PURPOSE: An accurate evaluation of the penetrance of BRCA1 and BRCA2 mutations is essential to the identification and clinical management of families at high risk of breast and ovarian cancer. Existing studies have focused on Ashkenazi Jews (AJ) or on families from outside the United States. We have characterized the mutations in 1050 carriers of the beta-thalassemia gene and analyzed their regional distribution in India. The majority of beta-thalassemia carriers were migrants from Pakistan and their pattern of mutations differed from the rest. The frequency of the 619-bp deletion was 33.3% among the migrants from Pakistan, 8-17% in the northern states, and less than 5% in the other states. We evaluated serotonin (5HT) uptake parameters in platelets of a group of bipolar I patients in both a depressive phase and a mixed state, before (t0) and after (t1) the first electroconvulsive treatment (ECT). The results showed the presence of significant changes in the number of the carrier proteins for 5HT at t1, which in most cases preceded the clinical improvement. Although other mechanisms are certainly involved, these data suggest that ECT may influence the 5HT transport through platelet membranes.

The effects of prophylactic tinidazole on the vaginal carrier state of anaerobic bacteria and the development of postoperative infections were studied in 50 patients undergoing gynaecological surgery. The results are based on a double-blind trial, using tinidazole (Fasigyn; Pfizer) and placebo. Although no postoperative infections other than urinary tract infections were observed in our series, this study shows conclusively that the isolation of anerobic bacteria from the vagina decreased markedly in the tinidazole group compared with the placebo group.

The humoral immune response to lymphocytic choriomeningitis virus (LCMV) was analysed by the use of a sensitive ELISA. A case of a 56-year-old, healthy carrier of Trypanosoma rhodesiense infection was identified in a total village survey for trypanosomiasis. All initial laboratory tests were negative for trypanosomes but two blood-inoculated mice were both positive 11 days after inoculation. The epidemiological significance of human healthy carriers of trypanosomes is discussed.
