Decortication in the Surgical Management of Complete Atypical Femoral Fractures: A Strategy to Accelerate Fracture Healing.

Background/Objectives: Surgical management of atypical femoral fractures (AFFs) stabilized with intramedullary (IM) nailing is frequently challenged by delayed union or nonunion due to the severely suppressed bone turnover characteristic of bisphosphonate-related bone pathology, often leading to a hypertrophic nonunion-like state at the fracture site. This consecutive case series aimed to evaluate the effectiveness of intraoperative percutaneous decortication at the hypertrophic cortex in promoting rapid bone healing in complete AFFs. Methods: This was a single-center consecutive case series o

The impact of subsidence on hip function: A comparative study between Proximal Femoral Nail Anti-rotation-2 and Halifax Femoral Nail in unstable trochanteric fractures.

BACKGROUND: Unstable trochanteric fractures pose a significant challenge in orthopedic surgery, particularly in elderly patients with poor bone quality. Achieving biomechanical stability while minimizing complications requires careful consideration of intramedullary nail design. This study aims to compare two commonly used implants - Proximal Femoral Nail Anti-rotation-2 (PFNA-2) and Halifax Femoral Nail - by evaluating their clinical, radiological, and functional outcomes, with a focus on surgical efficacy, complications, and functional recovery. METHODS: A prospective, randomized clinical t

Patient-Reported Outcomes After Intramedullary Nailing of Oncologic Impending or Pathologic Fractures With Carbon Fiber or Titanium Implant.

INTRODUCTION: Despite the benefits of intramedullary nailing (IMN) of impending or pathologic fractures in oncologic patients, literature on patient-reported outcomes (PROs) is scarce in patients treated with carbon fiber (CF) nails. Our study compared postoperative PROs after IMN with CF or titanium implants. METHODS: We conducted a retrospective propensity score-matched cohort study of patients treated at our institution with CF or titanium nails for impending or pathologic fractures from metastatic bone disease. Finite element analysis of PFNA in the treatment of osteoporotic intertrochanteric A2.3 fractures of the femur.

BACKGROUND: Poor bone quality in osteoporosis can compromise implant fixation and stability in intertrochanteric fractures. This study employed finite element analysis to evaluate the biomechanical performance of short (170 mm), medium (240 mm), and long (320 mm) proximal femoral nail antirotation (PFNA) implants in osteoporotic intertrochanteric A2.3 fractures. By modulating the elastic modulus of cancellous bone to simulate varying degrees of osteoporosis severity, we aimed to identify the optimal PFNA length for clinical practice. METHODS: A three-dimensional finite-element model of an ost

[Intramedullary nailing of fractures of forearm in adults.].

The authors present a three-year retrospective group of 32 adult patients with the diaphyseal fractures of the forearm treated surgically. Only in 3 cases a standard plate technique was used. All other surgeries were performed by intramedullary nailing using Prevot nails, strong Kirschner wires, True/Flex flexible nails and a bilaterally locked Foresight nail. Long-term results were in detail evaluated in a group of 15 patients with the implanted True/Flex nail. Minimum period of the follow-up was 6 months after operation, the average was 14,7 months. The nailing of both bones of the forearm w

Further clinical experience with the extensible nail.

The extensible nail for treatment of children with brittle bones offers distinct advantages over the conventional rod. It appears to elongate with growth and thus obviates periodic replacement of the rod as the child grows. Complications have been few in this and other series. Recent modifications of the technique for the femur and tibia, and various changes in the original design, constitute significant improvements in the intramedullary nail method.

Antibiotic-Coated Nail in Open Tibial Fracture: A Retrospective Case Series.

Implant-associated infections still represent one of the main problems in treatment of open fracture. The role of systemic antibiotic prophylaxis is now agreed and accepted; however, recent literature seems to underline the importance of local antibiotic therapy at the fracture site, and antibiotic nails have been shown to play a role in the treatment of open fractures in terms of fracture healing and lower risk of infection. We retrospectively analyzed our results, from January 2016 to March 2020, with the use of coated nails in the treatment of open tibial fractures, evaluating the rates of

Clamshell Corticotomy: A Technique to Address Challenges of Narrow Medullary Canal during Intramedullary Nailing of Tibial Shaft Fracture Fixation.

