Surgical treatment of winged scapula.

Injuries to the long thoracic and spinal accessory nerves present challenges in diagnosis and treatment. Palsies of the serratus anterior and trapezius muscles lead to destabilization of the scapula with medial and lateral scapular winging, respectively. Although nonoperative treatment is successful in some patients, failures have led to the evolution of surgical techniques involving various combinations of fascial graft and/or transfer of adjacent muscles. Our preferred method of reconstruction for serratus anterior palsy is a two-incision, split pectoralis major transfer without fascial graf

Role of the sternocleidomastoid muscle flap in modified radical neck dissection: A retrospective single-institution series of 32 patients.

BACKGROUND/OBJECTIVE: The sternocleidomastoid (SCM) muscle flap technique was incorporated into modified radical neck dissection (MRND-SCM flap) to reduce traction injury to the spinal accessory nerve (SAN), improve surgical visualization, protect the carotid artery, and maintain postoperative neck contour. This study aimed to evaluate the feasibility and safety of this approach in patients with N2-N3 head and neck cancer. METHODS: Thirty-two patients with clinically N2-N3 head and neck cancer underwent MRND-SCM flap at a single tertiary institution between May 2017 and December 2022 (34 tota

Levator scapulae and rhomboid transfer for paralysis of trapezius. The Eden-Lange procedure.

Spinal accessory nerve palsy leads to painful disability of the shoulder, carrying an uncertain prognosis. We reviewed the long-term outcome in 16 patients who were treated for pain, weakness of active elevation and asymmetry of the shoulder and the neck due to chronic paralysis of the trapezius muscle, as a result of nerve palsy. Of four patients who were treated conservatively, none regained satisfactory function, although two became pain-free. The other 12 patients were treated operatively with transfer of the levator scapulae to the acromion and the rhomboid muscles to the infraspinatus fo

Cost-Effectiveness Analysis of Combined Dual Motor Nerve Transfers versus Alternative Surgical and Nonsurgical Management Strategies to Restore Shoulder Function Following Upper Brachial Plexus Injury.

BACKGROUND: Restoration of shoulder function is an important treatment goal in upper brachial plexus injury (UBPI). Combined dual motor nerve transfer (CDNT) of spinal accessory to suprascapular and radial to axillary nerves demonstrates good functional recovery with minimal risk of perioperative complications. OBJECTIVE: To evaluate the cost-effectiveness of CDNT vs alternative operative and nonoperative treatments for UBPI. METHODS: A decision model was constructed to evaluate costs ($, third-party payer) and effectiveness (quality-adjusted life years [QALYs]) of CDNT compared to glenohume

Endoscopic Repair of the Proximal Hamstring.

BACKGROUND: Proximal hamstring injuries are increasingly common. While open repair with suture anchors placed in the ischial tuberosity has long been the gold standard technique for surgical management, endoscopic techniques can allow for smaller incisions, reduced wound complications, and an expedited recovery. INDICATIONS: Patients with full-thickness 3-tendon proximal hamstring tears, 2-tendon tears with retraction >2 cm, or partial tears that remain symptomatic despite conservative management are considered for surgery. High-demand patients are often treated acutely, and low-demand patien

[Indications and results of operative treatment in birth-related brachial plexus injuries].

INTRODUCTION: A review of the literature reveals that under conventional treatment alone or in combination with secondary muscle/tendon transfer about 4 to 43 % of cases show incomplete recovery with severe functional and/or aesthetic impairment. When these patients undergo early microsurgical brachial plexus revision, a regeneration without any significant functional and/or aesthetic impairment can be achieved in 80 to 90 % of the cases. Moreover, microsurgical reconstruction of the brachial plexus does increase the possibilities of secondary muscle/tendon transfers. MATERIALS AND METHODS: O

Patterns of lymph node metastases to the submuscular recess.

OBJECTIVE: To determine the incidence of lymph node metastases in the submuscular recess (level 2B) in selective neck dissections. The goal was to evaluate the oncologic need for dissection in this area owing to the increased morbidity from devascularization of the spinal accessory nerve. METHODS: A prospective analysis of 74 patients who underwent 80 neck dissections from July 2002 to March 6, 2004, was undertaken. Seventy-six of the 80 neck dissections were performed in patients with squamous cell carcinoma, whereas the remainder were done for papillary carcinoma of the thyroid. All submusc

Acute respiratory toxicity following inhalation exposure to soman in guinea pigs.

