Cell-mediated cytotoxic response to respiratory syncytial virus in infants with bronchiolitis.

A cell-mediated cytotoxic response to respiratory syncytial (RSV) was demonstrated in the peripheral blood of 4 of 22 infants with acute bronchiolitis. These 4 infants were aged 3 weeks to 3 months. No such response was found in infants older than 4 months. All 4 infants with a positive response had mild infections.

Interrelationship of chronic eosinophilic pneumonia, bronchiolitis obliterans, and rheumatoid disease: a hypothesis.

Three patients with histologically proven bronchiolitis obliterans are presented, two of whom had rheumatoid disease. All three patients had, in addition, clinical and radiological evidence of chronic eosinophilic pneumonia; open lung biopsy in two showed coexistent features of chronic eosinophilic pneumonia and bronchiolitis obliterans. The association of both rheumatoid disease are chronic eosinophilic pneumonia with bronchiolitis obliterans in these patients may simply be coincidental, but the striking similarities between the cases suggest that a real interrelationship of these disease ent

Serum migration-inhibitory activity in infants with respiratory syncytial virus bronchiolitis.

Using an unidirectional leucocyte migration-inhibition (LMI) assay, we investigated the migration-inhibitory activity (LIF) in the sera of 25 infants who developed bronchiolitis following respiratory syncytial virus (RSV) infection. Blood samples for serum-LIF activity detection were obtained from patients the day after admission and two weeks later. The LMI assay revealed serum-LIF activity in 17 (68%) blood samples taken during the acute phase of the disease. In contrast, no inhibition was found in the convalescent blood samples with the exception of 5 (20%). This difference was statisticall

Bronchiolitis and bronchitis in connective tissue disease. A possible relationship to the use of penicillamine.

Rapid onset of severe and irreversbile airflow obstruction developed in two women. One had eosinophilic fasciitis and the other had rheumatoid arthritis. Both were treated with penicillamine. In the first patient, aged 42 years, dyspnea developed after six months of therapy. Her roentgenogram showed hyperinflation. Forced vital capacity expired in one second (FEV1/FVC%) decreased from 75% to 40%, and the residual volume increased by 1 L. In the second patient, aged 54 years, cough and dyspnea developed after ten months of therapy. The FEV1/FVC% was 56%, the FEV1 was 0.9 L, and the roentgenogra

Acute respiratory failure in bronchiolitis and pneumonia in infancy. Modes of presentation and treatment.

The modes of presentation and the management of acute respiratory failure in 11 infants with severe lower respiratory tract infections (due to respiratory syncytial virus in eight) are described. Progressive respiratory difficulties leading to exhaustion, peripheral circulatory collapse, recurrent apnoeic attacks, or generalized convulsions were the main clinical presentations resulting in severe ventilatory failure. In nine infants preventilation carbon dioxide tensions exceeded 65 mm Hg. It seems likely that the use of intermittent positive-pressure ventilation in these patients contributed

The use of dupilumab in bronchiolitis obliterans after hematopoietic stem cell transplant.

BACKGROUND: Bronchiolitis obliterans is a serious pulmonary manifestation of chronic graft-versus-host disease (cGVHD) following hematopoietic stem cell transplant (HSCT). Bronchiolitis obliterans is typically treated with a combination of inhaled steroids, azithromycin, montelukast and immunosuppressants. There are no curative treatments other than lung transplant, however, and additional therapies remain an unmet need. CASE PRESENTATION: Here we report two patients who developed bronchiolitis obliterans following HSCT. Both developed strong type 2 inflammatory markers including high eosinop

Standardizing HFNC for bronchiolitis: a quality improvement protocol in an Italian children's hospital.

UNLABELLED: Bronchiolitis is the leading cause of non-elective hospitalizations in infants, with an increasing use of high-flow nasal cannula (HFNC) therapy outside the Pediatric Intensive Care Unit (PICU). At our hospital, HFNC was used to treat pediatric bronchiolitis patients without a standardized protocol for initiation, flow titration, escalation, or a PICU alert system in cases of clinical deterioration. This quality improvement project aimed to evaluate the feasibility of a multidisciplinary protocol designed to standardize HFNC use for bronchiolitis in patients under 24 months of age,

Intravenous Terbutaline as Rescue Therapy in Severe Bronchiolitis: A Report of Two Cases.

Bronchiolitis is a lower respiratory tract infection most frequently caused by viral infections and is usually managed with supportive care. The role of bronchodilators particularly intravenous (IV) β2-agonists remain uncertain. Emerging evidence suggests that bronchiolitis comprises heterogeneous phenotypes, with a subset demonstrating bronchodilator responsiveness. We report two infants (ten and three months old) with RSV confirmed severe bronchiolitis who exhibited respiratory distress and hypoxemia despite maximal supportive therapy, including oxygen, continuous positive airway pressure (C

Noninvasive Ventilation in Pediatric Bronchiolitis: Variables Related to Weaning Readiness and Approach.

