Biochemical parameters of Ca homeostasis in fetal and maternal plasma and placental transfer of Ca, Pi and vitamin D metabolites were measured during pregnancy and at parturition. Control pigs and pigs with inherited pseudo-vitamin D-deficiency rickets, type I (PVDR), which are devoid of renal calcitriol production, were used. Although sows with PVDR normally require treatment with massive doses of vitamin D at intervals of 4 weeks to stabilize Ca and Pi concentrations in plasma, these plasma parameters tended to normalize during the first 3 months of pregnancy when the vitamin D treatment was…

INTRODUCTION: Calcium-sensing receptor (CaSR) variants are a rare cause of congenital hypoparathyroidism. Current standard treatment consists of calcitriol and calcium supplements, but this regimen increases the risk of kidney complications due to hypercalciuria.
CASE PRESENTATION: Here, we present the case of a toddler with an activating CaSR mutation and severe complications of calcium and calcitriol treatment, who exhibited significant clinical improvement after initiation of teriparatide, an injectable parathyroid hormone analog.
CONCLUSION: Studies of teriparatide use in children have s…

Secondary hyperparathyroidism (SHPT) is characterized by the acceleration of bone turnover due to the over-secretion of parathyroid hormone, which is accompanied with parathyroid hyperplasia. The pathogenesis includes the reduced production of calcitriol due to the elevated serum fibroblast growth factor 23 (FGF23) level to maintain phosphate (P) homeostasis, and its resultant hypocalcemia. Although the mechanism parathyroid glands sense P remains unknown, the expressions of calcium (Ca) sensing receptor and vitamin D receptor, which sense serum level of Ca and calcitriol respectively, down-re…

Secondary hyperparathyroidism is a systemic disorder that associates with bone and cardiovascular disease, including arterial calcification. Treatment with calcitriol, the active form of vitamin D, reduces parathyroid hormone levels, but may result in elevations in serum calcium and phosphorus, increasing the risk of vascular calcification in dialysis patients. New vitamin D receptor activators (VDRAs) have been developed and investigated with the rationale to treat high serum PTH levels, with a reduced risk of hypercalcemia and hyperphosphatemia. Paricalcitol is a selective VDRA that suppress…

This review summarizes the factors involved in the development of hyperparathyroidism secondary (2nd-HPTH) to chronic kidney disease (CKD). Calcium and calcitriol act on their respective specific parathyroid cell receptors to inhibit parathyroid function. As well as the well-known effect of calcium and calcitriol on parathyroid cell function, there is experimental work that demonstrates that phosphate, changes in pH, PTHrP, estrogens, and some cytokines also have an effect on PTH secretion. These factors are relevant in patients with chronic kidney disease. However, low calcium, vitamin D defi…

Vitamin D is an important hormone for mineral homeostasis and the proper formation and maintenance of bone. In addition, vitamin D has broader functions in the body that expand its traditionally known role in mineral balance. In chronic renal failure, calcitriol deficiency contributes to the development and progression of secondary hyperparathyroidism, bone disorders, and altered mineral metabolism. Recent revelations of the broader role of vitamin D also suggest calcitriol deficiency may contribute to decreased cardiac and immune function in chronic renal failure patients. Research on vitamin…

In 4 of our patients on chronic dialysis, we were intrigued by the association of hypercalcemia +/- hyperphosphatemia and normal intact PTH, with anicteric cholestasis without cytolysis. This picture occurred in 2 patients after they resumed dialysis because of a transplant rejection and in a third one after discontinuation of corticosteroids, prescribed for an idiopathic thrombocytopenia. No patient was under calcitriol, CaCO3 therapy, and their hypercalcemia persisted on a low calcium dialyzate (1.25 mmol/l). Obvious etiologies of hypercalcemia were not found: vitamin D or A intoxication, hy…

