Centrally mediated hemodynamic effects of cocaine in rabbits: the role of local anesthetic actions and biogenic amine re-uptake blockage.

This study sought to identify the central nervous system-mediated cardiovascular effects of cocaine and to determine if these effects result from local anesthetic action or biogenic amine re-uptake blockade. New Zealand rabbits received multiple i.c.v. doses of cocaine, pseudococaine, lidocaine or desipramine while recording blood pressure and heart rate. Moderate dose cocaine produced a decrease in heart rate and mean arterial pressure while the highest dose produced increases in heart rate and blood pressure. Lidocaine and pseudococaine produced dose-dependent increases in blood pressure whi

Sudden neonatal death following circumcision associated with bupivacaine-induced Local Anesthetic Systemic Toxicity (LAST): a forensic case report.

This article presents a fatal case of local anesthetic systemic toxicity (LAST) in a healthy 26-day-old male following a ritual circumcision. Bupivacaine, a local anesthetic, was used to induce a dorsal penile nerve block and a ring block. No other medications were administered. Postoperatively, the mother noticed the neonate had become sleepy. She allowed him to rest, assuming he was asleep. Two hours later, she attempted to awaken him but found him dead. Postmortem examination revealed a circumcised penis with a localized hematoma, suggesting inadvertent vascular injection of bupivacaine. Di

The local anesthetic, lidocaine, suppresses the growth of colon cancer SW480 cells by decreasing the voltage-gated sodium channel subunit Nav1.5.

Lidocaine is an inhibitor of voltage-gated sodium channels (VGSCs) expressed in excitable cells, such as myocardial cells and sensory neurons, and is used in many clinical situations, including as an anti-arrhythmic and analgesic agent. Clinical studies have suggested that its perioperative application in cancer patients improves their prognosis. Nav1.5 encoded by the SCN5A gene, which forms the α subunit of VGSC and is a pharmacological target of lidocaine, is expressed in various types of cancer cells. The SCN5A gene is also identified as an upstream regulator of the invasive activity of col

Clinical Evaluation of Rectus Sheath Block Versus Local Anesthetic Infiltration in Laparoscopic Inguinal Hernia Repair: A Retrospective Comparative Study.

Background Laparoscopic inguinal hernia repair is frequently associated with early postoperative pain due to somatic nociceptive input from the anterior abdominal wall. Ultrasound-guided rectus sheath block (RSB) provides targeted analgesia by blocking terminal branches of the thoracoabdominal nerves within the rectus sheath. Although RSB is increasingly used in minimally invasive surgery, its comparative effectiveness against conventional local anesthetic (LA) wound infiltration in adult hernia repair remains inadequately characterized. Objectives The objective of this study is to evaluate th

Does Addition of a Longer Acting Local Anesthetic Improve Postoperative Pain After Carpal Tunnel Release? A Randomized Controlled Trial.

PURPOSE: Carpal tunnel release (CTR) is a simple and effective treatment for carpal tunnel syndrome in patients who have failed nonsurgical management. This surgery is often performed in the ambulatory clinic under local anesthesia, with lidocaine, a short-acting agent. Few studies have investigated the use of longer acting agents, such as bupivacaine, for outpatient CTR. Therefore, the aim of our study was to compare the postoperative pain experience after CTR with the use of either our standard lidocaine solution (control) or a mixture consisting of lidocaine and bupivacaine in equal amounts

Effectiveness of Hydro-Dissection of the Piriformis Muscle Plus Low-Dose Local Anesthetic Injection for Piriformis Syndrome: A Report of 2 Cases.

BACKGROUND Piriformis syndrome causes severe buttock and low back pain and numbness of the lower limbs, leading to difficulty in walking. The lack of unified diagnostic criteria for piriformis syndrome until 2018 made it difficult to diagnose; therefore, it is often underestimated, and some patients do not receive appropriate treatment. Treatment methods include local anesthetic injection, steroid injection, and local botulinum toxin injection. However, the most effective drug for the treatment of severe piriformis syndrome has not been established. This report aimed to propose a new and more

Local Anesthetic Injection Before Incision Decreases General Anesthesia Requirements in Pediatric Trigger Thumb Release: A Randomized Controlled Trial.

