Radiographic Follow-up After Normal Ultrasound Screening of the Hip in Breech Infants.

BACKGROUND: Breech presentation is one of the most important risk factors for developmental dysplasia of the hip, and all breech infants should be screened. The necessity of further follow-up of developmental dysplasia of the hip after normal clinical and sonographic screening is a controversial subject. The purpose of this study to identify the incidence of delayed dysplasia in breech infants after normal ultrasound screening and the necessity of further clinical and radiologic follow-up in these patients. METHODS: We included the 292 breech babies (128 boys and 164 girls) who showed normal …

Applying the Robson classification to routine facility data to understand the Caesarean section practice in conflict settings of South Kivu, eastern DR Congo.

INTRODUCTION: Sub-Saharan Africa has low Caesarean (CS) levels, despite a global increase in CS use. In conflict settings, the pattern of CS use is unclear because of scanty data. We aimed to examine the opportunity of using routine facility data to describe the CS use in conflict settings. METHODS: We conducted a facility-based cross-sectional study in 8 health zones (HZ) of South Kivu province in eastern DR Congo. We reviewed patient hospital records, maternity registers and operative protocol books, from January to December 2018. Data on direct conflict fatalities were obtained from the Up

Umbilical cord prolapse.

Umbilical cord prolapse complicated one of 385 pregnancies occurring at the University of Colorado Health Science Center between 1969 and 1982. Whereas malpresentation of the fetus is frequently associated with prolapse of the funis, nearly 50% of all cases occurred in vertex presentations. Obstetric intervention is associated with nearly one in five cases of cord prolapse and represents a readily preventable cause of maternal and perinatal morbidity.

Preterm breech delivery. Early and late complications.

One hundred and ten singleton breech infants with a birth-weight less than 2 500 g have been delivered at Huddinge University Hospital during the years 1972-78. The cesarean section rate was 30.1 per cent. Vaginal and abdominal deliveries were compared with regard to perinatal mortality and asphyxial morbidity at birth. All infants born with reduced Apgar scores or traumatic birth injuries have been followed up. During a 6-year period 3 children born with low Apgar scores were found to have persistent sequelae. Two of these were considered to relate to the delivery. We conclude that cesarean s

DESIGN: Prospective cohort s … an, after adjustment for possible confounding variables and gestational age (0.7, 0.4 to 1.0). With breech presentation, caesarean delivery had a large protective effect for fetal death. With cephalic presentation, however, independent of possible confounding variables and gestational age, intrapartum and elective caesarean increased the risk for a stay of seven or m …

Bilateral clavicle fracture in two newborn infants.

BACKGROUND: The fracture of clavicle is the most frequently observed bone fracture as birth trauma and it is usually unilateral. It is seen following shoulder dystocia deliveries or breech presentation of macrosomic newborns. CASE PRESENTATION: We report two macrosomic newborns with bilateral clavicle fracture and brachial plexus palsy due to birth trauma. Chest X-rays confirmed bilateral fracture of clavicles. Both patients were recovered without any sequel. CONCLUSION: Bilateral clavicular fracture should be considered in any neonate with bilateral absent Moro reflexes.

Reducing medical costs by adopting external cephalic version in a single perinatal center in Japan.

AIM: The rising rate of cesarean is a global public health concern. In Japan, the rate rose to 29.3% in 2023. Non-cephalic fetuses are typically delivered via cesarean; however, the external cephalic version (ECV) has the potential to reduce perinatal complications and healthcare costs. This study aimed to evaluate the medical cost savings and effectiveness of ECV. METHODS: This retrospective cohort study was conducted at a single perinatal center in 2023. ECV was performed under combined spinal-epidural anesthesia (CSEA). The primary endpoint was cost savings from ECV from a societal perspec

Mode of delivery of twin pregnancies with the first twin in breech position after the introduction of a policy of planned caesarean delivery for nulliparous women.

INTRODUCTION: To evaluate the impact of the mode of delivery of twin pregnancies with the first twin in breech position for all parities combined after introduction of a policy of planned caesarean section in 38 weeks' gestation in nulliparas. MATERIAL AND METHODS: A retrospective study of the mode of delivery of twin pregnancies with the first twin in breech position was conducted from January 2007 to December 2015 after the implementation of a planned caesarean section in 38 weeks' gestation in nulliparas. Maternal and neonatal outcomes were compared according to the decision of attempted v

Contraindications for external cephalic version in breech position at term: a systematic review.

OBJECTIVE: External cephalic version (ECV) is a safe and effective intervention that can prevent breech delivery, thus reducing the need for cesarean delivery. It is recommended in national guidelines. These guidelines also mention contraindications for ECV, and thereby restrict the application of ECV. We assessed whether the formulation of these contraindications in guidelines are based on empiric data. DESIGN: Systematic review. POPULATION: Pregnant women with a singleton breech presentation from 34 weeks. METHODS: We searched the National Guideline Clearinghouse, the Cochrane Central Reg…

Adverse perinatal events and subsequent cesarean rate.

OBJECTIVE: To determine whether severe intrapartum complications resulting in poor neonatal outcome increased obstetricians' cesarean delivery rates. METHODS: From July 1996 through June 1998 we prospectively studied 3008 deliveries by 12 obstetricians. We chose adverse neonatal outcomes that would be viewed by obstetricians as anxiety-provoking experiences that are rare in obstetric practice. Index events included head entrapment of breech infants, Apgar score less than 3 at 10 minutes, shoulder dystocia resulting in persistent brachial plexus injury, and intrapartum fetal death. After an in

[Hydronephrosis in pregnancy].

This is a report on the clinical course of a severe right-sided urinary retention in 21-year old patient, treated from the 24th week of pregnancy onwards. The special importance of the mechanical component involved in the genesis of this retention of urine is discussed on the occasion of the transitory spontaneous remission, observed, when the foetus turned to breech presentation.

Rupture of pregnant noncommunicating uterine horn with fetal salvage.

A case is presented of pregnancy in an uncommunicating rudimentary uterine horn. The pregnancy proceeded to 31 gestational weeks, when the rudimentary horn ruptured. At laparotomy the surviving fetus was lying free in the peritoneal cavity, was delivered breech and subsequently did well. This is the fifth case reported of rupture of a pregnant rudimentary horn with neonatal and maternal survival.

Study of vaginal delivery in patients with one previous lower segment caesarean section.

This retrospective study evaluated the prospects of having a vaginal delivery following a lower segment Caesarean section in Nehru Hospital; 685 (52%) of 1,315 patients with a previous Caesarean section were given a trial of vaginal delivery with a success rate of 69%. Vaginal breech delivery was allowed in 14 patients without affecting the outcome of term babies and without maternal morbidity or mortality. There were 12 cases (1.7% incidence) of scar disruption of which 4 were of scar rupture. There was no maternal or fetal death as a result of these disruptions even though 3 mothers had subt

Intrauterine constraint and craniosynostosis.

Intrauterine constraint is responsible for a number of structural defects of the craniofacial and peripheral skeleton including plagiocephal … t. The etiology of the intrauterine constrain was different in each case. Factors involved included breech presentation, primagravidity, uterine malformations, amniotic bands, and defects in fetal neuromuscular development, all of which are known to produce fetal deformations. As further evidence of severe constraint, fetal activity was greatly reduced during four of …
