Global black carbon aerosol: Spatiotemporal distributions, drivers, and health impacts.

Black carbon (BC) is a key short-lived climate pollutant and a toxic component of PM2.5. This review synthesizes global BC studies published since 2000 and evaluates its spatiotemporal distribution, source contributions, meteorological drivers, and health risks. The compiled evidence shows high concentrations are mainly found in South Asia and Africa, while low concentrations occur in Europe and North America. South American records were limited in the screened literature, and the apparent lower concentrations in this region may partly reflect sparse monitoring coverage rather than a regionwid

Contamination and Health Risk Assessment of Potentially Toxic Elements in Household Dust Across the Haze Season in Upper Northern Thailand.

Indoor exposure to potentially toxic elements (PTEs) presents a global health concern, yet comprehensive seasonal assessments in Thailand remain limited, particularly during air pollution episodes. We assessed 15 PTEs in household dust collected across eight provinces of upper northern Thailand (UNT) during the haze and non-haze seasons to evaluate contamination levels, identify sources, and assess health risks. Five elements (Cr, Mo, Ni, Pb, and Zn) showed significantly higher concentrations during the haze season (p < 0.05), accompanied by corresponding increases in contamination indices and

Effect of targeting normoxemia on supplemental oxygen-free days for adults with acute thermal burns: A stepped wedge cluster randomized clinical trial.

BACKGROUND: Supplemental oxygen is essential in caring for adults with acute thermal burns but can expose patients to excess inspired oxygen. We sought to determine the safety and effectiveness of targeting normoxemia (peripheral oxygen saturation [SpO 2 ] 90-96%) in adults with acute thermal burns admitted to a specialized burn unit. We hypothesized that targeting normoxemia would increase the number of supplemental oxygen-free days (SOFDs) and safely reduce exposure to hyperoxemia. METHODS: In this multicenter cluster-randomized, stepped-wedge trial, we randomized six US burn centers to cro

[Hemorrhage of the head and neck in death by burning].

The 202 deaths connected with burns in 1976-1986 included 134 in which death occurred during the fire. In 49 cases the face was so badly charred that it was not possible to judge whether hemorrhage had occurred in the eyelids and/or conjuctivae. Such blood extravasation was seen in 14 of the remaining 85 cases, taking the form of petechiae in some cases and in others the form associated with more extensive extravasation, in some cases hemolytic. No other features known to cause such hemorrhages were present, so that they are attributed to the burning process itself. Signs of extensive pronounc

Symptoms among chilli grinders.

In countries where the consumption of chillies is high, the grinding of dried red chillies into a fine powder is a new industry. The majority of workers employed in this process suffer from sneezing, watering of the nose, and cough, all of which are maximal at the beginning of their employment. They develop tolerance within a variable period, after which this triad of symptoms occurs only when the atmosphere becomes heavily contaminated with chilli-powder or on inhalation of the volatilized capsaicin which is liberated when the roasted chillies are being ground. A burning sensation of the skin

Early Autocalibrated Arterial Waveform Analysis for the Management of Burn Shock-A Cohort Study.

Adequate fluid therapy is crucial for resuscitation after major burns. To adapt this to individual patient demands, standard is adjustment of volume to laboratory parameters and values of enhanced hemodynamic monitoring. To implement calibrated parameters, patients must have reached the intensive care unit (ICU). The aim of this study was, to evaluate the use of an auto-calibrated enhanced hemodynamic monitoring device to improve fluid management before admission to ICU. We used PulsioflexProAqt® (Getinge) during initial treatment and burn shock resuscitation. Analysis was performed regarding

Methemoglobinemia resulting from smoke inhalation.

Methemoglobinemia following fire exposure is largely unrecognized. Fire related morbidity and mortality are customarily attributed to thermal injury, associated trauma, and carbon monoxide poisoning. More recently, cyanide poisoning has been described from the inhalation of hydrogen cyanide liberated from of burning plastics (1). Symptoms of tissue hypoxia and cyanosis resulting from methemoglobinemia may be difficult to diagnose in the presence of thermal injury, cyanide and/or carbon monoxide poisoning. Relatively low levels of methemoglobin could complicate concomitant carbon monoxide poiso

Factors influencing the early prediction of outcome from burns.

OBJECTIVE: 1. To reexamine the predictive value of the variables usually used in admission scores in burned patients (age, total body surface area burned (TBSAB), full thickness burn (FTB), inhalation injury (IHT) and sex). 2. To evaluate whether risk factors (alcohol abuse (AA), nicotine abuse (NA)) or preexisting diseases influence outcome significantly. DESIGN: Retrospective study of prospectively collected data. PATIENTS: 498 burned patients admitted to the burn ICU within a 5 years period. The mean TBSAB was 29% and the mean age 38 years. 42% of the patients suffered burns greater than

Personal protection against mosquitoes in Dar es Salaam, Tanzania, by using a kerosene oil lamp to vaporize transfluthrin.

