Neurorehabilitation across the Continuum: From the Neurocritical care unit to home.

Children admitted to neurocritical care units often experience new neurodevelopmental disabilities due to both their acquired neurologic injuries and deconditioning from prolonged hospitalizations. Rehabilitation for critically ill children is multifactorial and begins in the intensive care unit itself. The goals of rehabilitation include prevention of complications associated with immobilization and evolving tone, comprehensive evaluation and treatment of functional deficits, and implementation of adaptive strategies with the goal of maximizing recovery. As a child progresses along the medica

[Interesting cases in clinical practice concerning Kawasaki syndrome and child abuse].

Typical cases and families are reported to inform the reader about two new or newly recognized conditions of infants and young children, Kawasaki syndrome and child abuse. Although such patients have usually to be cared for in hospitals, it is the responsibility of the general practitioner to suspect the diagnosis in a very early phase and to refer the child immediately. Kawasaki syndrome, also called mucocutaneous lymph node syndrome, is an acute disease of infants and young children with yet unknown cause. The main symptoms are high fever, conjunctivitis, red and dry lips, palmar and plantar

[Clinical Validation of the Caregiver-Child Socioemotional and Relationship Rating Scale (SIRS) for Child Behavior in a Preschool-Age Sample].

Clinical Validation of the Caregiver-Child Socioemotional and Relationship Rating Scale (SIRS) for Child Behavior in a Preschool-Age Sample The description of child behavior in mother-child-interaction is important in early detection and treatment of psychiatric disorders in preschool children. The Caregiver-Child Socioemotional and Relationship Rating Scale (SIRS) may serve this diagnostic purpose. We aim to examine interrater-reliability of SIRS and concurrent, convergent, and discriminant validity to maternal behavior by Play-PAB, and a measure of mother-child-relationsship by Parent-Infant

The Role of Pediatric Early Warning Scores (PEWS) in Determining PICU Transfer Necessity and Length of Stay.

BACKGROUND: Early identification of clinical deterioration in hospitalized children remains a major challenge, particularly in determining the need for Pediatric Intensive Care Unit (PICU) transfer. Pediatric Early Warning Scores (PEWS) have been proposed as a tool to support clinical decision-making. This study aimed to evaluate the association between PEWS and PICU transfer, as well as patient outcomes in hospitalized children. METHODS: This cross-sectional study included all children hospitalized in the wards or referred to the emergency department of Aliasghar Hospital who required a PICU

Analysis of Cases Presenting With Concern of Child Abuse or Neglect to a Child Protection Team.

Limited data are available on cases presenting for triage by child abuse specialists, particularly data combining patient demographics with presenting concerns and outcomes. This study aims to provide a descriptive analysis of cases presenting to one child abuse medical subspecialty team. Data were collected for all referrals triaged by the Child Protection Program, a child abuse team located within an academic children's medical center. The program triaged a total of 928 cases and completed 345 in-person visits with a medical provider. Nearly, half of all provider visits were for evaluation o

[Cardiac arrest in a one-year-old child after an electric shock].

BACKGROUND: Cardiac arrest in toddlers is rare, but accidental electrocution may induce arrhythmias. CASE PRESENTATION: A previously healthy one-year-old child suffered electrocution from an old shoe-warmer and went into cardiac arrest. Her mother started cardiopulmonary resuscitation (CPR) and the ambulance arrived after seven minutes. The first registered rhythm was ventricular tachycardia and the child was defibrillated with an energy dose of 50 joules. One minute after the shock, the child regained spontaneous breathing and was transported to the nearby hospital. The child was unconscious

Notes from the field: Contamination of alcohol prep pads with Bacillus cereus group and Bacillus species--Colorado, 2010.

In October 2010, a child at The Children's Hospital (TCH) in Aurora, Colorado, with newly diagnosed leukemia developed clinical sepsis 24 hours after insertion of an implanted vascular access device. The child also developed extensive cellulitis at the insertion site, requiring surgical debridement, intensive care, antibiotics, prolonged wound management, and outpatient treatment. Cultures of the child's blood and tissue specimens grew Bacillus cereus. An investigation found neither breach of infection control procedures nor any violations of sterile surgical technique.

