this syndrome. We describe a case of RMO with dramatic focal changes in bone mineral density that corresponded to the underlying clinical course. Se

Coeliac disease presenting with bilateral fibular stress fractures.

We present the case of an apparently healthy 22-year-old female who presented with atraumatic stress fractures of both fibulae. Further investigation demonstrated that she had osteopoenia secondary to occult coeliac disease. The fractures were successfully treated non-operatively. The awareness of the prevalence of occult coeliac disease is increasing due to simpler diagnostic tests, as is its significance as a cause of secondary osteoporosis. Knowing the relationship between these two conditions will help orthopaedic surgeons treating such patients to refer them promptly to appropriate specia

[Relationship between bone mineral content and growth disorders in children with juvenile idiopathic arthritis].

UNLABELLED: In chronic inflammatory processes in children efforts are made to evaluate bone mineral content (BMC) with the use of densitometric parameters, which at the same time determine equivalent of lean body mass (LBM). This functional analysis of the musculoskeletal system by using DXA method seems to be particularly useful for the examination of bone mass in children suffering from juvenile idiopathic arthritis (JIA). THE AIM OF THE STUDY was to assess body mass content having regard of the BMC/LBM ratio in JIA children, depending on the degree of growth inhibition, disease advancement

Bone mineral density in female high school athletes: interactions of menstrual function and type of mechanical loading.

During adolescence, skeletal integrity of girls is largely dependent on menstrual function and impact exercise, yet currently there is limited research regarding the interaction between menstrual status and type of mechanical loading associated with various high school sports. Our purpose was to examine associations of menstrual status, type of mechanical loading, and bone mineral density (BMD) in female high school athletes participating in high/odd impact or repetitive/non-impact sport. Participants were 161 female high school athletes (15.7+/-1.3 years; 165.3+/-6.9 cm; 59.4+/-8.7 kg) repres

A review of strontium ranelate and its effect on DXA scans.

Strontium ranelate is a new orally administered agent for the treatment of women with postmenopausal osteoporosis that reduces the risk of vertebral and nonvertebral fractures. This review article examines the evidence for the antifracture efficacy and safety of strontium ranelate treatment and discusses the effect of DXA scans, biochemical markers of bone turnover, and bone histology. In the SOTI trial, three years treatment with strontium ranelate led to a 41% reduction in vertebral fracture risk (relative risk [RR]=0.59; 95% CI: 0.48-0.73; p<0.001), while in the TROPOS study there was a 16%

Postpartum osteoporosis associated with proximal tibial stress fracture.

A 33-year-old woman presented with acute nonspecific knee pain, 6 months postpartum. MR imaging, computed tomography and radiography were performed and a proximal tibia plateau insufficiency fracture was detected. Bone densitometry demonstrated mild postpartum osteoporosis. To our knowledge these findings have not been described in this location and in this clinical setting. The etiology of the atraumatic fracture of the tibia is presumed to be due to a low bone mineral density. The bone loss was probably due to pregnancy, lactation and postpartum hormonal changes. There were no other inciting

The use of calcaneal quantitative ultrasound for determining bone mass of the hip.

The calcaneal quantitative ultrasound (QUS) measures heel bone mineral density and predicts hip, vertebral spine, and peripheral fractures at the same odds ratio as dual X-ray absorptiometry (DXA). Its portability, performance ease, and low cost makes it an attractive alternative as a screening device, especially in regions with low incomes and high hip fracture rates. The high hip fracture incidence in our area prompted a pilot study comparing the calcaneal QUS and hip DXA in women We found this technology to be only 58% sensitive and 80% specific when compared to the DXA in detecting hip ost
