Historically, the management of cloudy, decompensated corneas centered around replacement of corneal tissue with either a through-and-through or partial-thickness replacement. This article presents a technique that is a radical departure from prior methods.

BACKGROUND: Knowledge of individual corneal thickness in individual eyes is proving to be more and more crucial for correcting Goldmann applanation tonometry, as well as to assess its predictive value regarding the possible progression of glaucomatous damage. It has become an important factor in determining appropriate glaucoma therapy. Only few pachymetric data exist about eyes with congenital or secondary pediatric aphakic glaucoma.
METHODS: 12 eyes with congenital glaucoma (6 patients, mean age 3.3 years, mean 3.9 previous operations) and 7 eyes with secondary pediatric aphakic glaucoma (4

BACKGROUND: The aim of curing cataract blindness is to restore the patient's vision after surgery. The aim of the study was to determine the proportion of surgeries done for cataract blind patients and the visual outcome with +10 diopter sphere (DS) aphakic correction in two consecutive years.
METHODS: Of the cataract operations done in the years 1995 and 1996, 8-15 cases were selected from each month by proportionate simple random sampling. Thirty-two (32) were excluded for incomplete or missing records. One hundred and eighteen (118) and one hundred (100) patient records were analyzed for 1

OBJECTIVE: To compare the rate of refractive growth in pseudophakic children's eyes to that of aphakic eyes.
DESIGN: Multicenter, retrospective observational case series.
PARTICIPANTS: 83 patients with pseudophakic eyes (100 eyes) and 74 patients with aphakic eyes (106 eyes), with an age of surgery between 3 months and 10 years and a minimum follow-up time of 3 years or more, depending on the age at surgery.
METHODS: A logarithmic model was used to analyze the rate of refractive growth for each eye.
MAIN OUTCOME MEASURES: Age at surgery, intraocular lens power, intraocular lens A-constant,

We present the first reported use in the United States of a black iris-diaphragm intraocular lens (IOL) for the treatment of traumatic aniridia and aphakic bullous keratopathy. The patient presented to a university-based practice with contact-lens-intolerant aniridia and aphakia with painful bullous keratopathy from a failed corneal graft. He was treated with combined penetrating keratoplasty and transscleral fixation of an aniridia IOL. The patient's preoperative symptoms of debilitating glare and photophobia resolved substantially after surgery, despite mild postoperative inflammation that r

OBJECTIVE: To determine the effect of optical defocus (such as what develops in spontaneous myopia and subsequent to cataract extraction) on visual acuity in dogs.
ANIMALS: 3 young adult male Beagles.
PROCEDURE: The effect of optical defocus on visual acuity was determined by sweep visual evoked potential, using a within-subjects/repeated measures design in which each dog served as its own control. Dogs were positioned so that the eye being tested was 60 cm in front of the video display, and the target was centered on the area centralis. To create ametropia relative to the video screen, a se

PURPOSE: To study the role of epikeratophakia in treating pediatric unilateral aphakia after traumatic cataract extraction.
METHODS: We performed epikeratophakia in 43 children to treat aphakia after traumatic cataract extraction, using the corneal lenses made by ourselves. The rehabilitation of the the postoperative vision and the prevention of amblyopia were observed with a mean follow-up period of 20 months.
RESULTS: All the lenses remained transparent. Postoperatively, 31 cases (72.1%) achieved uncorrected visual acuities (V.A) over 0.2, 32 cases (74.4%) achieved corrected V.A over 0.4.

This article describes the clinical presentation of glaucoma in eyes operated on for congenital cataract and reviews current theories as to its pathophysiology. The literature does not provide a conclusive explanation for the seeming increase in aphakic glaucoma in children. Long-term studies comparing like groups may help resolve this question.

The authors present the results of a new method for surgical treatment of aphakic eye glaucoma with the use of low-frequency ultrasound. The suggested surgery, ultrasonic cyclodialysis, was carried out in 15 patients with glaucoma on the aphakic eyes. A follow-up of 6 months has demonstrated a stable normalization of intraocular pressure. The mechanism of the hypotensive effect of the operation is explained largely by improvement of fluid discharge and a certain inhibition of the chamber humor production. The authors emphasize the sparing nature of the operation and a serene postoperative cour

The authors compared the results of in vivo spectrophotometry of the eye in persons with a normal lens, in patients with aphakia and after an operation of intraocular lens implantation. The results received show that in the range of 400-500 microns half of the light registered by the spectrophotometer is connected with the lens in the eye. This speaks for the assumption that in vivo spectrophotometry in this range of the wave length may be one more method enabling an objective evaluation of the transparency of the lens. In the case of patients after operation of implantation of an intraocular

Implantation of intraocular lenses in children was performed in 42 cases in our department. In 26 children it concerned primary implants, in 16 secondary implants. No major postoperative complications were observed in the period between the 6th and the 24th month after surgery. The visual acuity of 0.5 or better showed 70 p.c. of children after the primary and 66 p.c. after secondary implants.

Fifty pediatric aphakic patients, ranging in age from 7 days to 11 years, were fitted with the Silsoft extended wear contact lens. Compared to other extended wear lenses, the Silsoft lens has several advantages. These include easier handling, a less traumatic fitting procedure for the child, the use of fluorescein for evaluation of the lens-cornea relationship and a low loss rate. This lens appears to be safe, with no permanent adverse effects on the cornea.

The surgical management of glaucoma in aphakia has been limited by poor success in the control of intraocular pressure and serious postoperative complications that threaten vision. A consecutive series of trabeculectomy filtering procedures in aphakic eyes with a mean preoperative intraocular pressure of 38 mmHg was followed for an average of 26 months and revealed a 62% control of intraocular pressure at 21 mmHg or less. Five additional patients (24%) had pressures lower than 21 mmHg following digital massage. The complication of decreased visual acuity following surgery was significant but n

A surgeon analyzes his perception following removal of cataracts and repair of a retinal detachment. His conclusions are: aphakic eyes perceive ultraviolet light, the optic disc is perceived as a point rather than a space the size of the disc, a certain minimal similarity of light intensity on the two retinas is necessary for fusion of images, reduction telescopes can be made by a combination of contact lenses and trifocals which balance the image size in unilateral aphakia, the visual angle of cataract glasses may be only 56 degrees but is perceived as 90 degrees with a 22 degrees blind secto