PERFORMANCE OF SIX PREDICTIVE MODELS OF DEATH OF PATIENTS HOSPITALIZED FOR DECOMPENSATED CIRRHOSIS: A MULTICENTER STUDY.

BACKGROUND: The natural history of cirrhosis is characterized by an asymptomatic phase (compensated cirrhosis) followed by a rapidly progressive phase (decompensated cirrhosis). The ability to predict the survival of patients with cirrhosis is crucial for decision-making, some as complex as the indication for a liver transplant. Several models have been developed and validated. OBJECTIVE: To analyze and compare the performance of models in predicting 90-day mortality among patients hospitalized with decompensated cirrhosis. METHODS: A sample of 481 hospitalized patients, with a mean age of 5

Tigecycline-Based Regimens for Complicated Urinary Tract Infections Caused by Carbapenem-Resistant Gram-Negative Bacteria: Case Series.

There is existing controversy regarding the efficacy of tigecycline (TG) in treating complicated urinary tract infections (cUTIs) because of its pharmacokinetic concerns. We present three patients with cUTIs caused by carbapenem-resistant gram-negative (GN) pathogens successfully treated with high-dose tigecycline (HDT)-based regimens, as cefiderocol and aztreonam were not available in our country. The first case describes a 67-year-old patient with diabetes, prostate cancer, and double J ureteral stenting who was hospitalized with a febrile, complicated urinary tract infection (cUTI). Urine a

Severity assessment in children hospitalized with bronchiolitis using the pediatric component of the Comprehensive Severity Index.

OBJECTIVE: Practice variation in the management of children hospitalized with bronchiolitis may result in significant differences in resource utilization. Determination of cost-effective care requires an objective means of adjusting for severity. We examined the correlation of the pediatric component of the Comprehensive Severity Index (CSI) with resource utilization in children hospitalized with bronchiolitis at ten children's medical centers. DESIGN: Demographics, clinical findings, laboratory results, interventions, and outcomes were retrospectively extracted from the charts of 804 consecu

Issues related to human immunodeficiency virus transmission in schools, child care, medical settings, the home, and community. American Academy of Pediatrics. Committee of Pediatric AIDS and Committee on Infectious Diseases.

Current recommendations of the American Academy of Pediatrics (AAP) for infection control practices to prevent transmission of blood-borne pathogens, including human immunodeficiency virus (HIV) in hospitals, other medical settings, schools, and child care facilities, are reviewed and explained. Hand-washing is essential, whether or not gloves are used, and gloves should be used when contact with blood or blood-containing body fluids may occur. In hospitalized children, the 1996 recommendations of the Centers for Disease Control and Prevention (CDC) should be implemented as modified in the 199

A French hospital sentenced for unreasonable obstinacy.

On 2 June 2009, the Nimes administrative court condemned the Hospital of Orange (France) for unreasonable obstinacy after neonatal resuscitation. On 14 December 2002, an apparently stillborn child was resuscitated after approximately 30 minutes of foetal distress. Cardiac activity was recovered, but the child has since suffered from severe disabilities. The court did not find any fault committed by the hospital regarding maternal care. However, the hospital was sentenced to compensate for the injuries caused by unreasonable obstinacy. According to the court, the medical team should have taken

Unusual case of leptomeningeal dissemination of a diffuse pontine high-grade astrocytoma in a child.

A 12-year-old girl was seen in our hospital and diagnosed with diffuse pontine glioma. Treatment with radiotherapy was suggested, but the parents rejected this plan and took the child to another hospital where partial resection of the tumor was performed. The histological diagnosis was anaplastic astrocytoma. Returning to our hospital, the parents agreed to radiotherapy which was given in a total dose of 54 Gy. However, 3 months after completion of radiotherapy, the child developed widespread leptomeningeal dissemination of her disease and died 12 months after the initial diagnosis. Since no e