Respiratory toxicity and lung injury following inhalation exposure to chemical warfare nerve agent soman was examined in guinea pigs without therapeutics to improve survival. A microinstillation inhalation exposure technique that aerosolizes the agent in the trachea was used to administer soman to anesthetized age and weight matched male guinea pigs. Animals were exposed to 280, 561, 841, and 1121 mg/m(3) concentrations of soman for 4 min. Survival data showed that all saline controls and animals exposed to 280 and 561 mg/m(3) soman survived, while animals exposed to 841, and 1121 mg/m(3) resu

Anatomy, Imaging, and Clinical Significance of the Cervicothoracic (Stellate) Ganglion.

Background/Objectives: The stellate ganglion (SG), formed by the fusion of the inferior cervical and first thoracic sympathetic ganglia in approximately 80% of individuals, plays crucial roles in cardiac innervation, pain management, and autonomic regulation. This review examines the anatomical variations, histological structure, clinical applications, and therapeutic implications of the SG and stellate ganglion block (SGB), presenting original high-resolution magnetic resonance imaging (MRI) evidence of SG visualization, an underutilized approach in autonomic nervous system research. Methods:

Distal course and variability of the spinal accessory nerve: a cadaveric study with surgical implications.

PURPOSE: The spinal accessory nerve (SAN) is most vulnerable to injury along its distal extracranial course in the posterior cervical triangle. While proximal anatomy is well described, distal branching patterns and cervical plexus contributions to trapezius innervation remain poorly defined. This study aimed to characterise distal extracranial SAN branching and cervical plexus contributions and to assess whether proximal configurations predict distal trapezial innervation. METHODS: Ten sides from five adult cadavers were dissected from the emergence of the SAN at the posterior border of the

Embolization of bilateral cavernous sinus dural arteriovenous fistulas via the ascending pharyngeal artery using the "pressure cooker" technique: A case report.

INTRODUCTION AND IMPORTANCE: Bilateral cavernous sinus (CS) dural arteriovenous fistulas (DAVFs) are uncommon, and transarterial embolization (TAE) via the pharyngeal trunk of the ascending pharyngeal artery (AphA) has rarely been reported. PATIENT PRESENTATION: A 59-year-old female presented with a 1-month history of mild headache. On physical examination, no positive signs were found. Digital subtraction angiography revealed bilateral CS DAVFs. TAE via the pharyngeal trunk of the AphA using the "pressure cooker" technique was performed. After the distal pharyngeal trunk was catheterized wit

What's new in local anaesthesia?

In this paper I have explored four areas of current interest to pain control in dentistry. Articaine HCl, the most recent addition to the dental LA armamentarium, has become a favoured drug in many, if not most, countries in which it is available. Rapid onset and improved hard- and soft-tissue penetration enable articaine HCl to be administered with great success as a mandibular infiltration, precluding the need, in most situations, to employ it by inferior alveolar nerve block. The 'question' about an increased risk of paresthesia following articaine administration via IANB has been answered

Health-related quality of life in patients with T1N0 oral squamous cell carcinoma: selective neck dissection compared with wait and watch surveillance.

Management of the neck in patients with clinical T1N0 oral squamous cell carcinoma (SCC) is controversial. The aim of this study was to report the health-related quality of life (HRQoL) in a consecutive group of patients with stage 1 disease at a time closest to two years after primary surgery. Of 216 patients treated between 2007 and 2012 (after excluding early death and regional recurrence), 195 were eligible. HRQoL was measured using the University of Washington quality of life questionnaire version 4. The overall response rate was 65% (126/195). HRQoL outcomes were good, but compared with

Degenerative phenomena and reactive modifications of the adult rat inferior olivary neurons following axotomy and disconnection from their targets.

Adult olivocerebellar axons are capable of vigorous regeneration when provided with growth-permissive environmental conditions. To elucidate the contribution of intrinsic properties to the regenerative capabilities of inferior olivary neurons, we have examined the cellular modifications occurring in these neurons following axotomy and target deprivation in the absence of exogenous growth-promoting influences. Axotomized inferior olivary neurons undergo perikaryal shrinkage, dendritic atrophy and a loss of anti-calbindin immunoreactivity. A conspicuous cell death occurs during the first few wee

Morphological Study of the Foramen Transversarium of the Typical Cervical Vertebrae with Clinical Correlation.

BACKGROUND: The foramen transversarium (FT) is a distinctive anatomical structure in the cervical vertebrae for the passage of the vertebral artery, vein, and sympathetic plexus. The FT is a feature in all cervical vertebrae and is important for normal vertebral physiology. In this article, the anatomy of the FT is presented and discussed with respect to the variability of this foramen, its structural dimensions, and its clinical applications. The relevance for the contents of the FT are important because they contain nerves and blood vessels, and given its intimate relation with the neurovasc
