Biomechanical design using in-vitro finite element modeling of distal femur fracture plates made from semi-rigid materials versus traditional metals for post-operative toe-touch weight-bearing.

This proof-of-concept study designs distal femur fracture plates from semi-rigid materials vs. traditional metals for toe-touch weight-bearing recommended to patients immediately after surgery. The two-fold goal was to (a) reduce stress shielding (SS) by increasing cortical bone stress thereby reducing the risk of bone absorption and plate loosening, and (b) reduce delayed healing (DH) via early callus formation by optimizing axial interfragmentary motion (AIM). Finite element analysis was used to design semi-rigid plates whose elastic moduli E ensured plates permitted AIM of 0.2 - 1 mm for ea

Histological evaluation of three-dimensional rectangular grid plates in treatment of mandibular fractures: an experimental study.

AIM OF THE WORK: Evaluate the efficacy of 3D rectangular grid plates in treatment of mandibular fractures in comparison to the more commonly used locking plates. SUBJECTS AND METHODS: This experimental study was conducted on sixteen mature male Mongrel dogs (ranged from 1-2 years old), weighting about 15-20 kg each. A linear fracture was done using surgical sagittal saw with aid of mallet and chisel in the right side of the mandible in the premolar molar area for each animal of the study under general anesthesia. The animals were divided into two groups: Group A was fixated with 3D rectangula

[Biomechanical studies on plate prebending and pretensioning in compression osteosynthesis (author's transl)].

By tensioning plates which are contoured exactly to the bone shaft a torque is produced leading to gapping of the cortex opposite to the plate. The gapping depends on the amount of tension force and plate stiffness. In osteosyntheses performed on transversely osteotomized tibial and femoral shafts with plates adapted to the bone surface the gapping angles of tibia osteotomies with narrow plates are twice as large as those of femur osteotomies with broad plates. In order to eliminate the torque the plates must be prebent before application. Three phases have been defined to describe the tension

Effect of plate orientation on apparent thickness of mineral plates by transmission electron microscopy.

PURPOSE: Transmission electron microscopy (TEM) is widely used to study the ultrastructure of bone. The mineral of bone occurs as polycrystalline mineral plates about 3 to 6 nm in thickness. A problem in using TEM to make quantitative analyses of bone is that the orientation of the plates with respect to the plane of the section being imaged is expected to affect their apparent thickness. The purpose of this study was to test if this was true, if the apparent thickness of plates changed substantially as a result of tilt of the section. METHODS: We prepared TEM sections of samples of cortical

Clinical outcomes of absorbable plates (hydroxyapatite-poly-l-lactide composites) for phalangeal fractures - case reports.

We report two cases of the basal phalanx fractures of the thumb treated with absorbable mesh plates. In both cases, the mesh plates specifically tailored for each fracture were effective in obtaining bone union and healing. We conclude that absorbable mesh plates could be a practical option for phalangeal fractures, especially where proprietary pre-molded metallic plates do not neatly fit the reduced fracture area.

Biomechanical Behavior of Composite Bone-Osteosynthesis Constructs in Complex Proximal Humerus Fractures: A Synergistic Experimental and Finite Element Approach.

This study evaluates the mechanical behavior of bone-implant assemblies used in treating complex proximal humerus fractures, a clinical challenge due to the anisotropic nature of bone and variability in patient-specific conditions. The aim of this study was to compare the stability and stress distribution of three fixation methods: polyaxial locking plates, monoaxial locking plates, and intramedullary nails. Using 4th-generation composite humerus models, a four-part fracture (Neer IV) was simulated. The assemblies underwent axial compression testing using a universal testing machine, complemen

A New Quantitative Index of Trabecular Bone Microstructural Organization and Its Association With Tissue Failure in Human Vertebrae.

Trabecular bone is a lightweight porous tissue with critical load-bearing function that is optimized through load-driven structural remodeling. One critical feature of trabecular bone microstructure at the level of whole trabeculae is the predominance of plate-like and rod-like forms with distinct orientations, material properties, and mechanical roles. Trabecular plates primarily align in the direction of typical loads and dominate structural stiffness under such loads. Thinner, less dense trabecular rods primarily align transverse to typical loads, contribute little to structural stiffness,

Biocompatibility and Efficiency of Biodegradable Magnesium-Based Plates and Screws in the Facial Fracture Model of Beagles.

PURPOSE: A biodegradable magnesium alloy system has been developed as a substitute for conventional plates and screws made of titanium or absorbable polymer. However, previous studies were limited to small animal experiments using screws or wires. In the present study, we preliminarily evaluated the biocompatibility and effectiveness of human standard-size biodegradable magnesium-based plates and screws in facial fractures of beagles. MATERIALS AND METHODS: Fracture lines were created bilaterally in the zygomatic arches of 6 beagles. They were fixed in situ with plates and screws made of magn

Radiation dose inaccuracies near patient-specific osteosynthesis plates: an ex vivo dosimetric study.

