Intraoperative hemodilution and autologous platelet rich plasma collection: two techniques for collecting fresh autologous blood.

Intraoperative hemodilution (IH) and autologous platelet rich plasma (APRP) collection are two techniques used to obtain autologous blood in the operating room. They have been used to reduce allogeneic blood exposure in patients undergoing both cardiac and non-cardiac surgery. Both components have the advantage of providing fresh blood not subject to the storage lesion. Whole blood (IH) or platelet rich plasma is removed from the patient as anesthesia is induced and replaced with acellular fluid. The blood is transfused back after bypass or major bleeding has ceased. Although used commonly, th

Treatment of sickling disorders.

Sudden death in military recruits with sickle cell trait appears to be related to hyperthermia and its consequences and can probably be prevented by use of sensible precautions and heightened awareness of the risk. Sickle cell disease can be treated by decreasing the proportion of sickle cells through transfusion; indications and pathophysiology of such transfusions are beginning to become clear. Sickle cell disease can be prevented if erythrocytes can be prevented from sickling. Dilution of hemoglobin S within erythrocytes, by stimulating fetal hemoglobin production, increasing cell water, or

[Current status of viral safety of virus inactivated factor VIII and IX concentrates in treatment of hemophilia].

Based on the results of seven ICTH-controlled PUP studies only the virus-inactivating procedures by pasteurization (60 degrees C for 10 h), tri-n-butyl-phosphate/detergent treatment, and vapor heating (60 degrees C for 10 h and 1,200 mbar) have prevented the haemophilic patients from infections with hepatitis B, C and HIV. After the disaster of the HIV infection in haemophilic patients at the beginning of the 80's the factor concentrates given today seem to be safe for virus infections during substitution therapy. To prove this high therapeutical standard there is a need for a prospective long

Selection of platelets for refractory patients by HLA matching and prospective crossmatching.

A multi-site clinical study compared platelets chosen for refractory patients by prospective platelet crossmatching using stored donor platelets and HLA-based selection. Seventy-three patients who were refractory to random-donor platelets received two plateletpheresis components, one chosen by HLA-based criteria and the other by crossmatching. Patients were carefully evaluated to exclude nonimmune factors that could adversely affect transfusion results. Each of the five study sites used a crossmatch procedure with which it had experience. Results from this study indicate the following: 1) The

A mathematical model for fresh frozen plasma transfusion strategies during major trauma resuscitation with ongoing hemorrhage.

BACKGROUND: Randomized controlled trials of how best to administer fresh frozen plasma (FFP) in the presence of ongoing severe traumatic hemorrhage are difficult to execute and have not been published. Meanwhile, coagulopathy remains a common occurrence during major trauma resuscitation and hemorrhage remains a major cause of traumatic deaths, suggesting that current coagulation factor replacement practices may be inadequate. METHODS: We used a pharmacokinetic model to simulate the dilutional component of coagulopathy during hemorrhage and compared different FFP transfusion strategies for the

Improving the bacteriological safety of platelet transfusions.

Despite the increased application of aseptic techniques for blood collection and the preparation of platelet concentrates, morbidity and mortality arising from the transfusion of bacterially contaminated allogeneic platelet products persist. This problem exists because stored platelet concentrates represent a nearly ideal growth medium for bacteria and because they are stored at temperatures (22 degrees +/- 2 degrees C) that facilitate bacterial growth. The presence of bacteria in blood components including platelets has been a problem for many decades and currently is the most common microbio

End-tidal carbon dioxide monitoring to predict hypovolaemic shock and the subsequent need for blood transfusions in adult pre-hospital trauma patients: a systematic review.

INTRODUCTION: Traumatic haemorrhage is a major cause of preventable death, and blood transfusion is a crucial component of damage control resuscitation. Several diagnostic strategies exist to anticipate hypovolaemic shock and subsequent blood transfusion requirements, but their pre-hospital utility is inconsistent. Capnography or end-tidal carbon dioxide (EtCO2) measurement has received growing recognition for prognostic capability in metabolic, respiratory and cardiac disease. This systematic review examines EtCO2 efficacy in predicting hypovolaemic shock and subsequent blood transfusion in p

Haemotherapy for acute haemorrhage.

In this review blood component therapy for acute haemorrhage is summarised. As the haemotherapy is frequently the cornerstone of a successful outcome of haemorrhagic shock, attention to details in relation to the indications, safety and efficiency is essential. Massive blood transfusion brings with it many potential complications which may jeopardise a successful outcome for the patient after skillful medical and surgical care has controlled the basic problem.

ABO identical and washed blood transfusions as candidate strategies to reduce early mortality in acute promyelocytic leukemia.

