Twenty-two patients underwent 23 femoral and tibial lengthenings by the Wagner technique from 1977 to 1987. Average length gained was 5.8 and 5.2 cm, respectively. Long hospitalization in addition to multiple operative procedures and a high complication rate created a significant psychological impact. Fourteen of 22 patients experienced psychological problems, primarily adjustment disorders, all of which resolved without long-term sequelae. OBJECTIVE: Congenital absence of the tibia (congenital tibial aplasia) causes significant disability when walking. Lengthening of a nonfunctional posttraumatic below elbow stump via Ilizarov technique is described. Preoperatively, the 5 cm ulna in the residual forearm provided an insufficient lever arm for practical use of a standard below elbow prosthesis. An additional 5 cm of ulnar length was achieved (100% increase) by distraction histogenesis, ultimately limited by impending necrosis of adherent skin covering the tip of the residual limb. Utilizing the Ilizarov technique, ulnar lengthening converted the functional level of the patient's amputation from elbow disarticulation to below elbow status.

Inequality of leg lengths is associated with low back pain, greater trochanteric bursitis, and degenerative hip disease. The pelvic obliquity that results from a discrepancy of leg lengths may contribute to degenerative changes in the lumbar spine. The diagnosis of leg length inequality may be overlooked. If this occurs, extensive and costly investigations are then often ordered to evaluate these secondary manifestations of leg length inequality. This is unfortunate because a simple shoe lift is all that was needed to provide symptomatic relief for selected patients.
