Rescuing cardiac automaticity in L-type Cav1.3 channelopathies and beyond.

Pacemaker activity of the sino-atrial node generates the heart rate. Disease of the sinus node and impairment of atrioventricular conduction induce an excessively low ventricular rate (bradycardia), which cannot meet the needs of the organism. Bradycardia accounts for about half of the total workload of clinical cardiologists. The 'sick sinus' syndrome (SSS) is characterized by sinus bradycardia and periods of intermittent atrial fibrillation. Several genetic or acquired risk factors or pathologies can lead to SSS. Implantation of an electronic pacemaker constitutes the only available therapy

Catheter ablation for atrioventricular nodal reentrant tachycardia in patients with congenital heart disease.

BACKGROUND: Variability in atrioventricular (AV) node location in congenital heart disease (CHD) can make catheter ablation for atrioventricular nodal reentrant tachycardia (AVNRT) challenging. OBJECTIVE: The purpose of this study was to describe institutional technique and outcomes for slow pathway modification in a cohort with CHD. METHODS: The study consisted of a retrospective review of CHD patients who underwent study from 2001 to 2013 with a diagnosis of AVNRT. Outcomes for slow pathway modification were recorded. In cases in which ablation was deferred, the reasons for this choice wer

Electrophysiologic and histologic correlations in chronic complete atrioventricular block.

Electrophysiologic studies using the His bundle electrogram (HBE) and histologic studies of serial sections of the conduction system were correlated in two groups of deceased patients. Group 1 consisted of five patients with chronic complete atrioventricular block (CAVB) who had narrow QRS complexes and AH block (block proximal to the His bundle deflection). Group 2 consisted of four patients with chronic CAVB who had wide QRS complexes and HV block (block distal to the His bundle deflection). In group 1, the sites of the main lesion were not located in the approaches to the atrioventricular (

Clinical pathology of the cardiac conduction system.

Abnormal architecture of the conduction system affects both the normally structured and the congenitally malformed heart. While the majority of malformed hearts have more-or-less normal arrangements of the conduction system, there are others with significant deviations. In particular, the derangements are related to malalignment of the ventricular septum in the case of the atrioventricular conduction tissues, and to atrial arrangement with regard to the sinus node. Congenital heart block can occur in isolation or in association with congenital heart defects. The anatomical substrate in heart b

High prevalence of sleep apnea syndrome in patients with long-term pacing: the European Multicenter Polysomnographic Study.

BACKGROUND: Cardiovascular diseases leading to pacemaker implantations are suspected of being associated with a high rate of undiagnosed sleep apnea syndrome (SAS). We sought to determine the prevalence and consequences of SAS in pacemaker patients according to pacing indications: heart failure, symptomatic diurnal bradycardia, and atrioventricular block. METHODS AND RESULTS: Ninety-eight consecutive patients (mean age, 64+/-8 years) not known to have sleep apnea were included; 29 patients were paced for dilated cardiomyopathy (29%), 33 for high-degree atrioventricular block (34%), and 36 for

Supraventricular tachycardia in infants, children and adolescents: diagnosis, and pharmacological and interventional therapy.

Supraventricular tachycardia is the most frequent form of symptomatic tachydysrhythmia in children. Neonates and infants with paroxysmal sup … tachycardias. Adenosine is the drug of first choice in any age group for tachycardias involving the atrioventricular node; its advantages include short half-life and minimal or absent negative inotropic effects. Adenosine may also be used in patients with wide QRS complex tachycardia. Intravenous verapamil is contraindicated in neonates and infants because of the high …

Successful cryoablation of Mahaim tachycardia in a child with Ebstein's anomaly.

Mahaim fibers with decremental atrioventricular (AV) node-like conduction properties comprise less than 3 % of accessory pathways. Radiofrequency ablation of right atriofascicular pathways guided by a distinct Mahaim potential detected at the anterolateral to posterolateral tricuspid annulus or in the right ventricular free wall is a safe and highly effective treatment method. The case report presents a 16-year-old boy with Ebstein's anomaly and symptomatic wide complex tachyarrhythmia. The electrophysiologic study and the entire ablation procedure were performed using a three-dimensional mapp

Pediatric arrhythmias.

During the past decade, our awareness and understanding of arrhythmias in children has expanded immensely. This report discusses the more commonly encountered pediatric rhythm disturbances, including sinus node dysfunction, the various forms of supraventricular tachycardia, ventricular tachycardia, long QT syndrome, and the atrioventricular blocks. The electrocardiographic characteristics, electrophysiological mechanisms, clinical presentation, and current acute and chronic management options for each are described.

Supraventricular tachycardia. Changes in management.