BACKGROUND: Despite dramatically improved long term outcomes seen with all-trans retinoic acid therapy, and now arsenic trioxide, in acute promyelocytic leukemia (APL), early mortality remains a substantial challenge. Recent data from a single center study and the Surveillance, Epidemiology and End Results (SEER) registry report 30day mortality rates of 26% (n=18 of 70) and 17% (n=238 of 1400), respectively. Early deaths are predominately due to hemorrhage. Patients with APL invariably have abnormal laboratory hemostasis tests. The standard of practice is to prophylactically transfuse platelet

Le Fort I maxillary osteotomy in a Jehovah's Witness patient: strategies for minimizing blood loss and maximizing safety.

BACKGROUND: The Watch Tower Society, the main governing organization of the Jehovah's Witness (JW) faith, introduced the doctrine to refuse blood in 1945 and has been enforcing it since 1961. A member can be expelled for accepting prohibited blood components. Many reconstructive surgeries place patients at an increased risk for blood loss. There have been attempts at reducing the rate of transfusions in craniofacial surgery, even in patients not opposed to it. PRESENTATION: A 15-year-old female patient, who refused blood transfusion due to her faith, presented with a class III malocclusion, t

Evaluation and management of bleeding during cardiac surgery.

Patients undergoing cardiac surgery with and potentially without cardiopulmonary bypass (CPB) are at risk for excessive microvascular bleeding. This bleeding often leads to transfusion of allogeneic blood and blood components as well as reexploration. Excessive bleeding after cardiac surgery occurs because of alterations in the hemostatic system pertaining to dilutional thrombocytopenia, excessive hemostatic activation, and exposure to long-acting antiplatelet or antithrombotic agents. Pharmacologic interventions have been extensively reported as means to attenuate the alterations in the hemos

Transfusion support using filtered unscreened blood products for cytomegalovirus-negative allogeneic marrow transplant recipients.

It has been suggested that leukoreduced unscreened blood products can be used as an alternative to components from cytomegalovirus (CMV)-seronegative donors in order to prevent transmission of CMV from transfusions for CMV-seronegative marrow transplant recipients with CMV-seronegative donors, but confirmatory data are lacking. A retrospective chart review was undertaken for patients undergoing allogeneic transplantation over a 4-year period during which blood products were filtered for CMV-seronegative patients with CMV-seronegative donors when CMV-seronegative components were not available.

Haemovigilance: an effective tool for improving transfusion practice.

Haemovigilance is a tool to improve the quality of the blood transfusion chain, primarily focusing on safety. In this review we discuss the history and present state of this relatively new branch of transfusion medicine as well as some developments that we foresee in the near future. The top 10 results and conclusions are: (1) Haemovigilance systems have shown that blood transfusion is relatively safe compared with the use of medicinal drugs and that at least in Europe blood components have reached a high safety standard. (2) The majority of the serious adverse reactions and events occur in th

Induced Pluripotent Stem Cell-Derived Red Blood Cells and Platelet Concentrates: From Bench to Bedside.

Red blood cells and platelets are anucleate blood components indispensable for oxygen delivery and hemostasis, respectively. Derivation of these blood elements from induced pluripotent stem (iPS) cells has the potential to develop blood donor-independent and genetic manipulation-prone products to complement or replace current transfusion banking, also minimizing the risk of alloimmunization. While the production of erythrocytes from iPS cells has challenges to overcome, such as differentiation into adult-type phenotype that functions properly after transfusion, platelet products are qualitativ

Transfusion of Whole Blood Is Not Associated With Increased Rate of Hemolytic Transfusion Reaction.

INTRODUCTION: Low-titer cold-stored whole blood (WB) transfusion provides multiple advantages over component therapy (CT) in resuscitation of the hemorrhaging patient. However, concern regarding the risk of hemolytic transfusion reaction (HTR) is a barrier for continued expansion of the use of WB. HTR is most often seen after transfusion of packed red blood cells (RBCs), and the rate of HTR after WB transfusion remains largely unknown. We sought to investigate the incidence of HTR after WB transfusion at our institution via a retrospective observational study. Given that the pathogenesis of HT

Caregivers' and healthcare providers' perceptions of blood availability, safety, and acceptability for patients with cancer in southwestern Uganda.

Blood transfusion is crucial in supporting the treatment of patients with cancer. However, in poor-resource settings, blood and blood components remain scarce, and their perceived health risks may hinder their use. This study explored the perceptions of blood availability, safety, and acceptability among caregivers and healthcare providers of patients with cancer at Mbarara Regional Referral Hospital in southwestern Uganda. This study employed a qualitative design involving in-depth interviews and key informant interviews with caregivers of patients with cancer and healthcare providers, betwee

Multidisciplinary consensus document on the management of massive haemorrhage (HEMOMAS document).

Massive haemorrhage is common and often associated with high morbidity and mortality. We perform a systematic review of the literature, with extraction of the recommendations from the existing evidences because of the need for its improvement and the management standardization. From the results we found, we wrote a multidisciplinary consensus document. We begin with the agreement in the definitions of massive haemorrhage and massive transfusion, and we do structured recommendations on their general management (clinical assessment of bleeding, hypothermia management, fluid therapy, hypotensive
