We introduce a novel, simplified surgical technique for iris-claw intraocular lens implantation in aphakia cases without adequate capsular support. The Needle Assisted Iris-claw Lens (NAIL) technique utilizes a 10-0 prolene straight needle as an internal guide and pivot during enclavation, thereby enhancing centration, precision, and ease of intraocular lens implantation. There is a lot of uncertainty concerning intraocular lens (IOL) implantation for pediatric cataracts. The appropriate age which ocular abnormalities are contraindications and according to which formula IOL should be calculated are controversial. In addition to the imperative of identifying postoperative complications, such as secondary cataract formation and secondary glaucoma in a sufficiently timely manner, a modern management of aphakia with refractive compensation and occlusion is necessary. Some easy rules can help prevent pitfalls.

We present a report on a patient with acute glaucoma. Slit-lamp-adapted optical coherence tomography (SL-OCT) confirmed a pupillary block due to a claw lens. After laser iridectomy the situation stabilized. Thus, SL-OCT allows non-invasive, high-resolution imaging in acute angle-closure glaucoma and may be helpful with the differential diagnosis of pupillary block glaucoma and the documentation of the surgical result.

The investigations included 20 aphakic patients. Relations were established between visual acuity, refraction and the pattern reversal visual-evoked cortical potentials (PRVECP). Close correlations were found between the three values. The quality of binocular vision in unilateral aphakia was tested in presbyopic patients. 10 of them were corrected by a contact lens, 10 by an intraocular implant.
