Dramatic oral findings belonging to a pica patient: a case report.

Pica is a compulsive eating of non-nutritive substances and can have serious medical implications. The causes are not known, but the symptom is often associated iron-deficiency. Pica is an under-diagnosed problem. Although few dental problems associated with pica have been reported, because of its potential impact on blood measures, dental professionals must be aware of this common, but commonly missed, condition. This is an unusual case of a patient diagnosed with aggressive periodontitis (AgP) who had a dirt eating habit (geophagia) and was diagnosed as having pica.

Juvenile periodontitis: microbiology and the therapy approach.

Juvenile periodontitis is an inflammatory disease of the periodontium, characterized by rapid destruction of the periodontal tissues around more than one tooth in the permanent dentition. These lesions progress rapidly after onset, but tend to slow with time. The age of onset is circumpubertal, usually manifesting in a localized form around the incisors and first molars, although some generalized cases have been reported. Clinically, there is often a small amount of supragingival plaque and calculus that is not commensurate with the amount of destruction present. In addition to a unique bacter

Occurrence and appearance of cementum hypoplasias in localized and generalized juvenile periodontitis.

The occurrence and appearance of cementum hypoplasias have been studied on teeth affected by juvenile periodontitis. Scanning electron microscopy showed the presence of cementum hypoplasias on the root surface of all extracted first molars and one incisor from nine patients with localized juvenile periodontitis and on most extracted teeth from two patients with generalized juvenile periodontitis. No hypoplasias were found on the extracted third molars. The alveolar bone loss in these patients seemed to be correlated to the frequency and extension of the hypoplastic areas in the associated teet

Anti-cardiolipin from periodontitis patients induces MCP-1 production by human umbilical vein endothelial cells.

AIM: Periodontal diseases are associated with a variety of systemic diseases, including cardiovascular disease and stroke, and patients with periodontitis demonstrate elevated levels of anti-cardiolipin antibodies. We sought to determine if anti-cardiolipin antibodies from periodontitis patients induced monocyte chemotactic protein-1 production by human vascular endothelial cells. MATERIALS AND METHODS: IgG was purified from sera from 53 subjects, including chronic and aggressive periodontitis patients and periodontally healthy controls, with elevated or normal IgG anti-cardiolipin levels. In…

Interleukin-4 (C-590T) and interferon-gamma (G5644A) gene polymorphisms in patients with periodontitis.

BACKGROUND AND OBJECTIVE: Periodontitis is a multifactorial disease and immunologic and genetic factors have an important role in its pathogenesis. Mutation in the promoter regions of the interleukin-4 and interferon-gamma genes has been reported to modify the protein expression. The objective of this study was to evaluate the possible role of interleukin-4 (C-590T) and interferon-gamma (G5644A) polymorphisms in the susceptibility to periodontitis. MATERIAL AND METHODS: In this case-control study, 53 patients (36 women and 17 men), comprising 27 patients with aggressive periodontitis and 26 p…

Haim-Munk syndrome.

Haim-Munk syndrome is an extremely rare autosomal recessive disorder of keratinization characterized clinically by palmoplantar hyperkeratosis, severe early onset periodontitis, onychogryphosis, pes planus, arachnodactyly, and acro-osteolysis. Recently, germline mutations in the lysosomal protease cathepsin C gene have been identified as the underlying genetic defect in Haim-Munk syndrome and in the clinically related disorders, such as Papillon-Lefèvre syndrome and prepubertal periodontitis. The periodontal disease associated with these syndromes is particularly aggressive and unresponsive to

Treatment of a Hopeless Tooth with Combined Endodontic-periodontal Lesion Using Guided Tissue Regeneration: A Case Report with One Year Follow-up.

Endodontic-periodontal lesions have always been a challenge for treatment due to the reduced success rate in comparison to endodontic or periodontal lesions alone. This case report describes surgical/endodontic management of supra-erupted non-vital maxillary incisor with primary periodontal and secondary endodontic lesions with mobility grade III and severe horizontal and vertical loss of attachment apparatus in a 55 years old woman with aggressive periodontitis. The successful results at one-year follow-up revealed that with an appropriate case selection strategy, proper regeneration method a…

Repair potential in localized juvenile periodontitis. A case in point.