BACKGROUND AND PURPOSE: Osteoradionecrosis (ORN) inside osseous free flaps and native bone is a severe complication in multimodal therapies for head and neck cancer. The tissue proximity of titanium osteosynthesis plates may influence local radiation dose and planning accuracy, yet data from tissue-equivalent, image-guided models are limited. MATERIALS AND METHODS: An ex vivo dosimetric study was performed using two porcine tibiae, each plated with either one computer aided design and manufacturing (CAD/CAM) patient-specific 2.0 mm reconstruction plate or two 1.0 mm mini plates, thus reproduc

Comparison of the size of plates for fracture fixation with the size of phalanges and metacarpals in cadavers of Asian origin.

The size and volume of plates and screws for fracture fixation of the hand (1.5-mm screws and titanium miniplates, 2.0-mm screws and stainless-steel AO miniplates, and 2.7-mm screws and stainless-steel AO miniplates) were compared against the phalanges and metacarpal bones and the surrounding soft tissue from male cadavers of Asian decent. In the cadaver study, it was first established that the difference between anatomic measurements and radiologic measurements for the interarticular bone length and midshaft width were not significant (p = .09). Second, the volume occupied by the bone showed

Similar Clinical Outcome in Locking and Conventional Plate Osteosynthesis for the Treatment of AO 44-B2 Ankle Fractures.

IntroductionBiomechanical studies have proved that locking plates have better primary stability besides versatility regarding fracture pattern while reducing bone contact and bridging the gap, whereas conventional nonlocking plates (plus lag screw) depend on bone-plate compression. The clinical benefit of locking plates over nonlocking plates remains unanswered, however. Therefore, this retrospective cohort study was set up to test the hypothesis that the use of locking plates for unstable ankle fractures will result in fewer re-displacements, superior bony healing, and functional and clinical

Does Far Cortical Locking Improve Fracture Healing in Distal Femur Fractures: A Randomised, Controlled, Prospective Multicentre Study.

(1) Background: Bone healing is influenced by various mechanical factors, such as stability, interfragmentary motion, strain rate, and direction of loading. Far cortical locking (FCL) is a novel screw design that promotes bone healing through controlled fracture motion. (2) Methods: This study compared the outcome of distal femur fractures treated with FCL or SL (standard locking) screws and an NCB plate in a randomised controlled prospective multicentre trial. The radiographic union scale (RUST) and healing time was used to quantify bone healing on follow-up imaging. (3) Results: The study in

Biomechanical considerations in the design of patient-specific fixation plates for the distal radius.

Use of patient-specific fixation plates is promising in corrective osteotomy of the distal radius. So far, custom plates were mostly shaped to closely fit onto the bone surface and ensure accurate positioning of bone segments, however, without considering the biomechanical needs for bone healing. In this study, we investigated how custom plates can be optimized to stimulate callus formation under daily loading conditions. We calculated implant stress distributions, axial screw forces, and interfragmentary strains via finite element analysis (FEA) and compared these parameters for a corrective

Early temporary porosis of bone induced by internal fixation implants. A reaction to necrosis, not to stress protection?

Stabilization of the fracture using implants requires contact surfaces between implant and bone. Such contact has been observed to induce bone porosis first seen at one month after surgery. Bone loss in the vicinity of implants has hitherto been explained as being induced by mechanical unloading of the bone (stress protection). Experiments in sheep, dogs, and rabbits combining intravital staining of blood circulation and polychrome fluorescent labeling of bone remodeling leads to the conclusion that early bone porosis in the vicinity of the implants is the result of internal remodeling of cort

Palladium(II) Metal Complex Fabricated Titanium Implant Mitigates Dual-Species Biofilms in Artificial Synovial Fluid.

Metallodrugs have a potent application in various medical fields. In the current study, we used a novel Palladium(II) thiazolinyl picolinamide complex that was directly fabricated over the titanium implant to examine its potency in inhibiting dual-species biofilms and exopolysaccharides. Additionally, inhibition of mono- and dual-species biofilms by coated titanium plates in an in vitro joint microcosm was performed. The study was carried out for 7 days by cultivating mono- and dual-species biofilms on titanium plates placed in both growth media and artificial synovial fluid (ASF). By qPCR ana

Single-cell transcriptomic analyses of mouse idh1 mutant growth plate chondrocytes reveal distinct cell populations responsible for longitudinal growth and enchondroma formation.

Enchondromas are a common tumor in bone that can occur as multiple lesions in enchondromatosis, which is associated with deformity of the effected bone. These lesions harbor mutations in IDH and driving expression of a mutant Idh1 in Col2 expressing cells in mice causes an enchondromatosis phenotype. In this study we compared growth plates from E18.5 mice expressing a mutant Idh1 with control littermates using single cell RNA sequencing. Data from Col2 expressing cells were analyzed using UMAP and RNA pseudo-time analyses. A unique cluster of cells was identified in the mutant growth plates th
