Treatment of multidrug resistant (MDR) Acinetobacter baumannii infections causes some problems as a result of possessing various antibacterial resistance mechanisms against available antibiotics. Combination of antibiotics, acting by different mechanisms, is used for the treatment of MDR bacterial infections. It is an important factor to determine synergy or antagonism between agents in the combination for the constitution of effective therapy. The study aimed to determine In vitro interactions interpreted according to calculated fractional inhibitory concentration (FIC) index between sulbacta…

The aim of this study was to unravel, by focusing on cell surface properties, the underlying virulence factors contributing to the difference in the pathogenicity observed in two Acinetobacter baumannii strains isolated from the same patient. The two strains were phenotypically different: (i) a mucoid strain (AB-M), highly virulent in a mouse model of pneumonia, and (ii) a nonmucoid strain (AB-NM), moderately virulent in the same model. The study of the cell surface properties included the microbial adhesion to solvents method, the measurement of the electrophoretic mobility of bacteria, the a…

Multidrug-resistant pathogens are becoming more difficult to treat with significantly increasing infection rates. The lack of new antibiotics to combat these strains has led to the resurgence of older antibiotics. This case highlights the first reported use of colistimethate sodium treatment in a 23-week gestational-age neonate with multidrug-resistant Acinetobacter baumannii pneumonia who developed acute renal failure and seizures shortly after initiation of treatment.

There are limited antimicrobial options for the treatment of carbapenem-resistant Acinetobacter baumannii (CRAB). Sulbactam-durlobactam was recently approved for the treatment of CRAB in patients 18 years and older. However, CRAB infections can have mortality rates up to 80% in the intensive care unit. We describe the successful novel use of sulbactam-durlobactam in a pediatric patient using available adult pharmacokinetic data and preliminary pediatric dosing suggestions from a phase 1b trial.

The global rise of colistin-resistant Acinetobacter baumannii (CRAB) is a critical health threat, particularly in the Middle East. The pmrB c.235T > A (p.Leu79Ile) mutation is frequently observed in clinical isolates, but its comprehensive characterization is lacking. In this multicenter study, 465 clinical A. baumannii isolates were collected from Iran (n = 378) and Iraq (n = 87) between January 2023 and June 2024. Colistin susceptibility was determined by broth microdilution, and the pmrCAB operon was sequenced. The structural impact of p.Leu79Ile was assessed via 200-ns molecular dynamics s…

The multidrug-resistant sepsis pathogen Acinetobacter baumannii has diverged from model γ-proteobacteria in fundamental ways, hindering efforts to develop new lines of attack against the microbe. A major area of divergence is cell division. The pathogen lacks several widely conserved division enzymes, such as FtsEX, and instead possesses a suite of atypical gene products showing no sequence similarity to any well-characterized proteins. Key among these unusual proteins is AdvA. In previous Tn-seq studies, we identified AdvA as essential for A. baumannii division and fluoroquinolone resistance.…

BACKGROUND: Carbapenem-resistant Acinetobacter baumannii (CRAB) is a major cause of healthcare-associated infections, especially in intensive care units. Its rapid acquisition of antimicrobial resistance necessitates molecular epidemiology to inform infection control measures and treatment strategies. This study evaluated antimicrobial resistance phenotypes and the prevalence of resistance genes among clinical CRAB isolates in Thailand, complemented by MLST-based population structure analysis incorporating a temporal dimension of clonal distribution compared with previous reports.
METHODS: Th…

Acinetobacter baumannii is a critical nosocomial pathogen that causes infections in diverse host niches, including the respiratory tract, bloodstream, and urinary tract. Considering the contemporary landscape of antibiotic resistance, alongside targeting resistance determinants, there is a need to identify novel and alternative targets for therapeutic intervention. Central metabolism plays a major role in the success of a pathogen and represents a pool of plausible drug targets. However, metabolic networks are inherently complex and often employ redundant pathways to ensure the supply of essen…

Acinetobacter baumannii is an opportunistic pathogen causing several infections that are increasingly difficult to treat due to its ability to rapidly gain antibiotic resistances. These resistances can arise due to mutations through the activity of error-prone DNA polymerases, such as DNA polymerase V (DNA Pol V) in response to DNA damage. The regulation of the DNA damage response (DDR) in A. baumannii is not completely understood; the regulation of genes encoding multiple copies of DNA Pol V is not fully characterized. Through genome-wide mutagenesis, we have identified a novel TetR-like fami…
