Bioavailability of iron and cyanide from oral potassium ferric hexacyanoferrate(II) in humans.

After oral administration of 500 mg KFe[Fe(CN)6] labelled with 59Fe either in the ferric or ferrous position and with 14C in the cyanide group only 0.22% of the FeII and less than 0.04% of the FeIII were absorbed in three male volunteers. Only 2 mg non-complex bound 14C-labelled cyanide (0.03 mg CN-/kg body wt) were absorbed from 500 mg [14C]KFeHCF, which is about a factor of 20-100 below the lethal dose in humans (0.5-3.5 mg CN-/kg body wt). Therefore, iron(III) hexacyanoferrates(II) can be considered as safe antidotes, i.e. for inhibiting the intestinal absorption of radiocaesium or for acce…

Drug-induced liver injury. Part I: Classification, diagnosis and treatment.

Drug-induced liver injury (DILI) is a growing clinical problem. Antibiotics remain the most common cause of DILI in Europe. Their clinical s … n management is discontinuation of the drug suspected of causing liver damage. The list of specific antidotes that eliminate the effects of hepatotoxins is unfortunately very short. In symptomatic treatment, glucocorticosteroids and ursodeoxycholic acid have been used in selected cases. Liver transplantation is an optional treatment in patients with acute l …

[Management of emergencies in direct oral anticoagulant-treated patients: practice pearls].

Direct oral anticoagulants have become first-line agents in anticoagulation therapy in recent years; however, their reversal may be challenging in emergency situations, such as massive bleeding or urgent surgery. Conventional coagulation tests are not suitable for accurately assessing anticoagulant activity, making specific monitoring methods (e.g., anti-FXa, viscoelastic tests) essential. Among antidotes, idarucizumab is effective for rapidly reversing the effects of dabigatran, while andexanet alfa neutralizes apixaban and rivaroxaban, though its availability and thrombosis risk limit its us…

[The use of opium in Roman society and the dependence of Princeps Marcus Aurelius].

Opium was known and frequently used in Roman society. Medical practice recognized its usefulness as an analgesic, soporific, anti-tussic or anti-diarrheic agent, as well as other currently unsupported uses with quasi-magical properties. It was additionally used as an ingredient in antidotes, panaceas and poisons. The authors present a non-exhaustive compilation of opium use according to medical doctors, writers and encyclopaedists of the time. Mythological and literary representations of the opium poppy reflected its diverse roles, being associated with prosperity and fertility, sleep, death a…

Serious envenomation after a snakebite by a Western bush viper (Atheris chlorechis) in the Netherlands: a case report.

Venomous snakebites are a rarity in the Netherlands. In this report we describe the case of a 26-year-old male amateur snakekeeper who was bitten in his left index finger by a Western bush viper (Atheris chlorechis). His clinical condition deteriorated rapidly with acute renal failure and considerable blood loss due to coagulopathy. Antidote was not readily available and was finally supplied by a zoo in Antwerp, Belgium. One day after admission the blood loss diminished.

[The correction of free-radical oxidation processes in patients with chronic cholecystitis subjected to ionizing radiation exposure].

An activation was found of lipid peroxidation system as well as suppression of antioxidant defence tending to be more profound in blood erythrocytes, in 47 patients with chronic cholecystitis having been in the zone of abnormal radiation. Correction of the above alterations revealed unithiol, clinical and biochemical effects to be pronounced compare with those or tocopherol. Direct antioxidant effect or unithiol, its capability of serving as an antidote of heavy of heavy metals are believed to account for the above observation.

Clinical guidelines for acute poisonings in low- and middle-income countries.

Acute poisonings are a major public health concern in low- and middle-income countries, and require prompt, structured management. We present an updated standard operating protocol to support rapid clinical-decision-making in emergency settings. The protocol emphasises five core principles of toxicology: stabilisation, identification, decontamination, elimination, and antidote use. It covers common pesticide poisonings, including organophosphate and aluminium phosphide, corrosive ingestions, and emerging threats such as paraquat, opioids, and sedative-hypnotics.

