Noninvasive subject-specific monitoring of autonomic-cardiac regulation.

This paper presents a feasibility study of a model-based approach to noninvasive and subject-specific monitoring of autonomic-cardiac regulation. The proposed approach is built upon individualizing a physiologically-based model by applying a parameter estimation method to routine clinical observations, thereby assuring physical transparency, computational efficiency, and clinical adaptability. To develop an efficient parameter estimation procedure, a parametric sensitivity analysis was performed on the autonomic-cardiac regulation model to identify high-sensitivity model parameters whose chang

Correlation between fetal-placental doppler indices and maternal cardiac function in pregnant women with late-Onset preeclampsia or fetal growth restriction.

BACKGROUND: Fetal growth restriction (FGR) and late-onset preeclampsia (late-onset PE) reflect placental dysfunction but exhibit distinct maternal hemodynamics within the “maternal cardiovascular-placental-fetal unit”. INTRODUCTION: Our objective was to investigate the correlation between fetal-placental Doppler indices and maternal cardiac function in pregnant women with late-onset PE or FGR. METHODS: A total of 90 pregnant women at 35–39⁺⁶ weeks of gestation were enrolled and divided into three groups: Control (n = 30), FGR (n = 30), and late-onset PE (n = 30). Doppler ultrasonography was us

Hereditary haemorrhagic telangiectasia is a genetic disease, causing abnormal formations of blood vessels in skin, mucus membranes, lungs, liver, and brain. In the liver, the disease results in shunting of blood, bypassing the capillary bed. Recent studies have shown that the prevalence of liver shunts are more frequent than previously suggested. The patients present with symptoms related to high-output cardiac failure causing dyspnoea and oedema. Liver shunts can be shown using CT-scans and ultrasonography. The only curable treatment is a liver transplant; however, it is the last treatment op

Shoshin beriberi with severe metabolic acidosis.

A case of Shoshin beriberi found in a patient with severe metabolic acidosis and shock was presented. We believe the metabolic acidosis in this case is ascribable to overproduction of pyruvate and lactate resulting from lack of thiamine due to preferential intake of sake, precooked food, and rice, and also to shock. The hemodynamic studies done after recovery from shock revealed that the cardiac output, which was initially high, decreased to a slightly low level and then increased to a normal level, concomitant with an increase in the peripheral vascular resistance after introductory administr

Non-dipping blood pressure patterns and arterial stiffness parameters in patients with Behcet's disease.

Behcet's disease is a multisystemic vasculitis involving veins and arteries of various sizes. Non-dipping status, augmentation index and pulse wave velocity are important determinants of cardiovascular mortality and morbidity. We investigated the non-dipping status and arterial stiffness in patients with Behcet's disease. In this cross-sectional study, we examined the vascular parameters of 96 patients with Behcet's disease (53% female) and 60 age- and sex-matched control subjects. The non-dipping status and arterial distensibility were assessed using a Mobil-O-Graph Arteriograph, an automatic

Perioperative Metabolomic Profiling in Neonatal Cardiac Surgery Identifies a High-Risk Phenotype for Adverse Outcomes.

BACKGROUND: Neonatal cardiac surgery is associated with high morbidity and mortality. Perioperative metabolic stress may result in distinct metabolic phenotypes associated with adverse clinical outcomes. OBJECTIVES: The purpose of this study was to identify perioperative metabolomic phenotypes associated with adverse clinical outcomes in neonates undergoing cardiac surgery with cardiopulmonary bypass (CPB). METHODS: Plasma samples were collected from 230 neonates at 3 institutions at preoperative, post-CPB, and 24-hour time points. Targeted metabolomics was performed via mass spectrometry. F

Subretinal drusenoid deposits, age-related macular degeneration, and cardiovascular disease.

Decades of studies on age-related macular degeneration (AMD), cardiovascular disease and stroke have not found consistent associations between AMD and systemic vascular disease. This study suggests that there is in fact no general relationship, but instead a strong, specific association between only the subretinal drusenoid deposit (SDD) phenotype of AMD on retinal imaging and certain co-existent vascular diseases that are high risk for compromised cardiac output or internal carotid artery stenosis. Future screening initiatives for these high -risk vascular diseases (HRVDs) with fast, inexpens

Effects of high-dose intravenous methylprednisolone on circulation in humans.

We describe four patients who suffered asystole, myocardial infarction (in two cases) and supraventricular tachycardia at the time of high-dose methylprednisolone infusion. One patient died during infusion. We measured the effects of methylprednisolone and placebo infusion on heart rate, blood pressure, and cardiac output in six stable renal allograft recipients. A significant rise in arterial pressure, caused by an increase in total peripheral resistance, occurred during methylprednisolone infusion. Possible mechanisms include potentiation by methylprednisolone of the vascular effects of the

Minithoracotomy approach for redo mitral valve surgery: Feasibility, operative outcomes, and comparative analysis of myocardial protection techniques.