Intramedullary nailing for diaphyseal fractures is a gold standard procedure, that is, often associated with some intraoperative challenges. Fixing an intramedullary nail in a narrow canal in an unplanned surgery is a nightmare. Forceful implantation leads to iatrogenic fracture or comminution at the fracture site. Forceful reaming in such scenarios is also associated with thermal osteonecrosis and at times incarceration of flexible reamer head. It is not uncommon to face a challenge intraoperatively with regards to a narrow medullary canal, however, well we plan preoperatively. Clamshell oste

Innovative Technique in Extended Intramedullary Humeral Lengthening: A Case Report.

CASE: Intramedullary humeral lengthening is still a very uncommon procedure to treat humeral limb length discrepancy (LLD). This approach has various advantages compared with external devices but is limited in its ability to lengthen bones because of the length of the telescopic nail. This case report describes a middle-aged patient with a short right humerus secondary to a childhood growth plate injury that was successfully lengthened with an off-label application of a tibial distracting device. Hereby, a novel treatment technique with unlocking, backwinding, and reinterlocking of the telesco

Knee arthrodesis with modular megaprosthesis as salvage procedure for the limb following in a patient with an infected knee tumor prosthesis: A case report.

INTRODUCTION: The management of periprosthetic knee infections is a complex event, especially in the context of megaprostheses. An accepted option in these cases is knee arthrodesis, aiming to salvage the limb with acceptable and pain-free functionality. We present a successful case of knee arthrodesis using a hybrid model of modular nail-prosthesis in a patient with massive bone stock loss following the removal of an infected tumor-type prosthesis. CASE PRESENTATION: A 65-year-old male patient with a chronic periprosthetic infection related to a resection prosthesis implanted 5 years earlier

Humeral nailing revisited.

Unreamed interlocked humeral nailing for stabilisation of acute humeral fractures was introduced a decade ago. Antegrade and retrograde nail insertion are equally popular. The role of nailing as opposed to plating of humeral fractures is the subject of continuous debate. Between 1997 and 2005, 99 acute fractures of the humeral shaft were treated operatively with the unreamed humeral nail (UHN, Synthes) in our Level I Trauma Centre. The mean age of the patients was 63 years. Only eight patients (8.1%) were polytraumatised, nine patients had an open fracture (9.1%), five had a primary radial ner

Epiphysiodesis in slipped capital femoral epiphysis: a comparison of various surgical modalities.

This retrospective study surveys patients treated between 1960 and 1978 for slipped capital femoral epiphyses. The parameters reported are fusion time, blood replacement, hospitalization time, aseptic necrosis, cartilage necrosis, and complications. Twenty-eight hips (24 patients) were treated by pinning, and three hips (three patients) were treated by Smith-Petersen nails. The Heyman or Howorth bone pegging procedures were performed in 48 hips in 41 patients. The average fusion time was 9.4 months for hips treated with Knowles pins, 3.7 months for Hagie or Tachdjian pins, and 4.8 months for b

Phalangeal regrowth in rodents: postamputational bone regrowth depends upon the level of amputation.

Conflicting reports of distal phalangeal regrowth prompted a reexamination of bone growth following phalangeal amputation in mammals. Digits of neonatal and adult mice and rats were amputated at various levels. The short-term response was examined on histological sections, and long-term growth was documented by alizarin red-staining of KOH-digested digits. Three patterns of response were seen to correspond to three general levels of amputation. Complete bone regeneration occurred frequently by five weeks following amputation through the distal one-quarter of the distal phalanx. Amputation thro

Pitfalls in automatic limb lengthening - First results with an intramedullary lengthening device.