A 38-yr-old primigravida presented at 30 + 2 wk gestation with pregnancy-induced hypertension and was found to have severe hypercalcemia (adjusted calcium 3.19 mmol/L). Intravenous fluid therapy produced only transient improvement, and recurrent hypercalcemia required repeated inpatient management. PTH was suppressed, while both 25(OH)D₃ and 1,25(OH)₂D₃ (calcitriol) were elevated. Imaging and laboratory investigations revealed no evidence of malignancy or granulomatous disease. Delivery by elective cesarean section was undertaken at 35 wk gestation. The neonate developed transient hypocalcemia…

We report a patient who initially presented at 4 days old with hypocalcemia, hypoparathyroidism, and elevated phosphorous level. Treatment was initiated with calcitriol, calcium carbonate (CaCO3), vitamin D, and low phosphorous formula. Family history was positive for an activating calcium sensing receptor (CASR) variant (R990G) identified previously in 2 older siblings who were treated with CaCO3 and calcitriol. However, genetic studies were negative for the CASR variant in our patient. She maintained a large calcium requirement and was admitted for multiple episodes of hypocalcemia. Further …

Foreign body-induced granuloma is an uncommon yet clinically significant cause of hypercalcemia. The molecular mechanisms are uncertain, although extrarenal calcitriol production has been proposed. We describe severe hypercalcemia associated with increased levels of plasma calcitriol in a patient with HIV and local granulomatous reaction 5 years after injection of polymethylmethacrylate (PMMA) as dermal filler for cosmetic body sculpting. Extensive evaluation revealed no identifiable cause of increased calcitriol levels. Nuclear imaging was remarkable for diffuse uptake in the subcutaneous tis…

The intestinal absorption of calcium and seric levels of calcifediol and calcitriol were studied in 27 patients with essential blood hypertension (BH) and 20 normal patients in a control group. The hypertensive patients were divided in three groups depending on the plasmatic renin activity (PRA), (high, normal, low). We found a significantly higher rate of intestinal calcium absorption in the group of patients with BH and low PRA than in the control group (22.73 +/- 19.05 and 10.7 +/- 17.13% respectively) (p less than 0.025). The seric levels of calcitriol (1,25 (OH) 2D3) were significantly hi…

The 26 and 27 positions of vitamin D molecular structure of calcitriol were fluorinated with 3 atoms of fluorine each and the new compound was named falecalcitriol (F6). This new compound was found to be 10 to 100 times more active compared with calcitriol depending on the target organs. As a mechanism of strong action of F6 it was discovered that F6 is hydroxylated at 23 position which has almost the same activity as the mother compound. It was also demonstrated that the PTH suppressive effect was relatively stronger than the calcium absorption action from the intestine. Thus F6 was authorize…

(1) Severe malignant osteopetrosis is a very rare disease. The principal manifestations are anaemia, infections, sensory disorders and fractures, due to generalised bone condensation. The disease is generally fatal in childhood. The only treatment capable of modifying the natural outcome is bone marrow transplantation. The benefits of high-dose steroids and calcitriol are usually modest and transient. (2) Severe malignant osteopetrosis is a new licensed indication for interferon gamma-1b, a drug known to reduce the incidence of severe infections in children with chronic septic granulomatosis. …

BACKGROUND: Although bisphosphonates have been widely used to treat bone diseases characterized by increased bone resorption, there are limited data showing their possible usefulness in patients on hemodialysis (HD) with secondary hyperparathyroidism.
METHODS: The aim of this study was to evaluate the efficacy and safety of pamidronate in HD patients affected by severe secondary hyperparathyroidism and moderate hypercalcemia who were receiving intravenous calcitriol (Calcijex).
RESULTS: In this prospective one-year, open-labeled study, 13 patients (9 women/4 men) with a mean age of 64 +/- 9 …

Secondary hyperparathyroidism complicating chronic kidney disease requires therapy to minimize the effects of parathyroid hormone (PTH) on bone and other tissues. Low levels of calcitriol in blood play a major role in the initiation and maintenance of hyperparathyroidism. Accordingly, administration of calcitriol has been demonstrated to be an effective form of therapy. While this therapy is effective in controlling hyperparathyroidism, side effects of calcitriol, including increased intestinal absorption of calcium and phosphate, often complicate therapy by giving rise to hypercalcemia and hy…