OBJECTIVE: Redo mitral valve surgery (MVS) poses technical challenges and high operative risks as the result of adhesions and sternal re-entry. Right minithoracotomy has emerged as an alternative to repeat sternotomy. This study evaluated feasibility and perioperative outcomes of redo MVS through right minithoracotomy and compared 3 different myocardial protection strategies: cardioplegia, ventricular fibrillation (VF), and beating-heart. METHODS: All patients who underwent redo MVS via right minithoracotomy between 2015 and 2023 were identified from the Mini-Mitral International Registry. Th

Bile acid predicts congenital portosystemic venous shunt in patients with pulmonary arterial hypertension.

The etiology of pulmonary arterial hypertension (PAH) is complex, especially the investigation of rare pathogeny is difficult. Congenital portosystemic venous shunt (CPSS) is a rare congenital anomaly in which the portal blood completely or partially bypasses the liver through a congenital portosystemic shunt, and drains directly into the inferior vena cava (IVC) (Howard and Davenport in J Pediatr Surg 32:494-497, 1997).CPSS is an uncommon cause of PAH (Christiane et al. in J Pediatr Gastroenterol Nutr 56:675-681, 2013), and often covered by other pathogenic factors. The clinical manifestation

Lack of sustained vagal control of gastric mucosal blood flow. Why vagotomy is not effective in preventing recurrent hemorrhage from stress ulcers.

Vagotomy with drainage or resection has been advocated for control of hemorrhage from stress-related gastric erosions despite the high rate of associated rebleeding. The object of this study was to evaluate the effect of truncal and selective vagotomy on gastric mucosal blood flow under both normotensive and ischemic conditions to ascertain why rebleeding occurs. Fifteen miniature swine were divided into three groups according to the surgical procedure they underwent: (1) pyloroplasty alone, (2) truncal vagotomy and pyloroplasty, and (3) selective vagotomy and pyloroplasty. Four weeks postoper

[Pregnancy risks in acquired heart diseases].

Optimal management of pregnancies for patients with acquired heart disease requires exact knowledge of the hemodynamic influence of pregnancy-related cardiovascular adaptation processes on the heart disease. Maternal and fetal risks must be carefully considered and mutually weighed. Critical time periods, during which closely networked, interdisciplinary support for the patient is essential, are primarily during the 30th to 32nd week of pregnancy. This is the period in which maximum increases in heart rate, cardiac output, and plasma volume are observed. The peripartal phase represents another

Contemporary overview of current percutaneous mechanical circulatory support devices for advanced heart failure: a narrative review.

Mechanical circulatory support devices have become essential in the management of acute hemodynamic compromise, providing rapid, minimally invasive augmentation of cardiac output in heart failure, cardiogenic shock, high-risk percutaneous coronary intervention, post-cardiotomy ventricular dysfunction, and refractory cardiac arrest. Unlike conventional ventricular assist systems, percutaneous mechanical circulatory support devices are designed for swift deployment through peripheral vascular access and are intended for short-term support. Advancements over recent decades have substantially broa

Aortocaval Fistula Presenting as Type 2 Acute Myocardial Infarction in a Patient with Severe Arteriopathy.

Aortocaval fistulas (ACFs) are rare with varied etiologies. Symptoms can be acute or delayed with predominant manifestations being high output cardiac failure. Acute coronary syndrome due to ACF has not been widely reported. We present a case of a 68-year-old male who presented with signs and symptoms suggestive of acute coronary syndrome. This was confirmed by electrocardiogram changes and a rise in cardiac enzymes. A large abdominal aortic aneurysm was diagnosed initially by imaging without evidence of leak or rupture. A coronary angiogram showed only mild diffuse disease. On further reviewi

Recent strategies in treatment of pulmonary arterial hypertension, a review.

Pulmonary arterial hypertension (PAH) is a disease characterized by an elevation in pulmonary artery pressure that can lead to right ventricular failure and death. The pulmonary circulation has to accommodate the entire cardiac output in each cardiac cycle and evolution has adapted to this by making it a low-pressure high-flow system. However, pathology can affect both the arterial and venous components of this system. Pulmonary venous hypertension mainly refers to diseases that result in elevated venous pressure and occurs mainly from mitral valve and left-sided heart disease. Standard treatm

Pneumatosis intestinalis and hepatic portal venous gas after CPR.

Pneumatosis intestinalis and hepatic portal venous gas are usually associated with severe intra-abdominal pathologies. As diagnostic technologies advanced, a number of variant etiologies have been identified. We report 2 cases in which pneumatosis intestinalis and hepatic portal venous gas developed after prolonged cardiopulmonary resuscitation (CPR). The pathogenic mechanism was most probably bowel infarction caused by poor mesenteric perfusion during and after CPR. Limited cardiac output during prolonged resuscitation and severe vasoconstriction after large doses of epinephrine and vasopress
