INTRODUCTION: Arachnoid cysts are benign cerebrospinal fluid collection within a duplication of arachnoid membrane and, when found in the retrocerebellar site, they may be associated with tonsils herniation. This rare situation of coexisting retrocerebellar arachnoid cyst (AC) and Chiari malformation type 1 (CM-1) have been previously reported in few cases (10 patients) with syringomyelia and hydrocephalus described to be the most relevant issues. The aim of this paper is to describe 3 pediatric cases of this condition with a systematic review of the literature, underlining the importance of s

Opinions have varied regarding the optimal treatment of an unstable Hangman's fracture. C2 pedicle screw instrumentation is a biomechanically strong fixation which although done through a simple posterior approach, is a technically demanding procedure. This prospective, non-randomized multicentre study included 15 consecutive patients with displaced type II traumatic spondylolisthesis of the axis. There were nine males and six females with a mean age of 37 years at surgery. The cause of injury was a road traffic accident in 11 patients and a fall from height in 4 patients. All patients had a s

Sufficient internal fixation for occipitocervical (OC) or C1-C2 instability in pediatric patients with Down syndrome is difficult owing to small osseous structures, congenital deformities, and immature ossification. The purpose of this study was to evaluate the clinical outcomes of patients aged 8 years or younger with Down syndrome who underwent OC or C1-C2 fusion using freeze-dried allograft as bone graft substitute. The participants included seven consecutive patients aged 8 years or younger with Down syndrome who were treated for upper cervical disorders with posterior OC or C1-C2 fusion u

The authors describe a less invasive approach for a disc herniation at the high cervical region. A 68-year-old female patient presented with posterior neck and shoulder pain, and tingling sensation and numbness in the left hand after she fell down stairs 2 months before presentation. On neurologic examination, the power of flexion and extension of the left elbow and grasping of the left hand was decreased. Hoffmann sign was positive in both hands. Magnetic resonance imaging showed a huge herniation of the C2-C3 disc compressing the left paramedian area of the spinal cord. After a routine surgi

Fibrous dysplasia of the cervical spine is rare. No prior reports have discussed odontoid fractures in the setting of fibrous dysplasia. We describe a 26-year-old man who suffered a traumatic odontoid fracture in an area of preexisting fibrous dysplasia. The patient was treated conservatively in a sterno-occipito-mandibular immobilizer brace with a good result. A review of fibrous dysplasia is presented. Alternative methods of diagnosis and treatment options in our patient are discussed.

Of four hangman's fractures of the axis, three occurred in road accidents and were stable, undisplaced and free of neurological signs, with full recovery after six to twelve weeks in a cervical collar. The fourth fracture occurred in a fall with profound tetraparesis from haemorrhage into the spinal cord, and the patient died a week later. There are two types of hangman's fracture: that of the axis pedicle, which results from extension and compression of the cervical column, is more common than usually supposed because of the slight symptoms it causes.

PURPOSE: To evaluate the usefulness of routine performance of computed tomography (CT) of the craniocervical junction in unconscious patients with substantial head injury.
MATERIALS AND METHODS: In a prospective study, CT of the head and the cervicocranium was performed in 202 patients with substantial cranial trauma (Glasgow Coma Scale scores of 3-6). Plain radiography was performed in all patients. Radiographs and CT scans were then blindly interpreted.
RESULTS: Twenty-eight patients (13.9%) had C-1 or C-2 fractures; plain radiographs did not demonstrate cervical fractures in 11 of these p

PURPOSE: Atlantoaxial rotatory fixation and atlantoaxial rotatory subluxation are the most frequent manifestations of atlantoaxial rotatory dislocation (AARD) in pediatric population and are often treated conservatively. The objective of this study is to correlate the changes highlighted on MRI T2-weighted and STIR sequences with the duration of conservative treatment.
METHODS: We analyzed nine consecutive patients treated surgically between 1 Jan 2006 and 1 Jan 2010 at the Policlinico Umberto I of Rome. All patients underwent cervical X-ray, computed tomography and magnetic resonance imaging

Fifteen patients with atlanto-axial instability (secondary to os odontoideum in three, nonunion of an odontoid fracture in seven, acute odontoid fracture in three, and rheumatoid arthirtis in two) were treated by wedge compression arthrodesis of the atlanto-axial joint. One patient died at home eight weeks after fusion with the cause of death never established. Of the two patients with rheumatoid arthritis (ankylosing spondylitis), one had a non-union and in the other the posterior arch of the atlas fractured and the fusion had to be extended up to the occiput and down to the third cervical ve

STUDY DESIGN: Retrospective case series.
OBJECTIVE: Compensatory changes above a proximal junctional kyphosis (PJK) have not been defined. Understanding these mechanisms may help determine optimal level selection when performing revision for PJK. This study investigates how varying PJK location changes proximal spinal alignment.
METHODS: Patients were grouped by upper instrumented vertebrae (UIV): lower thoracic (LT; T8-L1) or upper thoracic (UT; T1-7). Alignment parameters were compared. Correlation analysis was performed between PJK magnitude and global/cervical alignment.
RESULTS: A tota

Some older adults with spinal deformity maintain standing posture via pelvic compensation when their center of gravity moves forward. Therefore, evaluations of global alignment should include both pelvic tilt (PT) and seventh cervical vertebra-sagittal vertical axis (C7-SVA). Here, we evaluate standing postures of older adults using C7-SVA with PT and investigate factors related to postural abnormality. This cross-sectional study used an established population-based cohort in Japan wherein 1121 participants underwent sagittal whole-spine radiography in a standing position and bioelectrical imp

BACKGROUND: Unilateral atlantoaxial osteoarthritis (AAOA) is an underdiagnosed condition characterized by severe occipitocervical pain and restricted head rotation. This study aimed to compare clinical outcomes and complications associated with different surgical methods used to treat AAOA in cases where conservative management fails.
METHODS: This was a retrospective cohort study conducted at Tampere University Hospital, Finland, with patients treated between 2004 and 2024. Patients were divided into two groups based on the surgical approach. Clinical outcomes were evaluated at a median foll

OBJECTIVE: Selection of the proximal upper instrumented vertebra (UIV) in multilevel posterior cervical fusion (PCF) remains controversial. This study compared sagittal alignment, craniocervical compensatory behavior, and perioperative outcomes between C2-T1 and C3-T1 PCF constructs.
METHODS: A retrospective cohort study was performed, including 79 adult patients who underwent PCF from either C2-T1 (n = 41) or C3-T1 (n = 38) with a minimum of 18 months of follow-up. Demographic, perioperative, and complication data were collected. Preoperative and postoperative radiographs were analyzed for g

INTRODUCTION: Morphometric evaluation of the pedicle and isthmus of second cervical vertebra (C2) (Axis) is extremely vital before contemplating any surgical stabilization involving the Craniovertebral region, in view of its proximity to the vertebral artery and the cervical nerve root. The dimensions of pedicles and isthmuses in C2 vary between individuals and there is paucity of data in the Indian population. This study strives to measure the average pedicle and isthmus dimensions in a sample of population, which would enable selection of screws with safest diameters to be used in C2; thereb

BACKGROUND: Neurofibromatosis type 1 (NF-1) can cause severe kyphosis in the cervical vertebrae. There is no consensus on the optimal surgical treatment for this rare condition, although long-segment fixation and combined approaches are generally preferred. To our knowledge, this study is the first to report the clinical outcomes of patients with NF-related cervical kyphosis who underwent stand-alone posterior pedicle fixation surgery.
METHODS: The outcomes of 14 patients who underwent surgery using the pedicle screw were retrospectively examined between 2015 and 2022. Only patients with at l