The effectiveness of a cheap and easy method of household protection against Culex quinquefasciatus Say and other mosquitoes (Diptera: Culicidae) was investigated in Dar es Salaam, Tanzania. Kerosene-burning lamps (korobois) were modified to heat and vaporize transfluthrin, a volatile pyrethroid insecticide. When transfluthrin was added to fuel of the lamp, protection against biting was poor unless a very high concentration of insecticide was used. A modified lamp (= vaporizing koroboi) was designed to overcome this problem by mixing the insecticide with vegetable oil and heating it to 120 deg

Voice, Airway, and Swallowing Function in Patients With Smoke Inhalation Injury to the Upper Airway-A Single Institution Review.

Voice and swallowing dysfunction are common after smoke inhalation injury; however, burn survivors often do not seek follow-up for ongoing dysphonia, dysphagia, or exercise intolerance, which may be a result of undiagnosed airway scarring or stenosis. The aim of this study is to evaluate trends in dysphonia, dysphagia, and upper airway injury after smoke inhalation injury. A retrospective case series with chart review was performed of all patients admitted with smoke inhalation injury from 2018 to 2022. Patient demographics, injury patterns, voice, airway, and swallowing symptoms, laryngoscopi

Intravenous Hydroxocobalamin for Cyanide Poisoning From Smoke Inhalation: A Comprehensive Scoping Review.

Smoke inhalation from closed-space fire can result in hydrogen cyanide (HCN) exposure, an underrecognized contributor to morbidity and mortality in burn and smoke inhalation victims. Hydroxocobalamin, a vitamin B12 precursor, is recommended by the American Heart Association for suspected HCN poisoning; however, evidence regarding its real-world use, particularly in the prehospital setting, remains fragmented. This scoping review aims to identify the existing literature on hydroxocobalamin administration for HCN toxicity secondary to smoke inhalation. BIOSIS Previews, Embase, PubMed, and MEDLIN

Hydrocarbon pneumonitis caused by the inhalation of wood preservative.

A 69-year-old man was admitted to our hospital with acute dyspnoea, which developed after using a wood-preserving agent in an enclosed space. Burn injuries were evident on his face, neck, chest, and both upper arms. Bronchoalveolar lavage was carried out. The collected fluid resembled wood preservative. Subsequently, it was established that kerosene was a major component of the wood preservative. A diagnosis of hydrocarbon pneumonitis was established. The patient's respiratory and general findings improved with intensive care, which included mechanical ventilation. Corticosteroid was not requi

Chronic Left Lower Lobe Pulmonary Infiltrates During Military Deployment.

Deployment to Southwest Asia is associated with increased airborne hazards such as geologic dusts, burn pit smoke, vehicle exhaust, or air pollution. There are numerous ongoing studies to evaluate the potential effects of inhaled particulate matter on reported increases in acute and chronic respiratory symptoms. Providers need to be aware of potential causes of pulmonary disease such as acute eosinophilic pneumonia, asthma, and vocal cord dysfunction that have been associated with deployment. Other pulmonary disorders such as interstitial lung disease are infrequently reported. Not all deploym

An analysis of the factors implicated in fire deaths in children.

The aim of this study was to determine: 1) the factors implicated in fire deaths in children, 2) the relevance of blood carbon monoxide (CO) and hydrogen cyanide (CN) concentrations in child fire victims. 34 child fire victims (17 males: 17 females) were studied. Mean age was 4.5 years. In 12 cases medico-legal autopsies were performed, but no toxicological analyses were done (group A). In 14 cases autopsies were not performed, but blood CO and CN concentrations were measured in the victims (group B). In the remaining 8 cases both toxicological analyses and medicolegal autopsies were done (gro

Morphologic changes and prognosis of the respiratory tract epithelium in inhalation injury and their relationship with clinical manifestations.

BACKGROUND: We set out to observe the morphologic changes and determine prognosis based on airway epithelial injury after inhalation injury of varying severity using a fiberbronchoscope and to explore the relationship between the severity of epithelial injury and its clinical manifestation. METHODS: Sixty burn patients with an inhalation injury underwent fiberoptic bronchoscopy. Morphologic changes of the airway epithelium were observed, and the inhalation injury was divided into 3 categories based on the severity of the epithelial injury: first-degree injury, in which the airway epithelium h

Vasopressor Use in Acute Burn Resuscitation: A Retrospective Study.

BACKGROUND: Vasopressors are often introduced in severe burn patients who respond poorly to fluid resuscitation to maintain adequate blood pressure and avoid fluid creep. This study aims to characterise these patients and explore potential correlations between vasopressor use and patient outcomes. METHODS: Medical records from 2010 to 2023 for patients aged ≥ 18 years with burns ≥ 20% of the total body surface area (TBSA) were extracted from the TriNetX Research Network, a global federated health research platform aggregating de-identified electronic health records. Out of the Fire, Into the Fog: Association Between Early Mechanical Ventilation and Delirium in Hospitalized Burn Patients.

Introduction Delirium is a common and clinically significant complication among hospitalized burn patients, associated with increased morbidity, mortality, and healthcare utilization. While multiple factors contribute to delirium risk, mechanical ventilation has been associated with delirium, although this relationship may reflect underlying illness severity and ICU-level treatment exposure. However, large-scale, multicenter administrative analyses comparing coded outcomes between mechanically ventilated and non-ventilated burn patients remain limited. Methods A retrospective cohort study was
