Finite element analysis of various fixation patterns in mandible bone fracture.

Mandibular fractures are among the most serious injuries to the maxillofacial region, with controversy surrounding their diagnosis, classification, and management Fixation mini-plate systems are commonly used for treatment of mandibular fractures. Previous studies have examined plate configurations with various locking systems. However, the impact of plate thickness on the performance of unlocking screw mechanisms has not been investigated in the literature. Also, there exist conflicting findings concerning the impact of the locking and unlocking screw mechanisms on the concentration of stress

Biomechanical performance evaluation of S2AI combine with LC-2 screw for day II pelvic crescent fracture dislocation via finite element analysis.

Plate fixation is a classic method for treating day II crescent fracture dislocation of the pelvic (CFDP). However, due to the advantages of minimally invasive techniques and reduced complications associated with internal fixation percutaneous cannulated screws have emerged as a promising alternative for treating Day II CFDP. In this study, we propose using an S2AI screw combined with an LC-2 screw (S2AI + LC-2) for the treatment of Day II CFDP. The aim of this study was to compare its biomechanical stability with that of two conventional fixation methods using finite element analysis (FEA). A

Lag screw fixation of mandibular fractures.

The lag screw is the simplest means of rigid internal fixation. By virtue of its ability to compress bone fragments without the use of a bone plate, it has wide application and achieves great stability. This article presents techniques to apply lag screws to fractures of the mandibular symphysis, the mandibular body, and the mandibular angle. Results are reviewed in studies of 315 patients whom the author has treated with lag screw fixation. The advantages of the lag screw technique over bone-plate fixation, such as a more rapid application, obviation of the need to adapt a bone plate, minimal

Biomechanical Evaluation of a Newly Developed Functional-Grade Composite Material for Pedicle Screws.

OBJECTIVE: Pedicle screw and rod systems are widely employed in spine surgeries and loosening due to insufficient mechanical stimulation on the bone is frequently encountered in pedicle screws. This mechanical stimulation problem also arises due to the high rigidity of the implant material. This study aimed to develop new pedicle screws with composite material to solve the pedicle screw loosening problem. METHODS: The vertebrae and vertebral disk were modeled in three dimension using computerized tomography images obtained from a patient. A commercially available pedicle screw was modeled usi

Effects of Acetabular Screws on the Initial Stability of Porous Coated Acetabular Implants in Revision Total Hip Arthroplasty.

INTRODUCTION: Revision total hip arthroplasty in the setting of acetabular bone loss remains a challenging clinical entity. Deficiencies of the acetabular rim, walls, and/or columns may limit the bony surface area and initial acetabular construct stability necessary for osseointegration of cementless components. Press-fit acetabular components with supplemental acetabular screw fixation represent a common technique aimed to minimize implant micromotion and allow for definitive osseointegration. Although acetabular screw fixation is commonly practiced in revision hip arthroplasty, few studies t

Screw Versus Plate Fixation for Chevron Osteotomy: A Retrospective Study.

The chevron osteotomy is a popular procedure used for the correction of moderate hallux abducto valgus deformity. Fixation is typically accomplished with Kirschner wires or bone screws; however, in cystic or osteoporotic bone, these could be inadequate, resulting in displacement of the capital fragment. We propose using a locking plate and interfragmental screw for fixation of the chevron osteotomy that could reduce the healing time and decrease the incidence of displacement. We performed a retrospective cohort study for chevron osteotomies on 75 feet (73 patients). The control groups underwen…

PMMA-augmentation of incompletely cannulated pedicle screws: a cadaver study to determine the benefits in the osteoporotic spine.

BACKGROUND: Pedicle screw pullout due to poor bone quality, mainly caused by osteoporosis, is a common problem in spine surgery. Special implants and techniques, especially PMMA augmentation, were developed to improve the fixation of pedicle screws. PMMA injection into a pilot hole or through a screw involves the same risks as vertebroplasty or kyphoplasty, regardless of the technique used. Especially when using fully cannulated screws anterior leakage is possible. OBJECTIVE: To prove PMMA injection is safe and possible without leakage through an incompletely cannulated screw and also increas

Comparison of pedicle screw loosening under uniaxial and multiaxial loading.

