Bilateral vestibular schwannomas (VSs) are the hallmark of neurofibromatosis type 2 and the crucial criteria for the diagnosis of this autosomal dominant disorder according to the criteria of the National Institutes of Health Consensus Statement. We describe three patients without VSs aged 47, 52 and 69 years, in whom a NF2 gene carrier status was diagnosed by spinal tumors, a cataract and schwannoma of cranial and peripheral nerves. In one case the diagnosis was ascertained by mutation analysis, which revealed a 163bp deletion in the NF2 cDNA, and by the fact that two daughters had the same d

The purpose of this investigation is to review the outcome of 83 consecutive acoustic neuroma patients, managed solely or in part by the Department of Otolaryngology at University Hospital, London, Ontario, over a 6-year period (1987 to 1993). The majority of patients underwent operative intervention, utilizing the middle cranial fossa, suboccipital, or most commonly, the translabyrinthine approach depending on the tumour size and level of residual hearing. Demographic, preoperative, intraoperative, and postoperative data are analyzed including complications of surgery. As well, we report on o

Of 432 patients referred for treatment of their cerebellopontine angle tumors, 53 with acoustic neuromas were managed initially without intervention but with adequate follow-up. Mean presenting tumor size in this subgroup of patients was 0.98 cm (range, 0.2 to 3.0 cm), and average growth rate was 0.16 cm per year. Twenty-one patients demonstrated tumor growth with a mean follow-up interval of 1.9 years. Of these 21 patients, 14 underwent microsurgical excision, 4 received radiation, 2 continued to be observed and 1 was lost to follow-up. The remaining 32 (60%) had no demonstrable growth with a

Existing mechanical and acoustic models of the human middle ear are used to investigate the expected performance of various middle ear reconstructive procedures. Although the results of these analyses generally are consistent with observed surgical results, such an analytic approach enables the formation of some simple rules for maximizing middle ear function after stapedectomy and tympanomastoid surgery. Model analyses predict the following: (1) stapes prostheses of 0.6 to 0.8 mm in diameter produce air-bone gaps of less than 12 dB; smaller diameter prostheses produce larger air-bone gaps; (2

We report a case a twenty-year-old male with purpura of his limbs, and acute, diffused inflammation of the skin of the external acoustic duct. Complaints occur in the course of upper respiratory tract infection treated with antibiotic. Administered treatment resulted in quick subsidence of general symptoms with prolonged healing of external acoustic duct requiring surgical debridement (incision, and evaluation of bloody contents).

We hypothesized that acoustic measures would predict dysphonic severity with differential results for pathological voice types. An instructional program based upon synthesized voice signals was developed to facilitate an awareness of prototypical voice types. Eighty phonatory samples representing normal subjects as well as patients with unilateral vocal fold paralysis, vocal nodules, and functional dysphonia were analyzed acoustically on the basis of four measures: average fundamental frequency (F0), jitter, shimmer, and harmonic/noise ratio (H/N ratio). Following training, 29 listeners classi

The human vestibular sensory epithelia of the macula utriculi were examined ultrastructurally in 13 cases with acoustic neurinoma. The sensory epithelia were fairly well preserved, with several morphological changes such as lipofuscin granules, intraepithelial cysts, pyknosis, vacuoles, laminated structures, banded structures and degeneration of subepithelial myelinated nerves evident. The pathological significance of these findings was discussed. There were thickened irregular membrane formations in spaces between the nerve calyces and type I hair cells which might be related to acoustic neur

Up to 15% of patients with acoustic neuroma may have a history of sudden hearing loss. Recovery from sudden hearing loss with acoustic neuroma is rare. In this paper, a case of bilateral acoustic neuromas presented as repeated sudden hearing loss with recovery on the left side after removal of right-sided tumor were reported. She experienced three episodes of sudden hearing loss, showing various types of audiograms and ABR on the left side. There was no relationship between the type of audiograms and ABR findings. Factor influencing ABR might be different from the cause of hearing impairment.