BACKGROUND: Animal studies have shown evidence of neurotoxicity from inhalational anesthesia, yet clinical studies have been less conclusive. While ongoing studies investigate the clinical significance of anesthesia-associated neurodevelopmental changes in young children, reducing anesthetic exposure in pediatric orthopaedic surgery is prudent. The primary objective of this study is to determine if local anesthetic injection before surgical incision versus after surgical release decreased inhalational anesthetic exposure in children undergoing unilateral trigger thumb release. The secondary ob

Local anesthetic volume in ultrasound-guided interscalene block and opioid consumption during shoulder arthroscopic surgery: A retrospective comparative study.

Interscalene block (ISB) is commonly performed for regional anesthesia in shoulder surgery. Ultrasound-guided ISB enables visualization of the local anesthetic spread and a reduction in local anesthetic volume. However, little is known about the appropriate local anesthetic dose for surgical anesthesia without sedation or general anesthesia. The purpose of our study was to evaluate the appropriate local anesthetic volume by comparing intraoperative analgesics and hemodynamic changes in ISB in arthroscopic shoulder surgery.Overall, 1007 patients were divided into groups 1, 2, and 3 according to

Ashi Acupuncture Versus Local Anesthetic Trigger Point Injections in the Treatment of Abdominal Myofascial Pain Syndrome: A Randomized Clinical Trial.

BACKGROUND: Chronic pelvic pain (CPP) is defined as recurrent or continuous pain in the lower abdomen or pelvis, non-menstrual or non-cyclic, lasting at least 6 months. There is strong evidence that up to 85% of patients with CPP have serious dysfunction of the musculoskeletal system, including abdominal myofascial syndrome (AMPS). AMPS is characterized as deep abdominal pain, originating from hyperirritable trigger points, usually located within a musculoskeletal range or its fascia of coating. In the literature, there are few studies that address AMPS. OBJECTIVE: This study aimed to compare

Creating an Outpatient, Local Anesthetic Hand Operating Room in a Resource-Constrained Ghanaian Hospital Builds Surgical Capacity and Financial Stability.

INTRODUCTION: Expanding surgical capacity is very difficult in resource-constrained countries. Financial input and additional physical operating room space are needed. The surgical patient volume exists, but the lack of operating room time causes postponement of cases. Hand surgery is particularly important as it improves patient function and allows for a timely return to the workforce after injury. Some hand surgery cases may be performed under local anesthesia with a very basic instrument set in a procedure room. This arrangement eliminates the need and financial burden of an equipped operat

Prospective Randomized Trial Comparing Femoral Nerve Block With Intraoperative Local Anesthetic Injection of Liposomal Bupivacaine in Total Knee Arthroplasty.

BACKGROUND: Postoperative pain after total knee arthroplasty (TKA) may impact long-term results and incidence of complications. Femoral nerve block (FNB) provides excellent pain relief after TKA, although associated risks include weakness, delayed participation in therapy, and nerve injury. Liposomal bupivacaine (LB) is a potentially longer acting local anesthetic that may reduce postoperative pain. METHODS: We performed a prospective, randomized, double-blind study of 373 TKA patients randomized to receive either an FNB (control group), or an intraoperative periarticular injection (PAI) with

Continuous intra-articular local anesthetic drug instillation versus discontinuous sciatic nerve block after total knee arthroplasty.

STUDY OBJECTIVE: Sciatic nerve block (SNB) is commonly used as adjunct to femoralis nerve block (FNB) to achieve high-quality pain relief after total knee arthroplasty (TKA). However, this combination is associated with considerable muscle weakness, foot drop and surgically related nerve injuries may be masked. The purpose of this study was to assess whether low risk continuous intra-articular anesthetic drug instillation is an adequate alternative to SNB when adding to FNB after TKA. DESIGN: Retrospective investigational follow-up study. SETTING: University teaching hospital. Interdisciplin

Severe post-burn neck contracture release and skin graft harvest using tumescent local anaesthesia as the sole anesthetic technique.

Severe post-burn contractures in the neck often cause anatomical distortion and restriction of neck movements, resulting in varying degrees of difficulty in airway management. Any mode of anesthesia that may obviate the need for imperative airway control may be desirable in such situations in which a difficult airway may be anticipated. Here we present one such situation where tumescent local anesthesia was employed to manage a case of severe post-burn neck contractures posted for contracture release and split-skin grafting. The other benefits of this method were minimal blood loss and excelle
