Metastatic Uterine Leiomyosarcoma as a Rare and Sinister Cause of Respiratory Distress: A Case Report and Literature Review.

Uterine leiomyosarcomas are an extremely rare subtype of uterine malignancy. This is a case report of a 47-year-old woman whose underlying u … preoperative work-up. These challenges are amplified where resources may be limited, such as in the Caribbean region, where radiographic imaging and treatment options may not always be readily available.

Variation in COVID-19 booster uptake in England: An ecological study.

INTRODUCTION: Variable and low uptake of the COVID-19 booster is a recognised problem, associated with individual characteristics including age, gender, ethnicity, and deprivation. Are there other relevant predictors at area level? METHODS: Anonymous grouped data was downloaded from the UK Government Coronavirus Dashboard for Middle Super Output Areas (MSOA) in England, along with demographic, employment, and health data from public sources. Mixed models with a random intercept for Upper Tier Local Authority were analysed as quasibinomial Generalized Additive Models. The estimated random effe

Influence of Climatic Factors on Human Hantavirus Infections in Latin America and the Caribbean: A Systematic Review.

BACKGROUND: With the current climate change crisis and its influence on infectious disease transmission there is an increased desire to understand its impact on infectious diseases globally. Hantaviruses are found worldwide, causing infectious diseases such as haemorrhagic fever with renal syndrome (HFRS) and hantavirus cardiopulmonary syndrome (HCPS)/hantavirus pulmonary syndrome (HPS) in tropical regions such as Latin America and the Caribbean (LAC). These regions are inherently vulnerable to climate change impacts, infectious disease outbreaks and natural disasters. Hantaviruses are zoonoti

Drought, Extreme Heat, and Intimate Partner Violence in Low- and Middle-Income Countries.

IMPORTANCE: Recent evidence has shown that elevated temperatures are associated with a higher risk of intimate partner violence (IPV), a serious violation of human rights. However, it remains unclear whether drought also heightens the risk and whether drought and extreme heat jointly affect IPV. OBJECTIVE: To investigate the interactive association between drought, extreme heat, and IPV in 42 low- and middle-income countries (LMICs). DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study included women answering multicluster surveys in sub-Saharan Africa, South and Southeast Asia, Lat

Metabolite profiling-based characterization and antibacterial activity of sea cucumber Isostichopus sp. aff. badionotus from the Colombian Caribbean Sea.

ObjectiveMarine organisms such as sea cucumbers represent valuable sources of specialized metabolites with promising applications, making them relevant targets for exploring metabolite distribution and subsequent in-depth characterization. This study aimed to chemically and biologically characterize the parent methanolic extract and its derived fractions, obtained through sequential liquid-liquid partitioning, from the sea cucumber Isostichopus sp. aff. badionotus collected from the Colombian Caribbean Sea.MethodsThe extract and fractions were analyzed using ultra-high-performance liquid chrom

Perspective: Infant Feeding Policies among Women Living with HIV in Latin America and the Caribbean: Should They Be Updated?

Among women living with HIV (WLHIV), adherence to highly active antiretroviral treatment (HAART) combined with undetectable maternal viral loads and exclusive breastfeeding during the first 6 mo of life dramatically reduces the risk of mother-to-child transmission of HIV. This knowledge has led to updated World Health Organization infant feeding guidelines for WLHIV, calling for governments to support safe breastfeeding practices among WLHIV who want to breastfeed their infants by providing universal access to HAART, viral load tracking, and high-quality breastfeeding counseling and other need

A homozygous truncating NALCN variant in two Afro-Caribbean siblings with hypotonia and dolichocephaly.

