A case of gastric retention caused by gallstones (Bouveret's syndrome) is reported. One of the two causal stones was lodged in the pylorus and the other in the cap of the duodenum. Preoperatively a fistula was demonstrated between the gallbladder and the pylorus. The clinical and roentgenologic findings are described. The diagnosis of Bouveret's syndrome can be established by gastroscopy.

A case of gallbladder empyema associated with multiple complications caused by a delay in diagnosis is presented. Recent literature is reviewed especially emphasizing the clinical presentation with scanty physical signs. Greater awareness of the indolent and chronic presentation might reduce the considerable morbidity and mortality that are associated with delayed operative intervention.

Between 1969 and 1982 8 patients of ours were operated on for gallstone ileus. Signs found in the preoperative x-ray examination indicating this condition are analyzed retrospectively. The frequency and specificity of the x-ray signs found in the abdominal survey radiograph are compared with those reported in the literature. The value of oral contrast media administration in the detection of a cholecystointestinal fistula and localization of the obstruction is discussed.

Two cases of gallstone ileus are reported. Treatment of the occlusion and the underlying bilio-digestive fistula should be carried out in one surgery session. Careful preoperative preparation of these patients from the hydric, electrolytic and metabolic viewpoints is the indispensable premise for single session treatment.