BACKGROUND: Pedicle screw loosening is a clinically-relevant failure mode following spinal fusion procedures, resulting from damage and compaction to the surrounding bone with loading of the bone-implant interface. Current in vitro tests for investigating pedicle screw loosening commonly apply uniaxial loading (e.g. screw pullout or toggle testing). However, these methods do not replicate multiaxial spinal loading associated with activities of daily living. The goals of this study were to (1) develop and assess a novel test method for inducing pedicle screw loosening under multiaxial loading c

Intraarticular migration of a broken biodegradable interference screw after anterior cruciate ligament reconstruction.

Poly-L: -lactic acid biodegradable screws have been used effectively for graft fixation in anterior cruciate ligament (ACL) reconstruction. The overall complication rate associated with the use of this implant is low, although some authors reported complications, such as osteolysis and aseptic effusion of the knee joint. We report a case of a 29-year-old female patient with a failure of a biodegradable interference screw at 22 months after ACL reconstruction using bone-patellar tendon-bone graft. In this illustrated case, the screw broke and migrated into the knee joint. In addition, we perfor

Measuring temperature rise during orthopaedic surgical procedures.

A reliable means for measuring temperatures generated during surgical procedures is needed to recommend best practices for inserting fixation devices and minimizing the risk of osteonecrosis. Twenty four screw tests for three surgical procedures were conducted using the four thermocouples in the bone and one thermocouple in the screw. The maximum temperature rise recorded from the thermocouple in the screw (92.7±8.9°C, 158.7±20.9°C, 204.4±35.2°C) was consistently higher than the average temperature rise recorded in the bone (31.8±9.3°C, 44.9±12.4°C, 77.3±12.7°C). The same overall trend between

Saw transection of retained compression screws with damaged heads instead of removal in revision first metatarsal osteotomy: a technique tip.

Distal first metatarsal osteotomy is performed to correct hallux valgus. The surgery is being performed throughout the UK in increasing numbers. The osteotomies used are commonly fixed with one or two variable pitch compression screws. Recurrence of the deformity or inadequate correction in the primary surgery may require revision osteotomy which necessitates removal of previous screws. Revision rates for scarf osteotomy have been reported at 5%. Removal of screws can be challenging as they are often buried in bone and/or the screw heads can get damaged even despite meticulous preparation. Var

Performance parametric formulation of carbon fiber-reinforced composite locking bone implant plates based on finite-element analysis.

The treatment of Giant Cell Tumor (GCT) in the distal radius poses challenges due to the intricate anatomical features of the bone. It often necessitates the use of long implant plates or the interconnection of multiple short plates after tumor excision. However, the deployment of metal plates may increase the risk of screw loosening and various complications. To address these challenges, this study proposes the adoption of carbon fiber-reinforced PEEK (CFRP) as the base material. As a unique strategy, performance parameters (PP) were developed to compare CFRP implant plates with a Ti-6Al-4V p

[The value of miniature compression screws in maxillofacial surgery].

According to the well-known principle of compression, a new kind of screw has been developed. It is a development of the miniaturized Michelet-Champy osteosynthesis device which we have used for the last 10 years in maxillofacial surgery. These traction-screws were initially elaborated for rigid stabilization after sagittal ramus osteotomies such as Obwegeser-Dal Pont. Usually, three screws are inserted at each side, via a transjugal approach. It is then possible to remove the intermaxillary fixation device at the end of the surgical procedure. However, these screws are also useful in other ca

Effects of fixation technique on displacement incompatibilities at the bone-implant interface in cementless total knee replacement in a canine model.

Bone-implant displacements can be caused by rigid body motion and by differences in material properties of the implant and bone. In the present study of the tibial component in total knee replacement, we tested a series of tibial component fixation designs to determine how certain design features influenced the magnitude of the tangential displacement between the component and supporting bone in a canine model. The transverse expansion of the proximal tibia under static axial loading was measured in the intact tibia and then in the same bone following implantation of tibial components with dif

Tenting screw-assisted membrane alveolar ridge augmentation in the anterior mandible.

BACKGROUND: Vertical ridge augmentation with a guided bone regeneration procedure in the anterior mandible requires membrane lingual stabilization. Until now, the only predictable way was the use of titanium-reinforced dense polytetrafluoroethylene (TR-dPTFE) membranes stabilized with self-drilling screws, which require large flaps for both placement and removal. This case report introduces a technique that does not require any membrane lingual fixation. METHODS: A tenting screw was placed in the middle of an anterior mandibular defect. A small TR-dPTFE membrane was bent 90° to take an L shap