NALCN encodes a sodium ion leak channel expressed in the nervous system that conducts a persistent influx of sodium ions to facilitate action potential formation. Homozygous or compound heterozygous loss of function variants in NALCN cause infantile hypotonia with psychomotor retardation and characteristic facies-1 (IHPRF1; OMIM 615419). Through exome and Sanger sequencing, we found two siblings of Afro-Caribbean ancestry who are homozygous for a known NALCN pathogenic variant, p.Arg735Ter, leading to failure to thrive, severe hypotonia, and dolichocephaly. The older sibling died suddenly with

Access to sexual and reproductive health services for women living with HIV in Latin America and the Caribbean: systematic review of the literature.

INTRODUCTION: Systematic reviews show that women living with HIV (WLHIV) have high unmet sexual and reproductive health (SRH) needs due to barriers to access sexual and reproductive health services (SRHS). In Latin America and the Caribbean (LAC), as of 2016, there were nearly one million WLHIV, but the existing evidence of their SRH needs comes from a few individual studies. This systematic review provides an overview of these women's needs to help define new and/or adapt existing public health strategies to the local context. This review synthesizes the evidence from the literature on the us

Evaluation of left ventricular remodelling in young Afro-Caribbean athletes.

BACKGROUND: Cardiac adaptation to intense physical training is determined by many factors including age, gender, body size, load training and ethnicity. Despite the wide availability of ECG analysis, with a higher presence of abnormalities in different races, echocardiographic studies on young Afro-Caribean (AA) and Caucasian athletes (CA) are lacking in literature. We aimed to assess the effect in the secondary LV remodelling of load training in young AA players compared to matched CA players. METHOD: Seventy-seven AA and 53 CA matched soccer players (mean age 17.35 ± 0.50 and 18.25 ± 0.77 y

Discrimination and sexual risk among Caribbean Latinx young adults.

Objectives: Discrimination, such as being treated unfairly due to race, contributes to stress. Individuals may cope with this by engaging in risky behaviors. Consistent with this premise, prior studies found that discrimination is associated with substance use. Research has also shown that sex while 'high' on alcohol and drugs is associated with increased risk for HIV and other STIs. The present study examines the relationship between discrimination and sexual risk. We investigate whether discrimination is associated with sex while high on alcohol and drugs. Design: Analyses focus on a sample

Early breastfeeding practices: Descriptive analysis of recent Demographic and Health Surveys.

The aim of this study was to describe early breastfeeding practices (initiation within 1 hr of birth, no prelacteal feeding, and a combination of both-"optimal" early breastfeeding) according to childbirth location in low- and middle-income countries. Using data from the most recent Demographic and Health Survey (2000-2013) for 57 countries, we extracted information on the most recent birth for women aged 15-49 with a live birth in the preceding 24 months. Childbirth setting was self-reported by location (home or facility) and subtype (home delivery with or without a skilled birth attendant; p

Effect of Fascioloides magna (Digenea) on fecundity, shell height, and survival rate of Pseudosuccinea columella (Lymnaeidae).

Infection with Fascioloides magna (Digenea) causes serious damage to liver tissue in definitive hosts represented by ruminants, especially cervids. The distribution of F. magna includes the indigenous areas in North America, and the areas to which F. magna was introduced-Central Europe, Southeast Europe, and Italy. The North American intermediate host of F. magna, the freshwater snail Pseudosuccinea columella (Lymnaeidae), is an invasive species recorded in South America, the Caribbean, Africa, Australia, and west and Southeast Europe. In Europe, Galba truncatula is the snail serving for trans

Genetic and morphological evidence of a geographically widespread hybrid zone between two crocodile species, Crocodylus acutus and Crocodylus moreletii.

Hybrid zones represent natural laboratories to study gene flow, divergence and the nature of species boundaries between closely related taxa. We evaluated the level and extent of hybridization between Crocodylus moreletii and Crocodylus acutus using genetic and morphological data on 300 crocodiles from 65 localities. To our knowledge, this is the first genetic study that includes the entire historic range and sympatric zone of the two species. Contrary to expectations, Bayesian admixture proportions and maximum-likelihood estimates of hybrid indexes revealed that most sampled crocodiles were a

Dengue in an area of the Colombian Caribbean.

