Central apnea at complex partial seizure onset.

Sudden Unexpected Death in Epilepsy (SUDEP) is the most common cause of epilepsy related mortality in treatment resistant epilepsy. Most SUDEPs occur after one or more seizure(s) during sleep. Nocturnal seizures may go unrecognized. Respiratory depression in the peri-ictal period is one of the primary potential causes of SUDEP. Ictal and postictal apnea is often overlooked because it is not routinely assessed, but appears common and has been a recent focus of SUDEP research. We report a 37 year-old man who had central apnea as the initial manifestation of partial complex seizures associated wi…

Sleep related expiratory obstructive apnea in children.

STUDY OBJECTIVES: We describe the respiratory, cardiac, and sleep-related characteristics of two types of sleep-related respiratory pauses in children that can fulfill current criteria of pathological apnea, but often seem to be benign: prolonged expiratory apnea (PEA) and post-sigh central apnea (PSCA). METHODS: All outpatient comprehensive overnight polysomnography completed on children without significant underlying medical conditions completed during an 18-month period were retrospectively reviewed for the presence of augmented breaths followed by a respiratory pause. Events were identifi…

Submucosal lingualplasty for adult obstructive sleep apnea.

OBJECTIVE: To measure quality-of-life outcomes, polysomnographic outcomes, and adverse effects for a new technique of tongue reduction in obstructive sleep apnea. STUDY DESIGN: Case series. SETTING: Tertiary hospital. SUBJECTS AND METHODS: Consecutively treated adult patients (N = 27) with obstructive sleep apnea having submucosal lingualplasty in 2007 were studied. All had concurrent or previous uvulopalatoplasty ± palatal advancement. Full polysomnography preoperatively and 3.7 ± 0.4 months postoperatively, scored using the American Academy of Sleep Medicine 2007 criteria, was recorded. S…

Predicting sleep apnea in bariatric surgery patients.

BACKGROUND: Because of the high prevalence and potentially serious complications of obstructive sleep apnea (OSA) in obese individuals, several prediction models have been developed to detect moderate-to-severe OSA in patients undergoing bariatric surgery. Using commonly collected variables (body mass index [BMI], age, observed sleep apnea, hemoglobin A1c, fasting plasma insulin, gender, and neck circumference), Dixon et al. developed a model with a sensitivity of 89% and specificity of 81% for patients undergoing laparoscopic adjustable gastric band surgery suspected to have OSA. The present …

Obstructive sleep apnea in Treacher Collins syndrome.

The aim of the present study was to investigate the prevalence of obstructive sleep apnea syndrome (OSAS) among the Norwegian population with Treacher Collins syndrome (TCS). A secondary aim was to establish whether TCS phenotype severity is associated with OSAS severity. A prospective case study design was used. Individuals who were 5 years old and above with a known diagnosis of TCS in Norway were invited to participate in a study. The study included genetic testing, medical and dental examinations and polysomnography. All participants demonstrated disturbed respiration during sleep; 18/19 m…

Work productivity in obstructive sleep apnea patients.

OBJECTIVES: Purpose of the study was to assess work productivity in otherwise healthy obstructive sleep apnea (OSA) patients and to explore correlations between work productivity and different characteristics of OSA patients. METHODS: Work productivity was assessed by the Endicott Work Productivity Scale (EWPS) to 115 polysomnographically confirmed OSA patients of working age, without comorbidities. Daytime sleepiness was measured by the Epworth Sleepiness Scale. RESULTS: A significant correlation was revealed between EWPS and Epworth Sleepiness Scale scores (r2 = 0.127, P < 0.001). Mean EWP…

Obstructive sleep apnea/hypopnea and systemic hypertension.

Obstructive sleep apnea/hypopnea (OSAH) syndrome is a highly prevalent condition. Severe OSAH affects 2-6% of the population, although only 10% of subjects are correctly diagnosed and treated. OSAH is an important and unresolved public health care problem because of its role in the development of cardiovascular events, negative impact on quality of life, and as a cause of traffic accidents. Longitudinal and cross-sectional studies have shown a strong association between OSAH and hypertension. Moreover, a number of open-label studies, the majority of 21 controlled studies included in the presen…

[Two cases of complex sleep apnea syndrome].