BACKGROUND: There is a lack of evidence related to weaning noninvasive ventilation in bronchiolitis. The purpose of this study was to identify variables related to NIV weaning readiness and approach in bronchiolitis. METHODS: A retrospective cohort study of 74 children less than two years of age with bronchiolitis, requiring NIV and admitted to a pediatric intensive care unit (PICU) from 2022 to 2023. Demographic variables, clinical metrics, NIV mode, and weaning metrics were collected. RESULTS: Mean total duration of NIV therapy was 40 h with weans lasting 13 h. Duration for children on CPA

Bronchiolitis obliterans complicating follicular lymphoma: a case series and clinical insights.

Tumor-associated bronchiolitis obliterans (BO) is a major cause of death and is frequently complicated by paraneoplastic pemphigoid (PNP). In this study, three cases of follicular lymphoma (FL) with BO were retrospectively analyzed. All cases were complicated by PNP. All patients achieved a tumor response with anti-CD20 monoclonal antibody-containing treatment, and two patients showed clinical improvement in PNP. However, BO continued to progress, and two of the three patients developed pulmonary infections.

Pulmonary Sarcoidosis Mimicking Eosinophilic Bronchiolitis Diagnosed by Cutibacterium Acnes Positivity.

A 63-year-old woman with bronchial asthma presented with diffuse centrilobular nodules and bronchial wall thickening, initially suggesting eosinophilic bronchiolitis. Surgical lung biopsy revealed well-formed non-caseating granulomas with concomitant eosinophilic bronchiolitis, while Cutibacterium acnes immunostaining established a diagnosis of pulmonary sarcoidosis. This case highlights that pulmonary sarcoidosis can mimic eosinophilic bronchiolitis.

Lung transplantation in a patient with severe bronchiolitis obliterans secondary to Castleman disease.

Castlemann disease (CD) is a rare but often benign condition of the lymph nodes. Seldomly coinciding with CD are other conditions, such as paraneoplastic phemhigus (PMP) and bronchiolitis obliterans (BO). When these occur, patients are at risk of developing severe complications, such as respiratory failure, which may subsequently result in death. Diagnosing and managing conditions related to CD poses a challenge and may cause diagnostic delay. In this case report, we describe the course of illness in a 31-year-old female diagnosed with CD. Prior to the diagnosis of CD, the patient had been dia

The severity of the first occurrence of bronchiolitis increased the risk of developing asthma symptoms.

AIM: The relationship between bronchiolitis and asthma is complex. We assessed whether patients admitted to a paediatric intensive care unit (PICU) with bronchiolitis had a greater risk of developing asthma than patients admitted to a paediatric ward. METHODS: We retrospectively included children under 1 year of age, who were hospitalised for bronchiolitis for the first time at the University Hospital of Caen, France, between 2010 and 2014. The children were divided into two groups: 89 were admitted to the paediatric ward and 89 were admitted to the PICU. We wanted to assess which group devel

The Role of Pulmonary Genetic Variations in the Pathogenesis of Pediatric Postinfectious Bronchiolitis Obliterans.

OBJECTIVE: Postinfectious bronchiolitis obliterans (PIBO) is a chronic airway disease. The severity of the damage and the subsequent obstructive and inflammatory processes varies from one individual to another. The objective was to identify genetic variations that may be associated with pulmonary diseases in patients with PIBO. MATERIAL AND METHODS: This retrospective descriptive study was carried out to define potential genetic changes that may be associated with PIBO. Medical records were used to obtain sociodemographic characteristics. Neutrophil, lymphocyte, platelet counts, immunoglobuli

Nasopharyngeal Microbiome-Epigenome-Wide Association Analysis in Infants With Severe Bronchiolitis.

BACKGROUND: Bronchiolitis exposes infants to both acute burdens (e.g., hospitalization in cases of severe bronchiolitis) and increased risks for chronic respiratory sequelae (e.g., asthma). In severe bronchiolitis, recent evidence suggests distinct pathobiological roles of microbiota (e.g., viruses, bacteria) and host responses influenced by genetic and epigenetic factors. However, the relationship of airway microbiota with host DNA methylation (DNAm) in infants with severe bronchiolitis remains unknown. METHODS: In a multi-center prospective cohort of 504 multi-ethnic infants with severe bro

[Post-infectious bronchiolitis obliterans in children: A rare and potentially devastating disease].

Post-infectious bronchiolitis obliterans (PIBO) is a rare but severe pulmonary disease in children, often associated with adenovirus infection. Risk factors include male sex, hypoxemia, and mechanical ventilation. Diagnosis is based on clinical history, bronchial obstruction, and radiological abnormalities. PIBO is characterized by chronic inflammation leading to tissue remodeling and bronchiolar fibrosis. Airway epithelial lesions, potentially linked to viral infection, are considered key mechanisms of PIBO. In the absence of specific treatments, research efforts aim to better understand mech