BACKGROUND: The treatment of leg length discrepancy and deformities has become more common over the last few decades due to newly developed implants. Lengthening using fully implantable intramedullary nails provides many advantages; however, only little data is available. Therefore, we aimed to determine: (1) safety of the implant, (2) the complication rate and (3) functional outcome after magnetic driven intramedullary bone lengthening with a telescopic implant. HYPOTHESES: Automatic bone lengthening with intramedullary nails provide good short-term outcome. PATIENTS AND METHODS: Ten patien

[Nail recession emergency treatment: a new technique for the repair of fingertip amputations].

INTRODUCTION: In spite of the recent advances made in microsurgery, the reconstruction of oblique fingertip amputations remains problematical. In cases where reimplantation is technically impossible, the surgeon can utilize a number of different flaps to preserve digital length. METHODS: In certain cases, instead of local flap repair using an advancement flap, the nail complex can be recessed so that after bone shortening of the distal phalanx the free edge of the nail can be stitched to the skin without resulting tension. Two longitudinal incisions are made, and a flap including the nail com

Salvage of congenital pseudarthrosis of the tibia by the induced membrane technique followed by a motorised lengthening nail.

An 18-year-old male with neurofibromatosis type I was treated for congenital pseudarthrosis of the tibia using the Masquelet induced-membrane technique with internal fixation by retrograde implantation of a transplantar intramedullary nail. Bone healing was obtained at the expense of malunion with external rotation and 5.5cm of lower limb shortening. A motorised intramedullary-lengthening nail (Fitbone®, Wittenstein, Igersheim, Germany) was implanted. This treatment was successful in correcting the rotational malalignment and limb length discrepancy. The motorised nail Fitbone® may be a valid

Systemic Amyloid Light-Chain (AL) Amyloidosis Revealed by Progressive Pan-Ungual Dystrophy: A Case Report.

Systemic amyloid light-chain (AL) amyloidosis is a multisystem disorder caused by extracellular deposition of monoclonal immunoglobulin light chains. Cutaneous manifestations are diverse, but nail involvement is uncommon and may lead to diagnostic delay. We report the case of a 53-year-old man presenting with progressive dystrophy affecting all 20 nails for 18 months. Clinical examination revealed marked nail plate atrophy and onychorrhexis. Dermoscopy demonstrated nail plate atrophy, subungual hyperkeratosis, and splinter hemorrhages. A longitudinal lateral nail-bed biopsy revealed abundant a

The Fixion nailing system for stabilising diaphyseal fractures of the humerus: a two-year clinical experience.

Previous studies have shown that the expandable nail system (Fixion) can provide rapid stabilisation of long bone fractures with reduced operative time and low complication rates. Patients with humeral shaft fracture were treated consecutively over a two-year period in our institution with the Fixion nail. Nineteen Fixion nailings were performed in 16 patients over a 2 year period. All fractures were diaphyseal and closed. Eight primary fracture stabilisations were performed and we recorded 2 nonunions in this group, both associated with rotational instability at the fracture site. Six nailing

[Metachondromatosis. Report of a family with 4 cases].

Metachondromatosis is a hereditary dominant condition, which was differentiated from multiple hereditary cartilaginous exostoses by Maroteaux in 1971. We observed a family with 4 patients in 3 generations. The condition manifested in early childhood, showed a spontaneous regression after cessation of growth, and could be diagnosed with certainty by radiological signs. We found multiple tiny periarticular calcifications, small exostoses especially on the short tubular bones, and enchondromatous bone changes. The vertebral column takes part in the bone dysplasia. In addition, our patients show d

Treatment of Malrotation and Limb Length Discrepancy in Osteogenesis Imperfecta Patients: Report of Two Cases.

Malunion of recurrent fractures in Osteogenesis Imperfecta (OI) patients causes limb length discrepancy and malrotation. These cause added difficulty for OI patients to ambulate. Lengthening with distraction osteogenesis using an external fixator in OI patients is challenging. Acute lengthening with autologous bone graft is a known method in a normal bone but not a known procedure in OI patients. We present two clinic cases of adolescent OI patients with limb length discrepancy and externally rotated lower limb that underwent acute lengthening and rotational correction using a locked intramedu