BACKGROUND: In Colombia, dengue is an endemic disease and the four serotypes have been reported. OBJECTIVE: To describe the frequency and severity of dengue in an area of the Colombian Caribbean (Department of Cordoba). METHODS: A retrospective study was conducted. Two data sources were analysed: The database from the Direction of Health in Córdoba, and clinical registers of patients diagnosed with haemorrhagic fevers and fevers of unknown origin in reference hospitals. RESULTS: The mean incidence of dengue between 2003-2010 was 36.5 cases/10(5) inhabitants (CI95%: 34.3-37.5) and adjusted f

Caribbean and La Réunion Chikungunya Virus Isolates Differ in Their Capacity To Induce Proinflammatory Th1 and NK Cell Responses and Acute Joint Pathology.

UNLABELLED: Chikungunya virus (CHIKV) is a mosquito-borne arthralgic alphavirus that has garnered international attention as an important emerging pathogen since 2005. More recently, it invaded the Caribbean islands and the Western Hemisphere. Intriguingly, the current CHIKV outbreak in the Caribbean is caused by the Asian CHIKV genotype, which differs from the La Réunion LR2006 OPY1 isolate belonging to the Indian Ocean lineage. Here, we adopted a systematic and comparative approach against LR2006 OPY1 to characterize the pathogenicity of the Caribbean CNR20235 isolate and consequential host

Ethnic differences in HIV risk perceptions and behaviors among Black 18-39 year-old residents of Broward County, Florida.

The study assessed ethnic differences in the perceived risks of HIV infection, sexual experiences, and HIV-antibody testing histories among Black populations in Broward County, Florida. Data were analyzed for 2,731 well-characterized survey respondents 18-39 years old who indicated they were African Americans, Caribbean Islanders, English-speaking, or Creole-speaking Haitians. Creole-speaking Haitians were least likely to consider themselves at risk of HIV infection, report using condoms in the last 12 months, and indicate that they had ever been tested for HIV. English-speaking Haitians were

Nitrous oxide emissions from the gulf of Mexico hypoxic zone.

The production of nitrous oxide (N(2)O), a potent greenhouse gas, in hypoxic coastal zones remains poorly characterized due to a lack of data, though large nitrogen inputs and deoxygenation typical of these systems create the potential for large N(2)O emissions. We report the first N(2)O emission measurements from the Gulf of Mexico Hypoxic Zone (GOMHZ), including an estimate of the emission "pulse" associated with the passage of Tropical Storm Edouard in August, 2008. Prestorm emission rates (25-287 nmol m(-2) hr(-1)) and dissolved N(2)O concentrations (5 - 30 nmol L(-1)) were higher than val

Human papillomavirus vaccine policy and delivery in Latin America and the Caribbean.

Cervical cancer caused by human papillomavirus (HPV) is a major preventable public health problem. Two vaccines are now available for primary prevention of HPV infection and their introduction offers new opportunities to enhance comprehensive cervical cancer prevention and control. Currently, HPV vaccine price is a significant barrier to rapid vaccine introduction and access. Therefore, making evidence-based decisions about whether and how to introduce HPV vaccine into the immunization schedule in the countries of Latin America and the Caribbean (LAC) requires a rigorous analysis of several fa

Incorporating ethnographic methods in multidisciplinary approaches to risk assessment and communication: cultural and religious uses of mercury in Latino and Caribbean communities.

The potential risk from cultural and religious practices involving mercury in Latino and Caribbean communities raises central methodological and ethical questions for risk assessment and risk communication. Here, specific cultural practices unfamiliar to most risk professionals carry significant inherent risks in the eyes of those professionals but not necessarily in the eyes of practitioners. Practitioners' past experience and history as targets of religious suppression and anti-immigrant sentiment create a reluctance to engage with researchers or public health officials in risk assessment or