Carbon Monoxide and Cyanide Intoxication: An Association to Remember.

Carbon monoxide and cyanide are toxins that induce cellular hypoxia; both can be produced in the context of domestic fires and may have a synergistic effect. We present the case of a man, victim of a house fire, who was initially diagnosed as having carbon monoxide poisoning, but that afterwards also presented signs and symptoms compatible with cyanide poisoning. He was successfully treated with an antidote. We want to highlight this relatively frequent association of poisonings and the need for urgent empirical treatment.

An outbreak of Cotyledon orbiculata L. poisoning in a flock of Angora goat rams.

An acute outbreak of C. orbiculata L. poisoning in a flock of 16 Angora Goat rams is described. Typical signs of acute bufadienolide cardiac glycoside toxicity were manifested and 6 animals died. In the 2 animals examined histopathologically multiple foci of myocardial degeneration and necrosis were present. Treatment consisted, inter alia, of drenching with a mixture of activated charcoal, potassium chloride and a commercial preparation, Universal Antidote (Centaur) and parenteral administration of atropine sulphate.

[The experimental treatment of cyanide poisoning with a methemoglobin solution].

The stroma-free methemoglobin solution proved to be an effective antidote against acute cyanide poisoning in experiment. The poisoning was induced by intraperitoneal administration to rats of cyanide solutions in doses of 5, 10 and 15 mg/kg. Methemoglobin solutions were injected intravenously in doses of 2 and 4 g/kg. All the rats given methemoglobin solution after the administration of cyanide survived. Spectrophotometry of rat urine demonstrated rapid excretion of methemoglobin cyanide.

Exploring research gaps and trends in the management of acute phosphide poisoning: a systematic review.

Metal phosphides are highly toxic pesticides that result in high morbidities and mortalities worldwide. This systematic review included 350 studies that fulfilled the eligibility criteria. There were significant rising trends of studies on acute aluminum phosphide (AlP) and zinc phosphide (Zn3P2) poisoning (p-values = <.001), pointing to an increased number of phosphide-intoxicated patients. Acute AlP poisoning studies represented 81%, 89.3%, and 97.7% of all descriptive, analytical, and experimental interventional studies included in this review, respectively. High AlP poisoning mortality exp

Reversal strategies in patients treated with direct oral anticoagulants.

Administration of direct oral anticoagulants (DOACs) for the treatment of venous thrombotic events (VTE) or non-valvular atrial fibrillation (AF) is now standard of care and has demonstrated clinical efficacy and safety in numerous clinical studies. Usually these substances have lower overall mortality and less risk of cerebral hemorrhage, but depending on the substance and study, they are more likely to cause gastrointestinal bleeding than vitamin K antagonists (VKA), the medication that used to be standard for VTE and AF. Since DOACs have very short plasma elimination half-lives compared to

[Methaemoglobinaemia induced by ingestion of alkyl nitrite, "poppers"].

We present a case report of an 18-year-old male who was referred to the emergency department with evidence of methaemoglobinaemia. He presented with classic symptoms with peripheral cyanosis and hypoxia. Arterial blood gas showed a methaemoglobin level of 36%. This was caused by ingestion of alkyl nitrate, a widespread party drug called "poppers". When inhaled it causes euphoria, reduced pain and relaxation of the anal sphincter, but oral use may induce life-threatening methaemoglobinaemia. The treatment of choice is the antidote methylene blue. After treatment the patient regained full recove

Hydroxocobalamin treatment of acute cyanide poisoning from apricot kernels.

Clinical experience with hydroxocobalamin in acute cyanide poisoning via ingestion remains limited. This case concerns a 35-year-old mentally ill woman who consumed more than 20 apricot kernels. Published literature suggests each kernel would have contained cyanide concentrations ranging from 0.122 to 4.09 mg/g (average 2.92 mg/g). On arrival, the woman appeared asymptomatic with a raised pulse rate and slight metabolic acidosis. Forty minutes after admission (approximately 70 min postingestion), the patient experienced headache, nausea and dyspnoea, and was hypotensive, hypoxic and tachypnoei