Demonstrating improved outcomes and cost containment are current health care imperatives. For many diseases, competing diagnostic tests are available. Each has a direct economic cost and indirect costs based on missed disease. It is often difficult to determine the most efficient testing strategy for a given patient. Clinical Decision Analysis can optimise diagnostic strategies to maximise cost-effectiveness. Analytic techniques are developed to allow analysis of diagnostic strategies that include multi-test protocols. The test parameters required for modelling these strategies are derived fro

Facial nerve tumors can present as masses in the internal auditory canal or cerebellopontine angle and may mimic an acoustic neuroma. These tumors can occur in any segment of the nerve from the brain stem to the neuromuscular junction. Prior to the advent of computed tomography and magnetic resonance imaging with gadolinium, facial nerve tumors were often difficult to diagnose. Even with these modalities it may be difficult to distinguish preoperatively between an acoustic neuroma and a facial schwannoma. Particular signs and symptoms associated with facial nerve tumors (in the spasms, and a f

With the invention of magnetic resonance imaging (MRI) and the auditory evoked brain stem responses (AEBR), it has become possible to diagnose acoustic tumours while they are still small. As a result, it has become obvious that the clinical presentation of smaller lesions can be some-what different from what is considered typical of an acoustic neuroma. Likewise, whereas the sensitivity of auditory brain stem responses for larger tumours is good, the sensitivity for smaller tumours has recently been in doubt, particularly if the patient presents early in the course of the disease with only mil

Seventy-three procedures involving 70 acoustic tumors were carried out over 3 years. The majority of tumors (65) were removed by the translabyrinthine approach. Tumor size ranged from 0.5 through 5.0 cm. All patients underwent intraoperative facial nerve monitoring: 22 patients developed immediate facial palsy (House-Brackmann grades II-VI); 11 patients recovered completely; 11 patients (15.7%) had persistent paralysis. The incidence of long-term, poor outcome (H-B grade IV-VI) was 5.7 percent. The overall incidence of facial palsy was not found to be related to tumor size; however, facial par

We present a very unusual case of an acoustic neuroma involving the left cochlea and internal auditory canal of a 24-year-old man. Clinical suspicion was aroused when the patient presented with a left total sensorineural hearing loss and continuing vertigo. The diagnosis was made pre-operatively with MRI after initial CT scanning was normal. The tumour was removed via a transotic approach. This case report demonstrates the MRI features of an intracochlear schwannoma and emphasizes the importance of MRI in patients with significant auditory and clinical abnormalities with normal CT scans of the

A two-part study was undertaken to investigate warning signal detection by motorcyclists. First the acoustic environment was established for both the helmeted and bareheaded rider between speeds of 0 and 100 mph. For the helmeted rider it was found that below 20 mph vehicle noise was dominant and between 20 and 40 mph there was a variable contribution from both vehicle and wind noise. At 40 mph wind noise became the dominant sound source and increased linearly with the log10 of speed from 90 dB(A) to reach 112 dB(A) at 100 mph. Sound levels were consistently 18 dB(A) greater for the bareheaded

The acoustic stapedius reflex (ASR) is the sound-evoked contraction of the stapedius muscle. The ASR is mediated by a neural network, receiving its afferent input from the VIIIth cranial (auditory) nerve and sending its efferent output to the VIIth cranial (facial) nerve. Several centers in the brain stem comprise the central portion of the reflex pathway. The stapedius muscle in each ear responds to ipsilateral, contralateral, and binaural stimulation. The normal ASR threshold is 90 to 95 dB SPL for tones and 70 to 75 dB SPL for wideband noise. In ears with sensorineural hearing loss, ASR thr

An unusual case of cholelithiasis in an 18-month-old boy with cervico-oculo-acoustic (Wildervanck's) syndrome is presented. Our patient had Duane's retraction syndrome, Klippel-Feil anomaly and congenital deafness. To our knowledge this is the first case in which a probable association between cholelithiasis and Wildervanck's syndrome has been recorded. On the other hand, the presence of mutual malformations and anomalies such as scoliosis, ventricular septal defect, ectopic kidney, hydrocephalus, hypoplastic thumb and growth retardation seems to suggest that Wildervanck's syndrome is a clinic