Continuous positive airway pressure (CPAP) therapy of obstructive sleep apnea syndrome (OSAS) is widely accepted. Recently it is reported that central type apnea increases in some patients with OSAS with the application of CPAP, and this type of sleep-disordered breathing is called complex sleep apnea syndrome (comp. SAS). However, until now its concept, mechanism and therapy have not been fully established. We treated 2 cases of comp. SAS with CPAP therapy. When we performed a polysomnography (PSG) examination in case 1 one year later, the symptoms had diminished and the central apnea had dec…

Impaired cerebral autoregulation in obstructive sleep apnea.

Obstructive sleep apnea (OSA) increases the risk of stroke independent of known vascular and metabolic risk factors. Although patients with OSA have higher prevalence of hypertension and evidence of hypercoagulability, the mechanism of this increased risk is unknown. Obstructive apnea events are associated with surges in blood pressure, hypercapnia, and fluctuations in cerebral blood flow. These perturbations can adversely affect the cerebral circulation. We hypothesized that patients with OSA have impaired cerebral autoregulation, which may contribute to the increased risk of cerebral ischemi…

[Obstructive sleep apnea syndrome and cardiac arrhythmias].

Obstructive sleep apnea syndrome (OSAS) refers to recurring episodes of upper respiratory track obstruction and frequent decreases in arterial oxygen saturation due to repetitive occlusions of the posterior pharynx during sleep. Its prevalence in adult population is 4% in men and 2% in women. The most important causes of morbidity and mortality in affected patients are traffic accidents and cardiovascular complications including systemic arterial hypertension, coronary artery disease, congestive heart failure, and cardiac arrhythmias. The initial phases of apnea are associated with a transient…

Apnea and macrocephaly-cutis marmorata telangiectatica congenita.

The authors report a case of an infant girl with macrocephaly-cutis marmorata telangiectatica congenita (Macrocephaly-CMTC). This patient pr … s not achieved, requiring tracheostomy tube placement. To our knowledge this is the first report of apnea in a patient diagnosed with M-CMTC, likely due to cervicomedullary cord compression and perhaps exacerbated by unilateral laryngeal hypertrophy. M-CMTC is a newly-described hemi-hypertrophy syndrome affecting the neurodevelopment of affected childre …

Sleep apnea, alertness, and motor vehicle crashes.

Sleep apnea causes impairment in performance and is associated with an increased risk of motor vehicle crashes compared with the general population of drivers. Despite this increased risk, the actual number of accidents is still quite low, although the implications are significant in commercial vehicle drivers. It is difficult for physicians to assess risk and ability to drive in many patients with sleep apnea, yet physicians are often mandated to make these assessments with obvious implications for patients. Because many patients may never have a crash, it is not practical or feasible to rest…

Adult obstructive sleep apnea: pathophysiology and diagnosis.

Obstructive sleep apnea (OSA) is a highly prevalent disease characterized by recurrent episodes of upper airway obstruction that result in recurrent arousals and episodic oxyhemoglobin desaturations during sleep. Significant clinical consequences of the disorder cover a wide spectrum, including daytime hypersomnolence, neurocognitive dysfunction, cardiovascular disease, metabolic dysfunction, and cor pulmonale. The major risk factors for the disorder include obesity, male gender, and age. Current understanding of the pathophysiologic basis of the disorder suggests that a balance of anatomicall…

Cardiovascular responses to apnea during dynamic exercise.

Breath holding maneuvers induce hypoxia, hypercapnia, and various cardiovascular responses typically including increases in total peripheral … no major differences between the conditions of rebreathing and free breathing. In contrast, 20 s of apnea led to a marked increase in MAP and a HR depression at both levels of exercise intensity. Additionally, there was a delayed MAP recovery after this stimulus. The present findings show that breath holding has marked effects on MAP and HR during dynam …
