New methods of treatment have greatly improved the success of limb salvage in severe lower extremity trauma. We report the use of a microvascular free tissue transfer and distraction osteogenesis in the reconstruction of soft tissue and bone defects complicated by infection. Soft tissue coverage and bony stabilization by external fixation resulted in eradication of the infection. The external fixator also allowed bone transport and regeneration to overcome a bone defect of approximately 6 cm. Bone transportation can be accomplished in a distal to proximal direction if the situation necessitate

Complex lower extremity trauma reconstruction remains a challenge. This study used an internal fixation composite structure of antibiotic cement plates as a temporary fixation to treat lower extremity Grade III open fractures; thus, reducing the treatment period and complications of external fixation. We aimed to assess the safety and efficacy of this technique in the initial surgery stage. Between January 2018 and March 2021, 20 patients with Gustilo grade IIIB/C open fractures received an antibiotic cement-coated locking plate as a temporary internal fixator during initial surgery. The range of applications of digital surface models of the bones in science and industry is wide. Three-dimensional reconstructions of bones are used in biomechanics, biomedical engineering, medical image processing, orthopedics, traumatology, radiology, patient education, anatomy, anthropometry, forensic anthropology, ergonomics, usability and human factors engineering, or accident and injury analysis and prevention. No open access database or repository of skeletal surface models of the full lower extremities exists. Therefore, the objective of this publication was to provide access to consi

Local rotation muscle flaps are useful for soft tissue reconstruction in open grade III fractures of the lower extremity. Gastrocnemius or soleus muscle flaps provide early bone coverage and avoid a more demanding free tissue transfer. Deep vein thrombosis is common in injured limbs and venous hypertension may result in acute muscle flap necrosis. Loss of bone coverage and an increased risk of osteomyelitis follow. We present a successful soleus rotation flap for tibial coverage in a lower extremity with a documented major venous thrombosis.

Four cases of rheumatoid arthritis with a total of 12 undisplaced stress fractures of the lower extremities are described. Pain, swelling and disability arising from these fractures were initially mistaken for rheumatoid synovitis. Three patients had osteoporosis, and one had osteomalacia. A high index of suspicion is necessary in the diagnosis of these fractures. Radiographs and, if necessary, bone scans are indicated in rheumatoid patients presenting with spontaneous pain in the lower extremities in association with tenderness localized over the bone rather than the joint, particularly in th

CONTEXT: Osteogenesis imperfecta (OI) type I is a heritable bone fragility disorder that is caused by mutations affecting collagen type I. We recently showed that patients with OI type I frequently have muscle weakness. As muscle force and bone mass are usually closely related, we hypothesized that muscle weakness in OI type I could contribute to increase bone mass deficit in the lower extremities.
OBJECTIVE: To assess the muscle-bone relationship in the lower extremities of children and adolescents with OI type I.
SETTING: The study was carried out in the outpatients department of a pediatr

The length of the lower extremities was equalized by the bone distraction method in four patients after replantation of lower legs. A Volkov-Oganesian or Ilizarov distraction apparatus was applied on the replanted lower leg after corticotomy of the tibia and osteotomy of the fibula, and distraction was carried out over 6 to 11 months in small, measured doses. The lower extremities were lengthened from 4 to 8 cm. The lengths of the lower extremities were fully equalized in two patients; they were partially equalized in the other two patients. This made it possible to achieve successful rehabili

Osteochondromas, which are benign bone tumors that usually develop on long bones, tubular bones, are rarely found in the spine. If they are located in the spinal canal, they may cause nerve root or spinal cord compression, which is a rare but potentially catastrophic manifestation of osteochondromas. In this article, we report a case of a 38-year-old man who presented with low back pain, paresthesia, and weakness of the right lower extremity aggravating gradually for 5 months. No family history of this disease can be traced. The L4-L5 level computed tomography scan showed an abnormal bony prot

Postoperative pneumonia, a prevalent complication arising from lower limb fracture surgery, can significantly prolong hospitalization periods and elevate mortality rates. Consequently, early prevention and identification of this condition are crucial in improving patient prognosis. In this study, clinical indicators pertaining to postoperative pneumonia in patients with lower limb fractures at Nantong University Hospital, spanning the years 2016 to 2023, were subjected to a analysis. The patients who encountered postoperative pneumonia subsequent to their lower limb fracture surgeries during h

BACKGROUND: The quality of model development and its applicability are still unknown, even though numerous studies have developed or validated predictive models to estimate the risk of perioperative deep venous thrombosis (DVT) in lower limb fractures. The aim of the study was to systematically assess the DVT risk prediction model in lower limb fracture patients in the perioperative phase. METHODS: PubMed, EMBASE, Web of Science, and China National Knowledge Infrastructure (CNKI) were assessed regarding studies on risk prediction models for perioperative DVT in limb fractures. The search perio

BACKGROUND: The association between ultrasound-assisted combined with conventional anatomical landmark paramedian spinal anesthesia regarding the first-pass success rate remains contentious. This study aims to clarify this relationship.
METHODS: In this retrospective cohort analysis of 146 patients with lower limb fractures, patients were divided into two groups based on their spinal anesthesia technique:ultrasound-assisted combined with conventional anatomical landmark (median or paramedian). The primary endpoint was the first-pass success rate, while secondary endpoints included total proce

McCune-Albright syndrome (MAS) is defined by a triad of fibrous dysplasia (FD), hyperfunctioning endocrinopathies, and café-au-lait spots. FD, characterized by the fibrous replacement of healthy bone, is a major source of morbidity in MAS. To characterize skeletal involvement and associated endocrinopathies in patients with MAS/FD, we conducted a retrospective review of patients with the diagnosis of both MAS and FD treated at our center between 1989 and 2024. Forty-three patients (29 female; median diagnosis age: 13.1 years) were included, with 41 presenting with polyostotic FD. Among these,

BACKGROUND: Lower-extremity fractures (LEFs) account for >30% of all skeletal injuries, contributing to the global health and economic burden. Fracture epidemiology in the Middle East and North Africa (MENA) region has been studied little. Health factors and disease epidemiology differ greatly among populations in MENA despite cultural, political, and economic similarities among the region's countries. This study examined the epidemiology of LEFs and the need for rehabilitation in MENA from 1990 to 2019.
METHODS: We examined the epidemiology of fractures of the pelvis, hip, femur, patella, ti

BACKGROUND Unicameral bone cysts (UBCs) are benign tumor-like lesions that are the most common cause of pathological proximal femur fracture in children. This study aimed to present the outcomes of acute, unstable, pathological proximal femur fractures secondary to UBCs in children. MATERIAL AND METHODS We retrospectively reviewed data on 12 patients with a mean age of 9.3 years (7-12 years) who were initially treated with decompression and grafting, followed by stabilization using a 120° fixed-angle low-contact locking pediatric plate (LCLPP). The Musculoskeletal Tumor Society (MSTS) scores,
