Epidural clonidine analgesia for intractable cancer pain. The Epidural Clonidine Study Group.

Although the vast majority of patients with cancer pain receive effective analgesia from standard therapy, a few patients, particularly those with neuropathic pain, continue to experience severe pain despite large doses of systemic or intraspinal opioids. Animal studies suggest intraspinal alpha 2-adrenergic agonists may be effective in such cases. Eighty-five patients with severe cancer pain despite large doses of opioids or with therapy-limiting side effects from opioids were randomized to receive, in a double-blind manner, 30 micrograms/h epidural clonidine or placebo for 14 days, together …

Intrathecal morphine and bupivacaine in advanced cancer pain patients implanted at home.

Fifteen patients with pain due to advanced cancer who no longer obtained relief from conventional treatment using oral or parenteral opioids were administered morphine and bupivacaine by continuous intrathecal infusion. Thirteen patients were implanted at home due to poor medical condition or refusal to be hospitalized. A summary score was derived to monitor the effects of the treatment. Thirteen patients required low doses of intrathecal morphine and bupivacaine and all reported good pain relief until death. Only minor side effects were evidenced. Implantation at home of an intrathecal cathet

Organic hallucinosis in patients receiving high doses of opiates for cancer pain.

We report 4 patients who developed organic hallucinosis while receiving opiate analgesics for cancer pain. In all patients, the symptoms rapidly responded to haloperidol and change in the type of opiate. The hallucinations were exclusively visual. Cognitive status, determined by the Mini-mental State Questionnaire (MMSQ), was normal in all patients. Organic hallucinosis occasionally occurs in patients receiving opiates and can be a major source of distress for patients and their families.

Plasma morphine concentrations during chronic oral administration in patients with cancer pain.

Plasma concentrations of morphine were studied in 16 patients with chronic pain due to advanced cancer, treated with regular doses of morphine chloride by the oral route. Doses ranged from 20 to 125 mg every 4 h and a significant, positive, linear correlation was found between dose (range 0.22-3.01 mg/kg) and mean plasma concentration (range 22-364 ng/ml). The profile of the 4 h concentration curve is described and found to be subject to considerable, individual variation. The present investigation supports the experience that good pain relief can be obtained, if individual requirements as to

Psychological determinants of pain, fatigue, and symptom burden in breast cancer survivorship.

Breast cancer survivors frequently experience persistent pain, cancer-related fatigue, and interconnected symptom clusters that impair quality of life, functional status, and treatment adherence. Psychological factors, including pain catastrophizing, anxiety, depression, fear-avoidance behaviors, and maladaptive cognitive appraisals, are associated with the amplification and maintenance of these symptoms through proposed mechanisms involving central sensitization, HPA axis dysregulation, inflammatory signaling, and behavioral deconditioning. This narrative overview, rather than a systematic re

Paracetamol and NSAIDs in Cancer Pain Management: Evidence Review and Treatment Considerations.

OPINION STATEMENT: Pain is one of the most prevalent and distressing symptoms in cancer, affecting up to 90% of patients and significantly impairing quality of life. Paracetamol (acetaminophen) and non-steroidal anti-inflammatory drugs (NSAIDs) are widely recommended by international guidelines for the management of non-surgical cancer pain, either alone or as adjuvants to opioids. In this paper, the current evidence for their efficacy, tolerability, and safety in this setting is reviewed. Evidence for paracetamol in cancer pain remains limited and of low quality. Small trials and systematic r…

Evaluation of dronabinol to decrease opioid use for cancer-induced bone pain.

BACKGROUND: Bone metastases (BM) from breast cancer cause significant cancer-induced bone pain (CIBP). Management of CIBP is primarily with opioids, which have notable side effects. In preclinical models, cannabinoid receptor (CB)2 and CB1 agonists were shown to decrease CIBP and bone degradation. We hypothesized that the addition of CB2/CB1 agonists would decrease opioid requirements in patients with BM. METHODS: We conducted a single-arm study among breast cancer patients with BM on opioid therapy. Patients were treated with 10 mg dronabinol BID for 8 weeks. Our primary objective was to det

Pain prevalence and intensity in advanced pancreatic cancer: a nationwide cohort study.

Pain is a major concern in patients with advanced pancreatic cancer, significantly affecting quality of life, treatment tolerance, and end-of-life experience. However, data on its prevalence, course, and determinants remain limited, particularly regarding the dynamics of pain as a primary outcome in real-world patient populations. This nationwide cohort study, conducted within the PACAP-PROM registry (2016-2023), included patients with locally advanced (LAPC) or metastatic pancreatic cancer (mPC). Pain was assessed using 5 items from the EORTC QLQ-C30, PAN26, and EQ-5D-5L questionnaires, with

When Cancer Mimics Pain: Maxillary Primary Intraosseous Carcinoma Misdiagnosed as Trigeminal Neuralgia.

Background: Primary intraosseous carcinoma (PIOC) is a rare and aggressive odontogenic malignancy that originates within the jaw bones without initial mucosal involvement. Its atypical and nonspecific symptoms frequently lead to diagnostic delays, especially in maxillary presentations. Methods: A 74-year-old male presented with persistent trigeminal-like neuralgic pain along the ophthalmic branch, initially misdiagnosed as secondary trigeminal neuralgia. MRI revealed a 45 × 46 × 34 mm mass occupying the right maxillary sinus with orbital wall destruction and dural invasion. Following histopath

Latent Profile Analysis of Pain Catastrophizing in Post-Operative Lung Cancer Patients.

BACKGROUND: Patients with lung cancer often experience high levels of pain and recurrent pain episodes after thoracoscopic surgery, compromising their adherence to rehabilitation exercises and quality of life. Pain catastrophizing is defined as an exaggerated and negative mindset towards actual or anticipated pain and is one of the significant factors influencing pain. To date, prior investigations on pain catastrophizing have predominantly employed variable-centered approaches, focusing on broader population levels while neglecting individual specificity. Latent profile analysis represents an

Chinese acupoint therapies in cancer pain management: research advances and future perspectives.

Cancer pain management (CPM) is crucial in oncology care, with current approaches including pharmacotherapy, radiotherapy, chemotherapy, nerve blocks, and psychological support. However, long-term drug use risks adverse effects and addiction, while physiotherapies often lack sustained efficacy. Therefore, identifying safer adjuvant analgesic therapies has become an urgent issue. Traditional Chinese Medicine (TCM) acupoint therapies (acupuncture, moxibustion, and acupoint injection) offer safer adjuvant analgesia with growing clinical validation. This narrative review aims to introduce the rese…

Case Report: Atypical prostate cancer presentation: rectal bleeding, pain, and psoriasiform dermatitis.

Prostate cancer is typically asymptomatic and usually diagnosed through concerted screening programs. However, in settings where there are no existing national prostate cancer screening programs, it may be picked up at the clinics in patients presenting with urinary symptoms, erectile dysfunction, and hematospermia/hematuria. Rare atypical presentations may also occur, delaying diagnosis and management. This case report discusses a 61-year-old male of Black-African descent, whose first presentation to the hospital for a condition ultimately diagnosed as metastatic prostate cancer, was due to l