An aggressive form of localized juvenile periodontitis (LJP) in a 12-year old West African female is reported. The case was treated with scaling, root planing, debridement, and tetracycline therapy, which resulted in complete resolution of the disease, including elimination of periodontal inflammation, regeneration of lost periodontal structures, and spontaneous repositioning of teeth that had pathologically migrated. A hopelessly involved mandibular right first molar was successfully replaced by an incompletely developed maxillary third molar tooth bud whose roots and pulp structure continued

Beyond the specific plaque hypothesis: are highly leukotoxic strains of Actinobacillus actinomycetemcomitans a paradigm for periodontal pathogenesis?

Actinobacillus actinomycetemcomitans is a facultative anaerobe implicated in a variety of periodontal diseases. Its presence is most closely associated with localized juvenile periodontitis (LIP), although the exact role of the organism in this and other periodontal diseases is not entirely clear. While A. actinomycetemcomitans produces several different putative virulence factors, the most widely studied is the leukotoxin. The leukotoxin selectively kills polymorphonuclear leukocytes and macrophages in vitro, constituting the host's first line of defense. Interestingly, even though all strain

Virulence factors of Actinobacillus actinomycetemcomitans relevant to the pathogenesis of inflammatory periodontal diseases.

There is strong evidence implicating Actinobacillus actinomycetemcomitans as the causative agent of localised juvenile periodontitis (LJP), a disease characterised by rapid destruction of the tooth-supporting tissues. This organism possesses a large number of virulence factors with a wide range of activities which enable it to colonise the oral cavity, invade periodontal tissues, evade host defences, initiate connective tissue destruction and interfere with tissue repair. Adhesion to epithelial and tooth surfaces is dependent on the presence of surface proteins and structures such as microvesi

Synergistetes cluster A in saliva is associated with periodontitis.

BACKGROUND AND OBJECTIVES: Synergistetes is a novel bacterial phylum consisting of gram-negative anaerobes. Increasing lines of evidence demonstrate that this phylum is associated with periodontal diseases. This study aimed to compare the presence and levels of Synergistetes clusters A and B, in saliva of patients with chronic periodontitis (CP), generalized aggressive periodontitis (G-AgP) and non-periodontitis subjects, and investigate their correlation with clinical parameters. MATERIAL AND METHODS: Saliva was collected from patients with CP (n = 20), G-AgP (n = 21) and non-periodontitis s…

Patterns of chemokines and chemokine receptors expression in different forms of human periodontal disease.

Current knowledge states that periodontal diseases are chronic inflammatory reactions raised in response to periodontopathogens. Many cell t … patterns of such factors' expression were found in gingival biopsies from patients presenting with aggressive periodontitis and chronic periodontitis. The expression of the chemokines macrophage inflammatory protein-1 alpha (MIP-1alpha) and interferon-gamma inducible protein 10 (IP-10) and of their respective receptors, CCR5 and CXCR3, were more prevalent and higher in a …

Unusual periodontal findings in an AIDS patient with Burkitt's lymphoma. A case report.

This report deals with a case of AIDS-related Burkitt's lymphoma in which some of the earliest findings were dental and periodontal in nature. The patient presented initially with painless and extremely loose teeth accompanied by progressive paresthesia of the lower jaw. Unique radiographic findings included extensive periodontal ligament space widening and bulbous, granuloma-like lesions about the apices of the teeth. These findings were associated with progressive tumor infiltration of the mandible and do not appear to be related to other reports of aggressive periodontitis associated with i…

Multidisciplinary management of Papillon-Lefevre syndrome as a result of consanguineous marriage.

Papillon-Lefevre syndrome (PLS) is a rare autosomal recessive syndrome, and consanguinity has been reported in 20%-40% of cases. It is characterised by palmoplantar hyperkeratosis associated with severe early-onset periodontitis and premature loss of primary and permanent teeth. This report describes a case of PLS in a female patient with consanguineously married parents. The patient reported mobile upper front teeth. Clinical examination revealed presence of marked palmoplantar hyperkeratosis.Symmetric, well-demarcated, yellowish, keratotic and confluent plaques were seen on the skin of her p
