Cement Factory Worker Presenting With Raynaud Phenomenon, Breathlessness, and Digital Ulcers.

A 54-year-old man who had worked in a cement factory for the past 30 years, presented to the chest clinic with complaints of insidious onset, gradually progressive breathlessness with intermittent dry cough of three years' duration. The symptoms were associated with bluish discoloration of fingers on exposure to cold. He also gave a history of digital ulcers at the fingertips of the same duration. These ulcers used to heal, leaving behind pitted scars. There was also an associated history of progressive tightening of skin involving the face, extremities, and trunk. He also complained of food g

Failure of the Femoral Component in a Cemented Total Knee Replacement: A Case Report.

CASE: The usual mechanism of failure of a total knee arthroplasty is aseptic loosening of the tibial component. We present a case of an atraumatic late failure by fracture of the femoral component in an active 83-year-old and review the very small number of similar cases within the literature. CONCLUSION: Although it is difficult to draw firm conclusions on such a rare presentation, it appears the heavy-set, active man may be at risk of a femoral component fracture in the long-term. This case also raises an important question about diligence in cementing techniques for the femoral component.

Removing a Retained Antibiotic Cement Nail from a Tibia: A Case Report.

CASE: We present a case of osteomyelitis after a grade 3A open tibial shaft fracture complicated by incomplete removal of an antibiotic intramedullary rod. The authors are unaware of any reports with this specific complication and provide a novel technique for cement mantle removal involving a distal tibial corticotomy and antegrade cement impaction. CONCLUSION: Antibiotic nails can successfully treat intramedullary osteomyelitis, but surgeons may encounter unexpected issues due to custom, intraoperative fabrication. This case describes one example of how to solve the intraoperative problem o

Atraumatic Fracture of Cemented Vanguard Total Knee Arthroplasty Femoral Component.

The authors report the case of an atraumatic femoral component fracture 10 years after primary total knee arthroplasty (TKA) with a modern cemented fixed bearing system. The patient, a 70-year-old man, had the complication without inciting trauma, and he subsequently had severe pain and disability. This rare mode of TKA failure occurred at the superolateral aspect of the femoral component's anterior flange. At the time of revision, no femoral osteolysis was seen and the backside of the prosthesis fracture fragment was found to be free of cement. To the authors' knowledge, this is the first cas

Influence of Selected Surfactants on Physicochemical Properties of Calcium Phosphate Bone Cements.

The influence of the three nonionic surface active agents such as Tween 20, Tween 80, and Tetronic 90R4 on hydrolysis, setting reaction, microstructure, and mechanical properties of alpha tricalcium phosphate (α-TCP) based materials was determined. The study revealed that the addition of any of the surfactants mentioned above slightly prolonged the setting time of the tested cements (up to 5 min). On the other hand, it was found that surfactants influence the long-term hydrolysis reaction. The addition of surfactants also affected the microstructure of the final materials, especially after inc

Circulating Tumour Cell Release after Cement Augmentation of Vertebral Metastases.

Cement augmentation via percutaneous vertebroplasty or kyphoplasty for treatment of spinal metastasis is a well-established treatment option. We assessed whether elevated intrametastatic pressure during cement augmentation results in an increased dissemination of tumour cells into the vascular circulation. We prospectively collected blood from patients with osteolytic spinal column metastases and analysed the prevalence of circulating tumour cells (CTCs) at three time-points: preoperatively, 20 minutes after cement augmentation, and 3-5 days postoperatively. Enrolling 21 patients, including 13

Neuroleptic Malignant Syndrome Following Bilateral Cemented Total Hip Replacements.

Neuroleptic malignant syndrome (NMS) is a rare, serious complication caused by neuroleptic medications. It is characterized by rigidity, hyperthermia, tachycardia, leukocytosis, and an elevated creatine kinase (CK). We present a case of a 50-year-old male who underwent bilateral total hip replacements and subsequently developed NMS. This condition is typically triggered by the sudden introduction, omission, or change in dose of a neuroleptic; in contrast with previous case reports of post-surgical NMS, however, no such trigger was identifiable for our patient. Moreover, this is the first repor

Cemented vs Cementless Total Knee Arthroplasty in Morbidly Obese Patients.

BACKGROUND: Primary total knee arthroplasty (TKA) in morbidly obese patients has been associated with greater postoperative complications. Cementless TKA has the potential for biologic fixation which may provide more durable long-term stability. METHODS: This was a multicenter review of 298 TKAs in 292 morbidly obese patients (body mass index, >40) undergoing TKA, with 154 TKAs (149 patients) in the cemented and 144 TKAs (143 patients) in the cementless group. RESULTS: There were significantly more revisions in the cemented group (n = 20) than in the cementless group (n = 1; 13.0% vs 0.7%).
