Excitatory neurotransmitters, the most important of which is glutamate, increase the activity of signal. receiving neurons and play a major role in controlling brain function. Glutamate exerts its effects on cells in part through three types of receptors that, when activated, allow the flow of positively charged ions into the cell. Of these, the N-methyl-D-aspartate (NMDA) receptor plays a particularly important role in controlling the brain's ability to adapt to environmental and genetic influences. Even low alcohol concentrations can inhibit the excitatory activity of the NMDA receptor. This

Alcoholic ketoacidosis is an acute metabolic disorder that occurs in ethanol abusers who because of gastritis or other concurrent illness stop eating and drinking and vomit repeatedly. This causes dehydration and ketoacidosis with little or no hyperglycaemia and glucosuria. A case of severe alcoholic ketoacidosis is presented and the literature on alcoholic ketoacidosis is reviewed.

Preview Liver damage due to alcohol abuse is a severe condition often leading to cirrhosis or death. Unfortunately, the exact mechanisms involved are not well understood, and traditional treatment options have had limited success in improving morbidity and mortality rates. The authors of this article review recent studies on pathogenesis and discuss the pros and cons of current medical and surgical management.

Alcohol intake is one of the multiple risk factors for developing osteoporosis. Alcohol has direct toxic effects on osteoblasts which determines lower osteocalcin levels at an early stage, and histomorphometric changes later on. Some authors found not only diminished bone formation in alcoholics, but also increased bone resorption. The effect of alcoholism on calciotropic hormones includes fall of PTH serum levels after an acute or moderate alcohol intake, causing transient hypoparathyroidism. In chronic alcoholism, serum levels of vitamin D and its metabolites are decreased independently of a…

Clinical observations over the past two decades have pointed to the relationship between heart disease and alcohol abuse, usually without evident malnutrition or cirrhosis. While the prevalence of heart failure in the alcoholic population is now known, subclinical abnormalities of left ventricular function in noncardiac alcoholics who were normotensive have a high prevalence with or without some degree of ventricular hypertrophy by echocardiogram. This is frequently a diastolic rather than systolic abnormality. Congestive cardiomyopathy is not infrequently associated with high diastolic arteri…

Chronic alcoholism is one of the most important causes of dilated cardiomyopathy, and a large proportion of chronic alcoholics demonstrate impairment of cardiac function. The development of cardiac dysfunction is apparently related to the total lifetime dose of ethanol. Studies in experimental animals have demonstrated that both acute and chronic ethanol administration impair cardiac contractility. However, the relationship, if any, between the acute effects of alcohol and the development of irreversible cardiomyopathy remain to be elucidated.

We have reviewed 156 papers which provided sufficient information to relate individual alcohol consumption to risk for a variety of physical damage. Overall, there was evidence for a dose-response relationship between level of alcohol consumption and risk of harm for liver cirrhosis, cancers of the oropharynx, larynx, oesophagus, rectum (beer only), liver and breast, and blood pressure and stroke. An increased risk of cardiac arrhythmias, cardiomyopathy and sudden coronary death was associated with heavy drinking. There was evidence for a protective effect of alcohol consumption against risk o

The character is presented of the changes in the pathomorphological picture of alcoholic hepatocellular damage. The most frequently observed forms of clinical picture of alcoholic hepatitis are discussed. In the diagnosis of this nosological entity the role is stressed of liver biopsy as the method making possible obtaining of the most reliable diagnostic-prognostic information. In comparison with pathological examination of liver biopsy specimen, biochemical investigations are less useful in view of their low specificity.

Gastric lavage, intestinal charcoal decontamination and specific drug antagonism, therapeutic measures widely applied in other intoxications, are of no clear benefit or unavailable in patients with alcohol intoxication. The gastric first-pass-effect is important in modifying the kinetics of alcohol, and clinically relevant drug-alcohol interactions occur in situations of both acute and chronic alcohol abuse. Alcohol-induced hypoglycemia and ketoacidosis should be considered in every severely ill alcoholic patient. The recognition of alcoholism is important and rewarding, since therapeutic acti

Anorexia, weight loss, fatigue, symptoms of alcohol withdrawal and hepatomegaly are common early presenting signs and symptoms of alcohol abuse. The clinical diagnosis of alcoholic hepatitis can be made in alcoholics with associated fever, leukocytosis, jaundice and tender hepatomegaly. Associated laboratory abnormalities may include leukocytosis or leukopenia, anemia, a prolonged prothrombin time and elevated liver enzymes, including aspartate amino-transferase (AST), alanine aminotransferase (ALT), alkaline phosphatase and bilirubin. An AST-to-ALT ratio greater than 2 is common in patients w…

Many studies describing the teratogenic effects of alcohol have been published since Fetal Alcohol Syndrome (FAS) was first identified in 1973. Specifically, it has been a widely documented that alcohol is a teratogen that causes brain, craniofacial, and limb abnormalities in children suffering from FAS. These children have also been shown to be at high risk for mental deficiencies. Teratogenicity has only been observed in offspring of mothers who consume large quantities of alcohol during pregnancy, while the effects of moderate drinking, though linked with adverse fetal alcohol effects in ma

Alcohol is a dominant cause of death in urban hypothermia. Drinking alcohol gives a pleasant feeling of warmth. However, experimental studies on humans during relatively short exposure to moderate cold have given inconsistent results concerning heat balance. Longer exposure to colder environmental situations has, particularly with addition of strenous exercise, revealed enhanced heat loss. A warning must be given to drink alcohol beverages in connection with outdoor activities in a cold environment. It gives a feeling of bravery and influences judgement leading to ignoration of prophylactic me

There are wide ranging effects of alcohol on the nervous system. Some interfere with physiological and neurochemical functions but ultimately structural damage occurs. During life one of the most impressive changes is brain shrinkage which can be visualized using neuroradiological imaging techniques. This article reviews the pathological explanations for brain shrinkage and addresses the question of the pathogenesis of the reversible component of this damage in relation to prolonged abstinence from alcohol. This shrinkage seems to relate to a loss of white matter. However, the cortex is also a

Some of the major effects of alcohol, and alcoholism, on the brain are reviewed, with reappraisal of evidence drawn from brain imaging, neuropathology, clinical psychology, and laboratory experimental work. A hypothesis is developed which may help to account for the wide variability encountered in individual susceptibility to alcoholic brain damage and its varied manifestations. Therapeutic implications are considered.

A case-control study compared the natural history of cigarette smoking in alcoholic and nonalcoholic populations to determine why alcoholism and smoking are closely associated. Eighty-three percent of alcoholics were smokers compared to 34% of the nonalcoholic subjects. Compared to other children, children who later became alcoholic were more likely to become regular users of tobacco once they had tried it. Only 7% of alcoholic smokers were successful in their attempts to quit smoking compared to 49% of the nonalcoholic smokers. More research comparing the smoking cessation efforts of alcoholi…

Multiple drug use occurs in alcoholics in an age-dependent relationship. The younger the onset of alcoholism, the more likely is additional drug use. Drugs used by alcoholics, in decreasing order of frequency, are marijuana, cocaine, and other stimulants, phencyclidine, benzodiazepines, barbiturates, and hallucinogens. Evaluation of intoxication and treatment or detoxification in alcoholics should always include consideration of other drugs. Multiple drug use in alcoholics does not preclude the short- or long-term treatment that traditionally is available for alcoholics, such as programmatic t…
