BACKGROUND: Corrective osteotomies of the upper extremities with patient-specific instruments (PSIs) are increasingly used. In this context, the concordance between planning and postoperative 3D radiographs as well as the association between 3D accuracy and clinical outcome has rarely been evaluated. Several congenital upper extremity anomalies may be treated using micro-surgical techniques. Long-term studies have shown the usefulness of microvascular toe transfer in the treatment of adactyly with the incorporation of the transferred digit into grasp-and-pinch function. The use of free fibular transfer for long bone deformities of the forearm has been shown to provide bony union as well as growth. Factors that must be considered include patient age, vessel availability, and lack of other possible reconstructive options.

Distal radius fracture (DRF) is the most common upper extremity fracture in the elderly. Patients with a DRF have a two to fourfold higher risk of a subsequent fracture than those with no history of fractures, and DRFs occur on average 15 years earlier than hip fractures. Therefore, patients with a DRF offer physicians an important opportunity to diagnose and treat osteoporosis to prevent a secondary fracture. In this review, we provide recent update in the diagnosis and treatment of bone frailty in patients with a DRF.

Extramedullary plasmacytomas are plasma cell tumors that occur outside the bone marrow. They constitute around 4 % of all plasma cell neoplasms. The most common site of extramedullary plasmacytoma is the upper aerodigestive tract-nasal cavity, paranasal sinuses and oronasopharynx. We are presenting a case of extramedullary plasmacytoma of the trachea. Trachea is an extremely rare site of plasmacytoma. When extraosseous plasmacytoma occur in uncommon sites, the distinction from B cell lymphomas showing extensive plasmacytic differentiation can be difficult and diagnostically challenging.

A 4-month-old male infant presented with recurrent cough for 2 months. He had a shortened right upper limb with absent right thumb and continuous murmur in the left parasternal area. The X-ray showed an absent radius and the first metacarpal and phalyngeal bones on the right side. Echocardiogram revealed aortopulmonary window and small secundum atrial septal defect. Aortopulmonary window was successfully treated by device closure. Holt-Oram syndrome with aortopulmonary window is an extremely rare association.

Alveolar soft part sarcoma (ASPS) is a rare subtype of soft tissue sarcoma that occurs predominantly in young patients. Despite its relatively indolent course, it generally has a poor prognosis with widespread metastases. The common metastatic sites from an ASPS include the lung, brain and bone. However, metastasis of an ASPS to the gastrointestinal tract is extremely rare. Here, we report a rare case of upper gastrointestinal bleeding and jejunal intussusception due to gastrointestinal metastases from an ASPS.

The fractures by plastic deformation in the forearm of children are not common and they are produced by an applied forces to the upper extremity. The produced stress over the bone structure is near to the plastic limit. If the deformity is angulated more than 20 degrees, the correction must be done surgically or by closed means; if it is less than 20 degrees and the patient is younger than five years old, the physiological remodelling should be permitted. The presented patient was injured while repairing a car. When the motor was running a metal blade of the fan broke off and struck him on the right side of his forehead. The blade sticking in the bone was removed surgically, and after 17 days he was discharged home without neurological deficit. After 60 days paresis of the left upper extremity and rhinorrhoea developed, and the patient had a second operation. Four months after the injury the first seizure appeared, provoked by discontinuation of anticonvulsants.

Forearm rotation is crucial for full upper extremity mobility. The two-bone unit with its proximal and distal radioulnar joints should be considered as a single bicondylar articulation. After a traumatic bony forearm injury, surgical treatment for complications, such as deformity, bone loss, or failed fracture healing is challenging because complete return of forearm rotation can be difficult to achieve. It is important to be aware of methods for assessing and managing posttraumatic forearm bony complications, including preoperative assessment and osteotomy techniques for malunited fractures.
