Thiamine deficiency-related neuropathy: A reversible entity from an endemic area.

BACKGROUND AND PURPOSE: Despite thiamine deficiency being a lesser-known entity in modern times, beriberi in various forms, including thiamine deficiency-related neuropathy, remains endemic in Kashmir due to the consumption of polished rice as a staple food. This observational study investigates cases of peripheral neuropathy of unknown etiology and their potential responsiveness to thiamine administration. METHODS: This prospective study enrolled adult patients presenting to the emergency department with weakness consistent with thiamine deficiency-related neuropathy and conducted a therapeu…

Thiamine Deficiency in a Nondrinker and Secondary Pulmonary Edema after Thiamine Replenishment.

A 48-year-old man was brought to our emergency room with acute abdominal pain and systemic edema, indicating acute circulatory failure with lactic acidosis. Furosemide treatment paradoxically worsened the systemic edema and induced confusion. He had no drinking history but hardly ate legumes or meats containing thiamine. Administration of fursultiamine dramatically improved the symptoms and subsequently caused pulmonary edema. Thiamine deficiency may occur in nondrinkers with an unbalanced diet. In this condition, diuretic therapy can worsen the symptoms before thiamine supplementation by prom

[Polyneuropathy deficiency among Xavante indians].

UNLABELLED: The authors present two cases of polyneuropathy deficiency among Xavante indians where the sole food was rice in case 1 and almo … N: The Xavante polyneuropathy deficiency is caused by thiamine (vitamin B1) deficiency, that is dry beriberi, owing to consumption of industrially processed rice as sole or almost sole food. The Xavante polyneuropathy is different from the neuropathy observed among Kreen-Akrore indians and from that of adolescent indians in the Xingu Park.

[Alcoholic beri-beri: a not yet clearly understood pathogenesis. Problems presented by a particularly clinical complex case].

A detailed clinical and autoptic study of a case of alcoholic beri-beri is presented. In the light of recent experimental literature, the symptoms support the as yet tentative new hypothesis that pathogenesis may relate to the parasympathetic system, in which case there may be a concrete possibility of immediate medical cure.

A cautionary tale for health education initiatives in vulnerable populations: Improving nutrition in Haiti prisons.

INTRODUCTION: Prisons in low-income countries have barriers to providing adequate nutrition to the incarcerated. This perspective discusses … a vitamin C intake insufficient to prevent scurvy and 91.6% had vitamin B1 insufficient to prevent beriberi. In the follow-up period, caloric intake decreased to a mean of 454 per day (p < 0.001). The proportion of incarcerated men who had insufficient vitamin C and vitamin B1 to prevent disease increased in the follow-up period. DISCUSSION: The caloric …

[Application and significance of fortification in prevention of micronutrient deficiency-induced diseases].

Fortification is defined as adding of one or more essential elements to food article, regardless of whether it has been already added to food or not, in order to prevent or correct deficiency of one or more nutrients in the general population or specific population group. Food fortification with minerals and vitamins helps eliminate diseases such as goiter, rickets, beriberi, and pellagra. Significant results have been also achieved in prevention of anemia and vitamin A deficiency. The aforementioned deficiencies can be prevented and eliminated by means of appropriate and diverse nutrition and…

Vitamin B1 functions as an activator of plant disease resistance.

Vitamin B(1) (thiamine) is an essential nutrient for humans. Vitamin B(1) deficiency causes beriberi, which disturbs the central nervous and circulatory systems. In countries in which rice (Oryza sativa) is a major food, thiamine deficiency is prevalent because polishing of rice removes most of the thiamine in the grain. We demonstrate here that thiamine, in addition to its nutritional value, induces systemic acquired resistance (SAR) in plants. Thiamine-treated rice, Arabidopsis (Arabidopsis thaliana), and vegetable crop plants showed resistance to fungal, bacterial, and viral infections. Thi…

Toxicity of glyoxals--role of oxidative stress, metabolic detoxification and thiamine deficiency.

Glyoxals are reactive alpha-oxoaldehydes that are formed endogenously from sugars, the levels of which are increased in various pathological … ple sugars increased the number of aberrant crypt foci/colon in the absence of clinical evidence of beriberi. This was associated with decreased plasma thiamine and low erythrocyte transketolase activity. Western diets, which are frequently poor in thiamine and high in sugars, could result in increased levels of endogenous glyoxals, which in turn may lead …

[Axel Holst and Theodor Frolich--pioneers in the combat of scurvy].

Axel Holst (1860-1931), professor of hygiene and bacteriology at the University of Oslo and paediatrician Theodor Frølich (1870-1947) became interested in a disease termed "ship beriberi" which afflicted the crews of sailing ships, and which showed an uncanny likeness to scurvy. They suspected a nutritional deficiency, and established an animal model that allowed systematic study of factors that led to disease as well as the preventive value of different substances. The choice of the guinea pig as the experimental animal for these studies was one indeed fortuitous, as that species has been sho…

The starvation in the fir …

Addressing Cognitive Biases in Interpreting an Elevated Lactate in a Patient with Type 1 Diabetes and Thiamine Deficiency.

We present the case of a young woman admitted for diabetic ketoacidosis with persistent, asymptomatic lactic acid (LA) elevation during the evolving COVID-19 pandemic. Cognitive biases in interpreting an elevated LA in this patient's care resulted in an extensive infectious workup instead of the low-cost and potentially diagnostic provision of empiric thiamine. We discuss clinical patterns and etiologies of LA elevation and the role of thiamine deficiency. We also address cognitive biases potentially affecting the interpretation of elevated lactate levels and provide guidance for clinicians to

Outbreak of beri-beri in The Gambia.

In 1988, an outbreak of beri-beri occurred in The Gambia during the rainy season. At least 140 people were affected, and 22 died. A clear response to thiamine was observed in 8 patients who were in hospital, and the outbreak resolved after distribution of thiamine in the community. Gradual replacement of traditional cereals with imported milled rice may have increased susceptibility to beri-beri.