The various forms of supraventricular tachycardia can be differentiated by careful review of a patient's electrocardiogram. If reentrant tachycardia involves the atrioventricular node, intravenous adenosine (Adenocard) provides a rapid means of converting the tachycardia to sinus rhythm. Adverse effects of adenosine, though common, are mild and of brief duration. Wide QRS complex tachycardia of ventricular origin is usually not affected by the drug.

[Right ventricular infarct and atrioventricular blocks].

Right ventricular infarction in inferior left ventricular infarction is very common. We have examined, by means of equilibrium gated radionuclide angiography, the relationship existing between right ventricular infarction and atrioventricular block. Seventy-two patients with inferior wall myocardial infarction were studied. Thirty-nine of them had ventriculographic evidence of right ventricular infarction. Fifteen of them (38%) had a-v block in the acute phase. Only 3 patients without right ventricular involvement (9%) had a-v block. The occurrence of a-v block is usually explained by the fact

Hyperhomocysteinemia Alters Sinoatrial and Atrioventricular Nodal Function: Role of Magnesium in Attenuating These Effects.

Patients with hyperhomocysteinemia (HHcy), or elevated plasma homocysteine (Hcy), are at higher risk of developing arrhythmias and sudden cardiac death; however, the mechanisms are unknown. In this study, the effects of HHcy on sinus node function, atrioventricular conduction, and ventricular vulnerability were investigated by electrophysiological (EP) analysis, and the role of magnesium (Mg(2+)), an endogenous N-methyl-D-aspartate (NMDA) receptor antagonist, in attenuating EP changes due to HHcy was explored. Wild-type mice (WT) and mice receiving Hcy in the drinking water for 12 weeks (DW) w

[Three AV-nodal pathways in a patient with atypical AV-nodal reentry tachycardia].

In a female patient with paroxysmal AV nodal reentrant tachycardias the electrophysiological study revealed three AV nodal pathways. During atrial extrastimulation a sudden AH interval prolongation of more than 50 ms ("break" phenomenon) was observed twice at one basic cycle length. During ventricular extrastimulation a sudden prolongation of the AH interval of the anterograde AV nodal conduction of the induced echo beats was recorded. Three AV nodal pathways were thus present. The atypical form of AV nodal reentrant tachycardia was induced, showing a varying cycle length (290-340 ms). After r

Electrophysiologic effects and efficacy of cibenzoline in patients with supraventricular tachycardia.

Electrophysiologic effects of intravenous (i.v.) cibenzoline were evaluated in 18 patients with accessory pathways or dual atrioventricular (AV) nodal pathways (12 men and 6 women with a mean age of 44 +/- 18 years). Twelve patients had accessory AV pathways, including 6 patients with a manifest accessory pathway. Six patients had AV nodal reentrant tachycardia (AVNRT). Electrophysiologic studies were performed before and after cibenzoline (1.4 mg/kg i.v.) infusion for 5 min. Sinus cycle length did not change significantly after cibenzoline administration. Cibenzoline increased both the AH (85

Electrophysiologic effects of sotalol on the immature mammalian heart.

Sotalol is a beta-blocker with class III antiarrhythmic properties that has recently been used in children for the treatment of supraventricular and ventricular arrhythmias. However, little is known about its electrophysiologic effects on the immature heart. Using intracardiac electrocardiographic recordings and stimulation techniques, 15 canine neonates (8-15 days) and 15 adult mongrel dogs were studied with cumulative doses of sotalol (0.5, 1, 2, and 4 mg/kg plus an additional dose of 8 mg/kg for neonates). Heart rate decreased significantly in the two groups, but more in adult dogs (-43% in

The Atrial Pacing Peri-ablation for Paroxysmal Atrial Fibrillation (PA3) Study: rationale and study design.

The Canadian Atrial Pacing Peri-Ablation for Paroxysmal Atrial Fibrillation Study tested the hypotheses that atrial pacing prevents paroxysmal atrial fibrillation (PAF) in patients without symptomatic bradycardia and that DDDR pacing is more likely to prevent PAF following total atrioventricular (AV) node ablation compared to VDD pacing. Patients with PAF who were refractory to or intolerant of antiarrhythmic drug therapy received a Medtronic Thera DR pacemaker 3 months prior to a planned total AV node ablation. Patients were randomized to atrial pacing or no pacing therapy. The time to first

The mechanism of paroxysmal supraventricular tachycardia--PSVT due to peri-AV nodal reentry.

To evaluate the role of perinodal tissue in the genesis of atrioventricular (AV) nodal reentry, echo beats were induced by programmed stimulation in nine superfused rabbit AV nodal preparations which included the crista terminalis (CT) input, interatrial septal (IAS) input and perinodal atrial tissues in addition to the AV node. Two patterns of AV reentry were observed: In six preparations, premature response was blocked within the AV node, but anterograde conduction continued along the perinodal fibers and entered the AV node by way of another input region. Reentry occurred via retrograde nod
